帽子는 추위나 더위로부터 머리를 보호하고자, 또는 장식적인 목적으로나 사회적 지위의 상징으로 착용되어 온 것으로 冠帽의 형태는 먼저 자연 그대로의 圭襲牀態의 머리를 어떻게 처리하느냐 하는 것에 좌우되었을 것이다. 한국 민족은 북방계 민족으로 遊牧生活을 영위하던 이른바 북방계 유목민이었으니, 이들 북방계 유목민들이 한반도와 滿州에 와서 유목생활을 하던 石器時代부터 이미 冠帽를 착용했으리라 생각된다. 이러한 관모의 始源形은 先史時代의 수행생활 내지는 농경생활과 관련이 되었을 것으로 추축되며, 머리가 흘러 내려오는 것을 감싸는 用具로 출발한 것 같다. 이러한 형태의 것으로 「巾」이 있는데, 이것이 한국 冠帽의 原始形態이면서 基本形態가 아닌가 한다.

스팀소독 멸균품의 유효기간에 관한 연구 : 포장재료, 보관환경을중심으로

The purpose of this study is to provide standards for steam sterilization products used in operating room by investigating validity periods of the products. The study was performed from July 5 to November 8, 1999. Packing materials for the study included new muslin, recycled muslin, new water-proof material, recycled water-proof material, new craft paper and gauze, nelatone catheter(rubber), k-wire(stainless), suture silk were packed by above mentioned packing materials. Open and closed areas of operating preparation rooms and those of operation rooms were randomly designated as the storage environments for the subject materials. The temperatures and humidities of those areas were not controlled. The cultivation test as a method of microbiological measurement was used. The cultivation results were recorded along with cultivation dates and storage environments. Based on the record, validity periods of the products were determined. Results of the study can be summarized as follows. 1) It was found that there were no germs generated in the product samples packed by new muslin, recycled muslin, new water-proof material, recycled water-proof material and a new craft paper. 2) It was found that there were no germs generated in the product samples stored in open and closed areas of both operating preparation and operation rooms during the investigation. Based on these results, it is thought that validity periods of steam sterilization products should be determined considering contamination incidents, as suggested by AORN(1997) and AAMI(1993) rather than considering conventional practices with no certain appropriate guideline. In doing so, a thorough infection control for preventing wound infection, reduced costs of sterilization product management and effective nursing practices can be expected.

DRG 지불제도 적용수술에 투입되는 재료비 산출 및 분석에 관한 연구

Purpose: This research analyzed the material cost of the operation adapted to DRG payment system. Method: This research was performed from April. 1, 2003 to Oct. 27, 2003 for 342 patients. We analyzed the output of operation material cost and the resulting factor of the difference by the research table about consumed materials. The 8 groups of operation that adapted in DRG system were divided into 12 lists and 3 factors ; disease-dependent factor, operator-dependent factor, and staff-dependent factor which induced the cost difference. Result: The most expensive operation was cataract surgery(\482,746) and the cheapest surgery was tonsillectomy(\7,217). Cesarean sections represented the greatest difference between the highest and the lowest cost, about \152,177, and tonsillectomy represented the smallest difference, about \3,128. The factors contributing to the difference were the operator-dependent factor(46.7%), the disease-dependent factor(37.5%), and the staff-dependent factor(15.8%). Conclusion: It is expected that the material cost of the operation can be reduced when all operator change their cognition about DRG payment system and voluntarily participate to reduce the cost of material.
