Effect of humeral anterior-posterior translation on shoulder impingement syndrome

Shoulder impingement syndrome (SIS) is related with glenohumeral translation, however anterior-posterior (AP) humeral translation has been overlooked compared to superior-inferior(SI) humeral translation. The purpose of this study is to investigate how humeral AP translation influences SIS. A 3D dynamic subject-specific musculoskeletal model of the shoulder was developed from MR images. Intact model, a rotator cuff tear model (without supraspinatus and infraspinatus) and another tear model with limited humeral posterior translation by imaginary force were developed. The shoulder elevation was simulated for each model and the range of motion (ROM) and impingement were measured. The intact model showed 86°of ROM without impingement. The rotator cuff tear model only elevated less than 50°and subacromial impingement appeared to prevent further humeral motion. However, the limited humeral posterior translation model showed full ROM recovered without impingement. This results suggests that shoulder impingement after rotator cuff tear is influenced by AP translation in addition to SI translation and the humeral AP translation needs further consideration for rotator cuff tear repair in the future.

이 글은 지속가능한 기술을 위한 정책적 도구로서 전략적 니치관리(Strategic Niche Management)를 개괄하고, 그 효용과 한계를 점검하고 있다. 전통적인 접근인 시장실패론적 접근과는 달리 전략적 니치관리의 접근은 시스템적 접근으로서, 다층적 관점(Multi Level Perspective)를 통해 기술혁신과 변화를 재분석해내고, 고착화된 사회/기술시스템의 전환을 이룩해내기 위한 방법론이다. 전략적 니치관리의 이론적, 경험적 미성숙에도 불구하고, 전략적 니치관리는 기술혁신과 발전에 대한 분석틀로서 뿐만 아니라, 급진적 혁신을 만들어 낼 수 있는 도구로서의 가치가 주목된다. 노령화 사회에 대비한 혁신정책 및 지속가능한 에너지 시스템 구축을 위한 혁신정책, 위험사회에 대응한 혁신정책들이 새로운 혁신정책의 영역으로 부상하고 있는 상황에서 전략적 니치관리는 이들 분야에서 중요한 분석 및 실천도구로 활용되며 새로운 영역을 개척할 수 있을 것으로 기대된다.

The purpose of this study is to suggest novel surgical technique for irreparable rotator cuff tear patients; superior pylon reconstruction (SPR) by comparing its surgical effect with superior capsular reconstruction (SCR) by a simulation study. We constructed total 4 models; intact, tear, SCR and SPR models based on MRI. Total 10 male subjects (age, 25±4 year old; weight, 66±10 kg; height, 170±5cm) implemented shoulder abduction in scapular plane. Range of motion (ROM), acromio humeral distance (AHD), and superior-inferior (SI) translation of humerus of SPR and SCR models were compared. There’s no significant difference in ROM between two models, however in AHD, SPR model increased by 0.4 mm (p = 0.001) compared with SCR model. SPR models showed decreased maximum superior translation of humerus by 1.0 mm (p = 0.001) compared with SCR model. These results implied that SPR could restore shoulder joint kinematics of massive irreparable rotator cuff tear patients as effectively as SCR.

전상향 파열 환자가 재활운동으로 가동범위를 회복하지 못하는 이유

Rotator cuff refers group of muscles surrounding shoulder joint which are supraspinatus, infraspinatus, subscapularis and teres minor. Tear of these muscles results in abnormal kinematics of shoulder joint. Previous study reported anterosuperior tear patients did not recover range of motion (ROM) despite rehabilitation. Purpose of this study is to find out why rehabilitation failed on anterosuperior tear patients. Two types of shoulder models were constructed: intact and anterosuperior (supraspinatus and infraspinatus) tear model. Anterosuperior tear model’s elevation ROM was observed in 3 different conditions: enhancing deltoids, teres minor and both muscles. Results showed that enhancing only teres minor increased ROM. However, enhancing only deltoids and enhancing both deltoids and teres minor did not, and impingement occurred. These results implied strengthening deltoids and teres minor in equal ratio could not be appropriate rehabilitation for anterosuperior tear patients.
