        Doppler 심 초음파에 의한 폐 동맥 혈류 측정에 있어서 관찰 View 에 따른 비교

        N/A The analysis of pulmonary blood flow by Doppler echocardiography is very useful in evaluating various cardiac patients with pulmonary hypertension. How- ever, determinations of pulmonary arterial blood flow can vary markedly when measured from different sites. One hundred subjects (90 normal subjects, 10 C.O.P.D. patients) underwent Doppler and two-dimensional echocardiographic examination of the pulmonary artery from multiple views to determine the variability in the magnitude of Doppler determined peak velocity, acceleration time of pulmonary arterial blood flow and pulmconary arterial diameter. The results were as follows: 1) The maximun diameter of the pulmonary artery was 2.0±0.5cm (mean±S.D) and was measured from the parasternal right ventricular outflow tract (PRVOT) view (78), the subcostal short axis (SCSAX) view (65) and the parasternal short axis (PSAX) view (51) in turn. 2) The peak. velocity of pulmonary blood flow was mostly detected from the PRVOT view (59%) and sometimes from the PSAX view (17%). 3) The highest flow velocity of the pulmonary artery was recorded from the SCSAX view in 24 patients including 8 C.O,P,D patients. 4) Our results indicate that the PRVOT view was most useful in evaluating the image and blood folw pattern of the pulmonary artery, and in some cases (such as C.O.P,D), the SCSAX view was much better than any other view.

        The importance of senior welfare centers is highlighted to solve the aging problem in modern society, but universal design considering the characteristics of the elderly is insufficient. The purpose of this study is to suggest the direction of improvement of the community space of the elderly welfare center considering universal design based on the characteristics of the elderly. Through prior research, community spaces in senior welfare centers were classified into three categories. Based on previous studies and universal design guidelines, the framework of the analysis was extracted into space, furniture and storage, color and finishing materials, door and window, lighting, and signatures. On-site surveys were conducted at three general welfare centers for the elderly in Seoul, and the results of the study are as follows. First, a warm atmosphere was formed in the educational space by using wide arrangement between households and wood-toned finishes, but there were cases in which a facility atmosphere was formed by the paint finish. In addition, in some cases, the number of windows was small or the structure was placed in front of the window, making ventilation difficult. Second, although the hobby and entertainment space consisted of button-type automatic doors or light doors, there were cases where it was difficult for the elderly to enter alone by using heavy glass doors. In addition, lighting with high visual fatigue was installed, which was not suitable for the elderly. Third, the sports space did not use furniture with rounded corners, and there was no space for storing mobility aids. In addition, no guide signs were provided, and the size of real-name letters in each space is small, making it difficult for the elderly to recognize them. In future studies, it is expected that the environment of the elderly welfare center will be improved by expanding the scope not only in Seoul but also on a national scale.

      • Enalapril 경구 투여가 혈중 Angiotensin 전환효소 활성도에 미치는 영향

        Enalapril, a angiotensin converting enzyme(ACE) inhibitor, was given in a single oral dose of 10mg to 13 patients with essential hypertension and to 10 control patients with normal blood pressure. Blood pressure, plasma renin activity, aldosteron, and SCE activity were measured before and after enalapril administration. The results were as follow : 1. At 3 hours. after administration of enalapril, plasma ACE activity decreased in both groups of patients(p<0.05) but more decreased in patients with essential hypertension from 19.4±8.8 to 9.7±5.7 nmole/min/ml(p<0.05). 2. Decresed levels of plasma ACE activity were accompanied by reduction of blood pressure in response to enalapril, especially systolic blood pressure in patients with hypertension. Blood pressure also decreased in both groups of patients including those with normal or low renin levels(p<0.05), but there was not a significant correlation between the magnitude of blood pressure reduction and baseline plasma renin activity. 3. After 10mg of enalapril administration, mean plasma renin activity rose(p<0.05), while plasma aldosterone concentration fell in both groups of patients. It suggest that enalapril is an effective ACE inhibitor and the measurement of plasma ACE activity is useful for monitoring the efficacy of ACE inhibitor and compliance to treatment with enalapril.
