        보건(지)소 단위의 1차진료사업 정보체계개발

        공공의료부문의 1차진료사업은 민간부문에서 다루기 어려운 계속적이고 포괄적인 가족중심의 예방서비스를 담당하여야 하며 기능강화로 국민의료비 절감효과를 가져올 수 있다. 이를 위해서 보건소, 보건지소 단위의 1차진료사업은 지역주민 전체를 사업의 대상으로 하며, 또한 현재 보건사업으로 추진하고 있는 결핵, 성병, 나병, 그리고 중요 쟁점으로 떠오르고 있는 기타 만성질환 등과도 연계되어 추진되어야 한다. 이와 같은 1차진료사업의 포괄적인 실시를 위해 가족단위의 지역주민에 대한 기초자료 구축, 타 보건사업과의 정보활용, 통계자료 산출 등을 위한 정보체계 구축이 필요하다. 내용으로는 사용서의 표준화, 진료기록부 작성체계 개선, 가족단위의 필요정보 마련으로 사업전개방식 변화, 만성질환 관리사업 등과의 연계방안 마련, 진료비 청구업무 전산화 등이 있다. The purposes of the primary clinic service in the public health sector are to provide the preventive health care service on the basis of the family unit consistently and comprehensively. It is difficult to provide this service through private health institution. This should effect the reduction of the national health expenditure by strenthening the function of the primary clinic service. To achieve these purposes, the primary clinic service in health center and health subcenter should target all residents of a community, and be managed with tuberculosis control, venereal disease control, leprosy control, and other chronic disease controls. For these tasks of health care service, the information system of the residents of a community is necessary, to build the database for them, to utilize the information of other health services, and to produce the statistics. The plans to implement the information system are to standardize the form for primary clinic service, to make up a record form for the family unit and provide the services on the basis of the family unit, to utilize information on chronic disease control and refer it to the treatment of patient, and to computerize the work related to medical care fees.

        일 종합병원 협업공간환경의 수준과 인지된 지원성이 간호사의 협업효과성에 미치는 영향

        연구배경: 본 연구는 일 종합병원 간호사의 협업을 위한 공간환경의 수준 및 인지된 지원성이 협업 효과성에 미치는 영향관계를 연구하고자 하였다. 연구방법: 협업공간의 수준은 업무공간 및 평면배치, 협업환경의 지원성은 편리성 및 산만성, 협업 효과성 등의 측정은 리커트 5점 척도로 실시하였다. 자료수집은 2014년 4월14일에서 21일까지 서울시 소재 450병상 이상인 일개 종합병원에 근무하는 간호사들을 대상으로 하였다. 사전검증을 통하여 수정 및 보완한 설문지 250부를 배포하였으며, 총 237명으로부터 자료를 회수하여 SAS 9.3 version으로 분석하였다. 연구결과: 종합병원 간호사의 협업공간 수준에서 업무공간 2.61, 평면배치 2.90, 협업환경 지원성은 편리성 2.72, 산만성 3.2, 협업 효과성 3.19 등 이었다. 협업공간 수준의 하부요인인 업무공간과 평면배치는 종합병원 간호사의 협업 효과성에 유의한 영향요인으로 확인되었다. 협업환경 지원성의 하부요인인 편리성과 산만성은 업무공간과 평면배치와 상관성이 존재하여 협업 효과성을 유의하게 추가적인 설명을 거의 하지 못하였다. 결론: 종합병원의 협업을 위한 업무공간과 평면배치는 간호사의 협업 효과성에 유의한 영향요인으로 확인된 바, 종합병원 간호사들의 협업 효과성 향상을 위하여 협업환경의 조성 및 활용, 그리고 이에 대한 적절한 평가 및 관리가 지속적으로 이루어져야 할 것이다. Background: This study aims to investigate the level and perceived support of workplace environment (workplace spatial settings) to collaboration effectiveness of nurses in a general hospital. Methods: Measurements of the variables were developed using a 5-point Likert scale: workplace spatial settings (spatial and layout), supportiveness of collaboration environment (facilitation and distraction), and collaboration effectiveness. Data collection was done from April 14 to 21, 2014, with nurses working at a general hospital with more than 450 beds in Seoul. A total of 237 cases were collected by using questionnaires, and analyzed with SAS 9.3 version. Result: The level of a general hospital nurses’ collaboration environment(workplace spatial settings) was shown to be 2.61 for spatial variables and 2.90 for layout variables, and the perceived support of collaboration environment was 2.72 for facilitation and 3.2 for distraction. and the collaboration effectiveness was 3.19. Spatial and layout of workplace spatial settings were statistically significant to explain the collaboration effectiveness of nurses in a general hospital. Facilitation and distractions of collaboration environment support were not significant to explain the collaboration effectiveness because of their correlations with workplace spatial settings(level of spatial settings and layout). Conclusion: Workplace arrangement and spatial settings for collaboration of nurses in a general hospital are confirmed to be significant factors to the collaboration effectiveness of them. Design and utilization of the collaboration environment(workplace spatial settings) should be designed, managed and assessed in order to improve the effectiveness of collaborative work of nurses in general hospital.

        종합병원 임상간호사의 직무배태성과 상사-구성원 교환관계가 조직몰입에 미치는 영향

        Objectives: The study was aimed to investigate the effects of job embeddedness and leader-member exchange on organizational commitment of clinical nurses. Methods: Data were collected by self-administered questionnaire from 9th to 13th March, 2015. Respondents were 250 clinical nurses working in a tertiary university-based hospitals in Seoul, Korea. We analyzed 245 subjects using SPSS 21.0 version program. Results: Job embeddedness and leader-member exchange were significantly influenced to organizational commitment of clinical nurses. Sacrifice, subfactor of job embeddedness, was significantly positive effect on subfactors of organizational commitment such as identification, attachment, and continuous service. Contribution, subfactor of leader-member exchange, was significantly positive effect on identification. Loyalty, subfactor of leader-member exchange, was significantly positive effect on continuous service. Conclusions: Hospital management strategy to improve nurses’ job embeddedness and leader-member exchange could be effective to enhance organizational commitment of clinical nurses.

        배우자와의 나이차이와 건강상태 및 의료이용의 관련성

        연구목적 : 본 연구는 배우자와의 나이차이와 건강상태 및 의료이용의 관련성을 파악하고자 하였다. 연구방법 : 자료는 한국의료패널 연간데이터 중 2012년 데이터를 활용하였으며, 배우자자료가 있는 40세 이상의 응답자 6,910명을 분석대상으로 선정하였다. 배우자와의 나이차이에 따라 건강상태(주관적 건강상태, 동년배 비교 건강상태, 불안/우울)와 의료이용(외래이용횟수, 재원일수)의 관련성을 파악하였다. 연구결과 : 남자(여자)는 배우자보다 평균 3.7세(3.4세) 많았으며(적었으며), 남자와 여자 모두 3세 차이가 가장 많은 빈도(최빈값)를 차지하였으며, 배우자보다 1세~5세 많은 남자(1세~5세 적은 여자)의 경우가 각각 59% 수준이었다. 남자의 경우, 배우자와의 나이차이는 주관적 건강상태 및 동년배 비교 건강상태와 모두 약한 음의 상관성이 있었으며, 불안/우울, 외래방문수, 재원일수 등과는 약한 양의 상관성이 있었다. 여자의 경우, 배우자와의 나이차이는 자신의 주관적 건강수준 및 동년배 비교 건강상태와 약한 양의 상관성이 있었으나, 불안/우울과는 음의 상관성이 있었으나, 유의하지 않았다. 결론 : 배우자와의 나이차이에 대한 기술통계, 그리고 배우자의 나이차이와 건강상태 및 의료이용의 상관성은 건강을 고려한 배우자 선택과 보건의료정책에 활용성이 기대된다. 그러나, 연령대, 생활습관, 배우자의 지지, 혼인기간, 혼인상태 등을 고려한 추가연구를 통하여 보다 유용한 의미를 도출하여야 할 것이다. Objective: The purpose of this study was to investigate the relationships of age gap between partners with health status and utilization of health service. Methods: We used the data of year 2012 in the annual Korea Health Panel administered by joint supervision of Korea Institute for Health and Social Affairs and National Health Insurance Corporation. We analyzed data for 6,910 individuals that had a spouse and were above 40 years old. Results: Man(woman)`s average age gap with partner was 3.7(-3.4) years, and mode of man`s(woman`s) age gap with partner was 3(-3) years. For men, age gap between partners had weak positive correlations with frequency of outpatient service utilization, anxiety and depressive symptoms, and length of hospital stay, and weak negative correlations with self-rated health status. For women, age gap between partners had weak positive correlations with self-rated health status, and weak negative correlations with frequency of outpatient service use. Conclusion: The relationships of age gap between partners with health status and health service utilization have implications to culture of marriage and health policy on family life. Further research on the mediating factors with the relationships are needed.

        재미있는 작업환경이 병원 간호사의 직무만족 및 작업열의에 미치는 영향

        Objectives: Culture and climate of hospital would be an important factor on quality of services in these competitive healthcare industry. This study aimed to investigate the effects of workplace fun to job satisfaction and work engagement of hospital nurses. Methods: Data were collected by self-administered questionnaire from 3rd to 9th May, 2013. Respondents were 196 nurses working in four hospitals in Seoul and Gyeonggi-do province. Four dimensions pf workplance fun, such as The effects of workplace fun on job satisfaction and work engagement of nurses were tested through multiple linear regression analysis by controlling population and social variables. Results: Global fun at work and personal freedoms, dimensions of workplace fun, were significantly influenced to satisfaction with rewards of hospital nurses. Global fun at work and socializing with coworkers were significantly influenced to satisfaction with hospital nurses` coworkers and superiors. Global fun at work, personal freedoms, and socializing with coworkers were significantly influenced to work engagement of hospital nurses. Global fun at work was the most influential to organizational effectivieness among four dimensions of workplace fun. Conclusions: Workplace fun in general hospital revealed as effective factor to job satisfaction and work engagement of hospital nurses. Sub-factors of workplace fun should be used cautiously except global fun at work. We recommend to hospital managers to build fun culture and climate to nurses in general hospitals.

        종합병원의 60세 이상 입원환자의 요양병원 이용의향 관련 요인

        Objectives: The purpose of this study was to investigate factors that have effects on the intention of use long-term care (LTC) hospital in inpatients over sixty. Methods: Data were surveyed from over 60 years inpatients at two hospitals located in Seoul and Gyeonggi-do province during February and March, 2015. Predisposing factors, needs and enabling factors by Andersen`s behavioral model to medical were mapped into measure items. Total 180 cases were analyzed. Results: Age as predisposing factor of intention to use of LTC hospital was significant. Self-rated health status as need factor of intention to use of LTC hospital was not significant. Enabling factors of intention to use of LTC hospital, such as residence type, and awareness and perception of LTC facilities or hospital, were significant. The sixties group have more intention to use LTC hospital than over 70. The patient group being aware of LTC facilities had more intention to use LTC hospital than the not-being aware of them. The patient group that has indirect experience of LTC hospital had more intention to use LTC hospital than the group that does not. The patient group of self-owned residence showed less intention of using LTC hospital than the group of monthly-rent residence. Conclusions: Intention to use LTC hospital of the elderly over sixty was different in their age, residence type, awareness and indirect experience of LTC service/facilities. Therefore, this results can be applied to LTC hospital management and government`s LTC policy promotion.

        종합병원 간병인의 직무몰입에 대한 영향요인: 직무스트레스, 수면의 질, 의사소통 능력

        Objectives: This study aimed to investigate the influencing factors to job involvement of caregivers in general hospital such as job stress, sleep quality, and communication competence in their workplace. Methods: Data were collected by self-administered questionnaire survey from March 2 to 20, 2015. Respondents were 182 caregivers working at four hospitals located in Seoul and Gyeonggi-do province. Job stress, sleep quality, communication competence and job involvement of caregiver were measured using Likert scale. Data were analyzed using SPSS 21.0. Results: Concentration in communication, recovery after sleep, interpersonal job stress, and health level were identified as influencing factors to the job involvement of caregivers in general hospital. Affective job involvement was explained positively by concentration in communication, recovery after sleep, and health level, and negatively by interpersonal job stress. Normative job involvement was explained positively by concentration in communication, and recovery after sleep. Conclusions: Work-based programs and policies for caregivers in hospital should be developed and implemented to improve the job stress, sleep quality, and communication competence of them.

        병원간호사의 전문직업관 및 조직몰입이 간호업무수행에 미치는 영향

        Objectives: This study investigated the effects of professional consciousness and organizational commitment on nursing performance of hospital nurses. Methods: Data were collected using a structured self-administered questionnaires (5-point scale) from nurses working in three hospitals, over 500 beds in Gyeonggi-do. We analyzed 305 cases using SPSS 21.0. Results: Professional consciousness and organizational commitment were significantly correlated with nursing performance. Professional consciousness of hospital nurses was a positive influencing factor to nursing performance when controlling socio-economic characteristics. Organizational commitment was not significantly influenced to nursing performance because it was significantly correlated with professional consciousness. Among socio-economic factors, working area was influencing factor to nursing performance that nurses working in nursing and care-giving integrated ward were more positively influenced to that than those working in general care ward. Conclusions: Education and training program planning for enhancing professional consciousness of hospital nurses would contribute to improve quality of nursing and nursing performance. Motivation and extended enforcement is needed to improve nursing performance for nurses working in nursing and care-giving integrated services ward.
