Tuberculosis rarely involves the esophagus, and most esophageal tuberculosis occurs secondary to adjacent tuberculous lymphadenitis. An esophago-mediastinal fistula is a very unusual complication of tuberculous lymphadenitis. Herein, we report a case of an esophago-mediastinal fistula due to tuberculous lymphadenitis. A 28-year-old woman who had dysphagia was assessed by chest computed tomography, endoscopy, esophagogram, and a lymph node biopsy. An esophago-mediastinal fistula was found and an antituberculous agent was considered initially. However, because of her severe dysphagia, she managed with endoscopic clipping as an alternative. However, the fistula remained on follow-up esophagography. A gastrostomy was eventually performed, and she was treated with an antituberculous agent. The fistula had closed after 4 weeks of medication. Antituberculous agents are the mainstay treatment for esophago-mediastinal fistula due to tuberculous lymphadenitis; endoscopic clipping may be a complementary treatment. 침향추출물 (Aquilaria Malaccensis Agarwood)의 지표 성분 분석 및 피부 효능에 대한 연구

Stem wood of Aquilaria Malaccensis agarwood produced at Indonesia was extracted using ethanol and fractionated with hot water and 1.3 butylene glycol for the purpose of applying to cosmetic products. GC-MS and LC-MS techniques were applied to analyze the marker components contained in Aquilaria extract. In the GC-MS, α, β, γ-Eudesmol which belong to triterpenes were primarily identified, while Genkwanin which was one of flavonoids was assayed in the LC-MS. In-vitro and in-vivo tests on anti-oxidation, anti-aging, melanin content and anti-inflammation exhibited Aquilaria extract delayed the oxidation process, and promoted collagen synthesis, and suppressed melanin synthesis as well as inhibited an expression of NO (Nitric Oxide) which was one of proinflammatory mediators to skin. It was expected that Aquilaria extract might contribute in enhancing the skin efficacies as active components with functional markers.
