심음을 이용한 SVM 기반의 심장 질환 판별에 관한 연구

심음은 심장이 수축, 확장 시에 심장의 움직임과 혈류의 흐름에 의해 발생하는 음향이다. 심음은 여러 신호원으로 이루어져 있고, 매우 복잡하고 비고정적인 신호이다. 심장의 질환에 따라 심음의 소리는 다르게 나타난다. 심음을 구분하여 심장 질환의 유무를 판단하는 가장 기초적인 기준이 될 수 있다. 본 연구에서는 Support Vector Machine 기법을 이용하여 심음을 통한 심장 질환 판별 검출 알고리즘을 제안하였다. Support Vector Machine은 신경망의 한 종류이며 이진분류에서 좋은 성능을 보인다. 또한 Polynomial Radial Basis Function, Multi-Layer Perceptron Classifiers를 위한 대안적인 학습방법으로 사용된다. 이러한 특성을 사용하여 심음의 데이터들을 일정한 기준에 의하여 (+)데이터와 (-)데이터로 분리한 후, 각 데이터들을 학습시켜 최적의 데이터를 만든다. 이후 각 데이터들은 점층적인 추가 학습을 시킴으로써 적은 양의 학습 데이터만으로도 높은 분류 성능을 표현할 수 있다. 이 연구에서 제안된 SVM을 실제 심음 데이터에 적용한 실험에서 심장 질환의 유무 판별에 우수한 성능을 보임을 확인할 수 있을 것으로 판단된다.

Oral lichen planus (OLP) is a relatively common chronic inflammatory disease of uncertain etiology. OLP lesions rarely undergo spontaneous remission and have the potential to undergo malignant transformation. The risk of malignant transformation in OLP is estimated to be between 0.4% and 5.3%; two clinical studies in Korea reported the frequency of malignant change as 2.9% and 2.85%. However, to the best of our knowledge, there has been no individual case report in the Korean dermatological literature to date regarding the malignant transformation of OLP. Herein, we report the case of a 77-year-old Korean woman with long-standing OLP that underwent transformation to oral squamous cell carcinoma. This is the first well-documented case report of squamous cell carcinoma that developed from OLP. (Korean J Dermatol 2015;53(8):635∼637)
두피 건선 환자의 삶의 질에 미치는 국소 스테로이드제 단독 요법과 경구 사이클로스포린 병합 요법의 효과 비교

Background: Scalp psoriasis severely affects patients’ quality of life, and patients may occasionally be refractory to topical steroid application. Although systemic therapy combined with topical steroid application is recommended for treatment-resistant scalp psoriasis, previous research has not reported whether such combination treatment improves patients’ quality of life. Objective: This study investigated whether combination therapy using oral cyclosporine and topical steroid reduces the severity of scalp psoriasis and improves patients’ quality of life compared with the use of topical steroid alone. Methods: The study included 23 patients with scalp psoriasis who were categorized as patients who received topical steroid therapy alone vs. those who received combination therapy with topical steroid and oral cyclosporine for 12 weeks. The severity of psoriasis and quality of life were evaluated at 4, 8, and 12 weeks of treatment. Results: No significant improvement was observed in the severity of psoriasis or in patients’ quality of life during the treatment period in patients receiving topical steroid alone. Patients treated with a combination of topical steroid and oral cyclosporine showed a reduction in the severity of psoriasis from the 4th week of treatment and their quality of life improved from the 12th week of treatment. Conclusion: Combination therapy with oral cyclosporine and topical steroid may reduce the severity of scalp psoriasis and improve patients’ quality of life. (Korean J Dermatol 2019;57(9):513∼518)
피부과 환자에서 저용량 요법으로 투여된 사이클로스포린의 누적 용량과 신장기능 이상 사이의 연관성에 대한 고찰

Background: Cyclosporine (CsA), an immunosuppressant that specifically regulates T-lymphocytes, has long been used in dermatology practice. However, nephrotoxicity, a well-known adverse effect associated with high-dose (≥5 mg/kg/d) and long-term administration (≥2 years) has limited the use of CsA. Objective: We investigated the association between the cumulative dose of CsA and renal dysfunction, as well as the long-term effects of low-dose CsA on renal function in patients who presented with dermatological conditions. Methods: The study included 697 patients who received CsA at an outpatient dermatology clinic between January 2015 and May 2019. Logistic regression analysis was performed to determine the association between the cumulative dose of CsA and renal dysfunction. Results: Compared with patients who received the lowest cumulative dose (<7,000 mg), those who received the highest cumulative dose of CsA (≥30,600 mg) showed a 64% higher risk of renal dysfunction; however, the difference was not statistically significant (odds ratio 1.64, 95% confidence interval 0.39∼6.84). Conclusion: Among patients who receive a low-dose CsA regimen, those who are treated with cumulative doses higher than the critical value may be predisposed to renal dysfunction, whereas those treated with a cumulative dose lower than the critical value are unlikely to develop nephrotoxicity. Carbon dioxide (CO2) laser is one of the most widely used lasers in dermatology. CO2 laser can precisely ablate the skin with good hemostasis, which makes it an ideal surgical laser. With the development of pulsed CO2 laser, which enabled cleaner ablation with less residual thermal damage, CO2 laser resurfacing became the gold standard for skin rejuvenation. The rejuvenation effect of full-face CO2 laser resurfacing is unparalleled to other methods; however, it is associated with a relative high incidence of side effects and prolonged downtime. Fractional CO2 laser was developed to overcome this problem. With a new technology of fractionating the laser beam into hundreds of microbeams, fractional CO2 laser resurfacing can improve various skin conditions including skin laxity, photodamage, and acne scars with short downtime and significantly lesser side effects. For optimal treatment results, clinicians have to decide the treatment depth and density based on proper understanding of the laser tissue interaction and underlying skin conditions. SVM을 기반으로 한 심음 기반의 심장 질환 판별에 관한 연구

In this paper, Support Vector Machine Algorithms were used with promising results in various critical problems, concerning heart sound classification. In general this classification problem can be divided in many sub problems, each one dealing either with one morphological characteristic of the heart sound or with difficult to distinguish heart diseases. We proposed a ventricular fibrillation detection algorithm based on support vector machine classifier and Short Time Vector Machine, which could offer benefits to reduce the learning costs as well as good classification performance. It showed similar or higher values. Consequently, we could find that the proposed input features and SVM classifier would one of the most useful algorithm for cardiac disorder detection.
