Background: Insulin resistance has been reported as one of major factors in the development of NIDDM. Serum androgen levels are known to be associated with insulin sensitivity. One study suggests that adrenal androgen or sex steroid hormane is related to controlling the insulin sensitivity of glycogen synthesis system. We investigated whether serum sex hormone and SHBG(Sex hormone binding globulm) levels are associated in the metabolic index of patients with NIDDM. Methods: We radioimmunoassyed serum total and hee testosterone, es and SHBG in the blood from 41 female patients with NIDDM and 40 female normal subjects, and 25 male patients with NlDDM and 16 male normal subjects. Results: In female pateints with NIDDM, the serum SHBG levels were significantly lower than that of the normal female subjects(46.5+-23.4nmol/L vs 76.5+-37.5nmol/L, p$lt;0.05), and the free testosterone levels were sigmficantly higher compared to that of the normal female subjects(1.59+-1.67ng/dL vs 0.57+-0.15ng/dL, p$lt;0.05). In male group, either the SHBG levels or the free testosterone levels did not differ between patients with NIDDM and the normal subjects. Conclusion: Our data showed hyperandrogenecity in female patients with NIDDM compared to female normal subjects which suggests the relationship between the development of NIDDM and androgenecity in women.
