GABA 동작성 신경이 악안면 영역의 유해자극 반응에 미치는 영향

It was reported that the periaqueductal gray matter was related with descending analgesia of orofacial region. This experiment was performed to study the effect of GABAergic drugs in descending analgesic mechanisms from periaqueductal gray matter. Sprague-Dawley rats were divided into five groups as saline, muscimol(45ng, Sigma), bicuculline(15ng, Sigma), muscimol following bicuculline and baclofen(0.5㎍, Sigma) injected group. The experimental animals were anesthetized and a pair of noxious stimulating electrode were inserted into palatal mucosa. Anterior belly of digastric muscle was exposed and a pair of electrode were inserted into muscle to record the dEMG. The threshold intensity of noxious stimulus which induced the minimal dEMG activity was established and palatal mucosa was stimulated with twice a threshold value, 100μsec duration, 166 sec interval. In each groups, dEMG were recorded for two hours at 15 minutes interval after the drug was microinjected into periaqueductal gray matter. The dEMGs were a little decreased after bicuculline was injected, but immediately recovered. The dEMGs were decreased after muscimol was injected and this analgesic effect of muscimol was blocked by bicuculline. The analgesic effect were also observed after baclofen injection.

This study is based on 247 patients(348 cases) with facial bone fracture who were admitted to the department of oral and maxillofacial surgery, Kangnam sacred heart Hospital during the period of Jan. 1983 through sep. 1988. The patients with mandible fracture were analysed with the following items such as complications with treatment method undertaken : Bicortical osteosynthesis vs Monocortical osteosynthesis, advantages and Disavantages. The results obtained are as follows : 1) The most common age groups for the facial bone fracture were 20 years 2) The ratio of Men to women was 5 : 1 3) The most frequent location of facial bone fractures was the Mandible (76.7%), Maxillae(10.6%), Zygoma(9.5%), Nasal bone(3.2%) 4) The ratio of Mandible to Maxillae was 7.2 : 1 5) In mandible fractures, the most frequent fracture site was Symphysis(48.7%) 6) In maxilla fractures, the most frequent case was Le fort III fracture(51.4%), followed by Le fort I fracture(29.7%), Le fort II fracture(18.9%) 7) The main contributing causes of facial bone fractures were Traffic Accident(37.7%), Fight(31.6%), Accident(27.5%) 8) In open reduction methods of mandible fracture, the frequency of postop, malocclusion was 15% in interosseous wiring 7% in Bicortical osteosynthesis, 3% in Monocortical osteosynthesis. The miniplates show advantages over other forms of fixation, in that they are malleable and easy to insert, they achieve rigid fixation and stability.

The zygoma is a heavy bone that composes a part of orbit and it was considered to be rarely fractured. However the rate of zygoma injury is increasing due to the outdoor life and motorvehicle accidents. The authors use a bone hook for direct reduction of fractured zygoma in combinaton with a miniplate osteosynthesis for rigid fixation and we reconstruct an infraorbital with a Lyodura.

Diagnostic ultrasound has the advantages of being noninvasive without any known deleterious biologic effect, rapid, painles, inexpensive and easily reproducible. The author used a real time sector scanner (Picker, Artis USA:5-7.5 MHz) for examing of lipoma, abscess, hematoma, lymphnode enlargement etc. and evaluated ultrasonograph as a diagnostic aid in head and neck region.
顎矯正術을 받은 顎顔面 寄形患者의 SKELETAL PATTERN의 變化에 關한 硏究

In the treatment of dentofacial deformities, analysis of skeletal pattern, with evaluation of plaster dental case, is essential procedure. This study was based on 20 patients with dentofacial deformity who were admitted to the department of oral and maxillofacial surgery Kngnam sacred heart Hospital during the period of Jan 1989. We studied serial cephalometric radiogarphs of patients (pre-op, postop immediately, postop 6 moth). The obtained results indicates the good resistance of rigid fixation against relapse in orthognathic surgery.

The analysis of soft tissue profile is an important paramenter in the esthetic aspect of facial deformity correction. The authors have studied on the soft tissue changes after the orthognathic surgery. We grouped the 30 patients according to the operation methods ; group 1 S.S.R.O. only, group 2 S.S.R.O.+Genioplasly, group 3 LeFortⅠ+S.S.R.O. The results were obtained as follows ; 1. In Group 1, the ratio of horizontal changes of hard tissue to soft tissue at 1 to Stmi, B to Si, Pog/SPog is 1.09, 0.96, 1.12 with postop. immediately, 0.98, 0.89, 0.98 with postop. 6 months. 2. In Group 2, the ratio of horizontal changes of hard tissue to soft tissue at 1/t Stmi, B/Si, Pog/SPog is 1.09, 1.11, 1.12 with postop. immediately, 0.92, 0.88, 0.99 with postop. 6 months. 3. In Group 3, the ratio of horizontal changes of hard tissue to soft tissue at A to Sn, 1 to Stms, B to Si, Pog to SPog is 1.15, 1.09, 1.15, 0.89, 1.05 with postop. immediately, 0.98, 0.94, 0.99, 0.90, 0.93 with pstop. 6 months.
