        Efficient 1-5 Regioselective Acylation of Primary Hydroxyl Groups of Fermentative Derived Xylitol Catalyzed by an Immobilized Pseudomonas aeruginosa Lipase

        The experiment described in this paper synthesizes xylitol acylated products from fermentative derived xylitol and acid anhydrides of various chain lengths in the presence of Tetrahydrofuran (THF) and acetonitrile using immobilized Pseudomonas aeruginosa (PL) lipase as a biocatalyst (97% residual activity up to five cycles) at 37°C, 200 rpm. This study examines a number of different acid anhydrides for their highly selective and efficient lipase-catalyzed acylation of primary hydroxyl groups in xylitol. Of those studied, the best results are obtained with butanoic anhydride, 80.12% after 4 h in acetonitrile followed by vinyl acetate, which results in 77.79% conversion after 8 h of incubation in THF as analyzed through high performance liquid chromatography (HPLC).

        Evaluation of Direct Anterior Approach for Revision Total Hip Arthroplasty: A Systematic Review

        The direct anterior approach (DAA) is an established approach for total hip arthroplasty (THA) but has been sparingly tried for revisions. The purpose of this study was to examine the available literature in order to consolidate information available on revision THA using the DAA. A PubMed, Embase, and Scopus search was performed using relevant keywords. Studies reporting on patients undergoing revision THA using DAA were included for analysis. In a review of the literature, nine studies matched the pre-decided inclusion criteria with 319 hip joints undergoing revision THA. Mean follow-up of all included studies was 34 months. The indications of revision after primary THA in decreasing order were aseptic loosening (53%), prosthetic joint infection (20.7%), peri-prosthetic fracture (16.9%), dislocation (7.2%), psoas impingement (1.9%), polyethylene wear (1.2%), pain (0.6%), and instability (0.3%). Of the 319 revisions evaluated, 107 underwent a stem revision, 142 underwent cup revision, 49 underwent a combined revision, and 21 underwent isolated liner/head change. A statistically significant improvement in functional score (P<0.05) was observed for all studies reporting on functional outcomes. A low complication rate (51/319, 16.0%), which includes dislocation (12), infection (12), loosening of the acetabular shell (5), peri-prosthetic fractures (6), haematoma (4), and transient nerve palsy (6), was reported. Based on available level Ⅲ-IV evidence, DAA appears to be a reliable alternative for revision of the failed hip arthroplasty with acceptable complication rates. Evidence of a higher quality is needed to further characterize its role in revision scenarios.

      • A comparative study of granular activated carbon and sand as water filtration media with estimation of model parameters

        The use of Granular Activated Carbon (GAC) and naturally occurring silica (Sand) as filtration media in water and waste water treatment systems is very common. While GAC offers the additional functionality of being an "adsorptive" filter for dissolved organics it is also more expensive. In this paper we present an experimental evaluation of the performance of a bed of GAC for colloid removal and compare the same with that from an equivalent bed of Sand. The experiments are performed in an "intermittent" manner over extended time, to "simulate" performance over the life of the filter bed. The experiments were continued till a significant drop in water flow rate through the bed was observed. A novel "deposition" and "detachment" rate based transient mathematical model is developed. It is observed that the data from the experiments can be explained by the above model, for different aqueous phase electrolyte concentrations. The model "parameters", namely the "deposition" and "detachment" rates are evaluated for the 2 filter media studied. The model suggests that the significantly better performance of GAC in colloid filtration is probably due to significantly lower detachment of colloids from the same. While the "deposition" rates are higher for GAC, the "detachment" rates are significantly lower, which makes GAC more effective than sand for colloid removal by over an order of magnitude.

      • Immunohistochemical Profile of Breast Cancer Patients at a Tertiary Care Hospital in New Delhi, India

        Background: To assess the immunohistochemical expression of estrogen receptor (ER), progesterone receptor (PgR) and human epidermal growth factor receptor-2 (HER2) neu receptor in breast cancer and their associations with various clinicopathological characteristics. Materials and Methods: This is a retrospective analysis of women who presented with primary, unilateral breast cancer in the Department of Medical Oncology at Rajiv Gandhi Cancer Institute and Research Centre, Delhi, India during the period from January 2008 to December 2011. Data were retrieved from the medical records of the hospital including both early and locally advanced cancer cases. ER, PgR and HER2neu expression in these patients was assessed and triple negative patients were identified. Associations of triple negative and non-triple negative groups with clinicopathological characteristics were also evaluated. Results: A total of 1,284 women (mean age 52.1 years, 41.9% premenopausal) were included in the analysis. Hormone receptor positivity (ER and/or PgR) was seen in 63.4% patients, while 23.8% of tumors were triple negative. Only 23.0% were HER2 positive. Around 10.0% of tumors were both ER and HER2 positive. ER and PgR positivity was significantly associated with negative HER2 status (p-value <0.0001). Younger age, premenopausal status, higher tumor grade, lymph node negativity, advanced cancer stage, and type of tumor were strongly associated with triple negativity. Significantly, a smaller proportion of women had ductal carcinoma in situ in the triple negative group compared with the non-triple negative group (35.6% versus 60.8%, p-value<0.01). Conclusions: The present analysis is one of the largest studies from India. The majority of the Indian breast cancer patients seen in our hospital present with ER and PgR positive tumors. The triple negative patients tended to be younger, premenopausal, and were associated with higher tumor grades, negative lymph nodes status and lower frequency of ductal carcinoma in situ.
