Mature cystic teratomas of the ovary (MCT) are usually observed in women of reproductive age with the most dreadful complication being malignant transformation which occurs in approximately 1% to 3% of MCTs. In this case report, we present a patient with squamous cell carcinoma which developed from a MCT during pregnancy. The patient was treated conservatively without adjuvant chemotherapy and was followed without evidence of disease for more than 60 months using conventional tools as well as positron emission tomography-computed tomography following the initial surgery. We report this case along with the review of literature.

After the discovery of melanocytes in the cervix in 1959, it was recognized that primary malignant melanoma of the cervix exists as a separate entity. A 74-year-old woman visited hospital for vaginal bleeding from a black colored cervical mass. The pathology of cervical punch biopsy showed a malignant melanoma with positive immnunohistochemical stainings for S100 protein and HMB- 45 antibody. Abdominal radical hysterectomy with pelvic and paraaortic lymphadenectomy was performed. The fi nal pathology was a malignant melanoma of the cervix with metastases for both external iliac lymph nodes and tumor involvement in the margin of vaginal resection. She received cisplatin based concurrent chemoradiotherapy postoperatively. But 6 months later, she received another chemotherapy with dacarbazine and cisplatin for recurrence. We report a case of a 74-year-old patient with a malignant melanoma of the uterine cervix with a brief review.

The present study was designed to clarify whether scuba diving at 5 meters of seawater influences cerebral hemodynamics, hematological and biochemical variables. Twenty healthy young men well trained scuba diving participated in this study. The blood flow velocity in the right and left middle cerebral arteries (L-MCAV and R-MCAV), blood pressure (BP), heart rate (HR), CBC and differential count, prothrombin time (PT), activated partial thromboplastin time (aPTT), biochemical variables, D-dimer and interleukin-8 (IL-8) levels were determined before, immediately after scuba diving for 30 min, and after 30 min of rest (Pre-scuba, Scuba and R-30m, respectively). L-MCAV and R-MCAV tended to increase, but the only significant increase was in L-MCAV in Scuba. SBP and HR significantly declined in R-30m compared with those of Pre-scuba and the Scuba. IL-8 levels were elevated in Scuba and R-30m compared with that of Pre-scuba. In Scuba and R-30m, hematological variables except PT and biochemical parameters excluding glucose and lactic acid did not significantly changed in comparison with those of Pre-scuba. PT level at Scuba and glucose level at R-30m significantly declined in Scuba, while lactate level at R-30m increased compared with each in Pre-scuba. However, PT level at Scuba was within a normal range. These results suggest that scuba diving at 5 m of seawater for 30 min has no adverse effects, is safe and useful for improving health. However, further study must be performed to clarify the mechanism of elevated IL-8 level following scuba diving.

Tuberculous peritonitis in pregnancy is a rare form of extrapulmonary tuberculosis that is not easily diagnosed. The clinical presentations of tuberculous peritonitis are usually non-specific and mimic those of other diseases, such as ovarian malignancy or chronic liver disease, and this non-specificity can cause diagnostic delays and complications. The authors report the case of a 31-year-old primigravida woman who presented with uncontrolled fever, dyspnea, elevated liver enzymes, and mild abdominal distension at 13+2 weeks of gestation. At 14+2 weeks, a therapeutic abortion was conducted and tuberculous peritonitis was confirmed by laparoscopic excisional biopsy of peritoneal nodules and histopathologic examination. The patient recovered on antituberculosis therapy and abdomen and chest follow up radiographic findings have confirmed improvement.

흰무늬엉겅퀴 열매 추출물의 자외선에 대한 피부 보호 효과

자외선은 피부 광노화를 일으키며 홍반, 일광 화상 등 피부 광손상의 원인이 된다. 실리마린은 밀크씨슬로도 알려져 있는 흰무늬엉겅퀴(Silybum marianum; S. m) 추출물에 존재하는 폴리페놀 화합물로 항산화 효과 및 자외선에 대한 피부 보호 효과가 알려져 있다. 본 연구에서는 피부의 표피층에서 실리마린을 함유하는 S. m 추출물의 광보호 효과를 이해하고자 하였다. 표피층을 구성하는 세포들이 자외선에 노출되면 DNA 손상이 발생하는데 S. m 추출물이 DNA repair를 촉진할 뿐만 아니라 UV filter로 작용하여 DNA 손상을 방지하는 것도 확인할 수 있었다. 특히, 홍반이 발생하지 않는 suberythemal dose (SED)의 자외선 노출에도 각질 산화와 DNA 손상이 일어날 수 있으며, S. m 추출물이 이런 미세 손상이 발생하는 것을 억제하는 것도 확인하였다. 또한 자외선에 의하여 각질이 산화되어 카르보닐화 단백질(protein carbonylation)이 증가하는 현상도 S. m 추출물을 도포한 경우는 억제됨을 확인하였다. S. m 추출물은 흡광의 성질이 있지만 광독성 유발 가능성은 낮은 것으로 평가되었다. 따라서 S. m 추출물은 광노화를 방지하고 피부를 보호하는데 활용 될 수 있을 것이다. Ultraviolet rays (UV) cause photoaging by inducing skin photodamages such as erythema and sunburn. Silymarin is a mixture of antioxidant polyphenols extracted from Silybum marianum fruit (S. m), which is known as milk thistle. It is known to protect skin tissues from UV treatment and antioxidant effects. In this study, we aimed to identify the photoprotective effects of S. m extract, which has silymarin in the epidermis layer of the skin. We found that the extract can function as a UV filter, so it can reduce DNA damage by UV treatment. Especially, we found that, in the stratum corneum, the extract can suppress the protein carbonylarion and DNA damages caused by suberythemal dose of UV treatment which does not induce erythema in the skin. UV treatment also increased protein carbonylation levels in the stratum corneum by oxidation, but it was prevented by applying the extract. The extract can absorb UV with minimal phototoxicity. Together, our study suggests that S. m extract can be used as a photo-protective ingredient to avoid photoaging of the skin.

The hematogenous spreading of an infectious pathogen via the portal vein from a mucosal injury in the gastrointestinal tract has been considered as one of the pathologic mechanisms of pyogenic liver abscess. Several studies have presented the association between colorectal cancer and pyogenic liver abscess. However, the cases of stomach cancer concomitant with pyogenic liver abscess have rarely been reported in the world. Herein, we present a case of advanced gastric cancer concomitant with pyogenic liver abscess in a patient who previously underwent subtotal gastrectomy due to peptic ulcer perforation. Spontaneous complete chorioamniotic membrane separation (CMS) without invasive fetal procedure is extremely rare and associated with adverse perinatal outcomes. A woman with complete CMS which was detected at the 21 weeks` gestation. She did not take any fetal invasive procedures before the diagnosis. At 27 weeks` gestation, an emergency Caesarean section was performed because of fetal distress. The defect of the uterine muscle was detected on the fundus. The baby has grown well without any morbidity. This is the first reported case of complete CMS relative to uterine scar. And we suggest that the pregnancy can be maintained successfully if there is no fetal abnormality when complete CMS is detected on ultrasound.
Uterine prolapse in a primigravid woman

Uterine prolapse during pregnancy is an uncommon condition. It can cause preterm labor, spontaneous abortion, fetal demise, maternal urinary complication, maternal sepsis and death. We report the case of uterine prolapse in a 32-year-old healthy primigravid woman. She had no risk factors associated with uterine prolapse. She was conservatively treated, resulting in a successful vaginal delivery. This report is a very rare case of uterine prolapse in a young healthy primigravid woman, resulting in a successful vaginal delivery.

Objective: The aim of the study is to re-validate CRS as a prognostic factor for ovarian cancer patients who received front-line maintenance therapy or intra-operative chemotherapy. Methods: The retrospective analysis was performed with the medical records from tubo-ovarian high grade serous carcinoma patients who received neoadjuvant chemotherapy followed by interval debulking surgery between January 2007 to December 2021 at five tertiary medical institutions in South Korea. Progression-free survival (PFS) and overall survival (OS) were obtained using Kaplan-Meier analysis; the aforementioned was used to evaluate the effect of BRCA mutation, Poly-ADP Ribose Polymerase (PARP) inhibitor, bevacizumab, hyperthermic intraperitoneal chemotherapy (HIPEC), and CRS. Results: A total 549 patients were analyzed. BRCA 1/2 mutations were detected in 178(32.4%) patients and 117(21.3%) patients were treated with PARP inhibitors, and 57(10.4%) patients were treated with bevacizumab as a front-line maintenance therapy. CRS3 in patients with BRCA wildtype was associated with improved PFS (25.9 vs 15.2 months, p<0.001) compared to CRS 1 or 2. However, there is no significant PFS prolongation of CRS3 in patients with BRCA mutation compared to CRS 1 or 2 (22.0 vs 19.3 months, p=0.17). In addition, CRS3 in PARP inhibitors-naive patients improved PFS significantly longer than CRS 1 or 2 (24.7 vs 15.5 months, p<0.001), whereas CRS3 in patients with PARP inhibitors as a front-line maintenance therapy were not significantly associated with prolongation of PFS (24.3 vs 22.4, p=0.85). Similarly, in opposition to bevacizumab-naïve patients, no statistically significant PFS prolongation of CRS3 was revealed compared to CRS1 or 2 in patients with front-line bevacizumab maintenance therapy (27.5 vs 17.5 months, p=0.202). Conclusion: Contrary to previous research, the CRS system may not be a prognostic factor in patients affected by BRCA mutation or treated with front-line maintenance therapy such as PARP inhibitors or bevacizumab.
