Simultaneous Borderline mucinous tumor, Mature cystic teratoma and Endometriosis in the Same Ovary

The synchronous presentation of borderline mucinous tumor, mature cystic teratoma, and endometriosis in the same ovary has never been reported in the literature. Ovarian endometriosis coexisting with a dermoid cyst is extraordinarily rare, although these both conditions are common in reproductive women. A 42-year-old woman presented with progressive dysmenorrhea and abdominal fullness. Pelvic magnetic resonance imaging revealed a 21×9.0×19.0 cm sized multiseptated cystic mass with internal fat lobules and calcification in the whole pelvis. Laboratory examination showed an elevated serum CA-125 of 110.6 U/ml and CA19-9 of 186 U/ml. On section, the tumor showed multilocular cystic mass, filled with mucinous fluid, sebum and hairs. The authors present a case of the simultaneous borderline mucinous tumor associated with mature cystic teratoma and endometriosis in same ovary in a reproductive woman. Microscopically, three tumor types were revealed: borderline mucinous tumor, mature cystic teratoma, and endometriosis. All ovaries with mature cystic teratoma should be thoroughly sectioned because additional histogenetically distinct lesions occurring in a mature cystic teratoma could easily be missed through inadequate sectioning.
Splenic sarcoidosis discovered concurrently with the diagnosis of ovarian adenocarcinoma

Sarcoidosis is a multisystem inflammatory disease of unknown etiology characterized by noncaseating epithelioid granuloma formation. Although the relationship between sarcoidosis and malignancy has been noted in recent decades, there are few case reports describing the concurrent diagnosis of sarcoidosis and malignancy. A 66-year-old woman, gravida 4 para 1, visited to Pusan National University Hospital complaining of low abdominal discomfort and weight loss. She denied any cutaneous or ocular symptoms. She had a history of hypertension and her brother died from colon cancer. Computed tomography (CT) of the abdomen showed a 100×47 mm sized multiseptated cystic mass in the right ovary; multiple extensive lymphadenopathy including multiple 2 to 6 cm sized lymph nodes in the splenic hilum, celiac trunk, hepatic hilum, right perihepatic space and retroperitoneum; and a low density lesion measuring approximately 12×27 mm in the spleen. Histopathologic findings revealed noncaseating granulomatous inflammation without tumor cells. A polymerase chain reaction-based assay was negative for mycobacterium tuberculosis, leaving sarcoidosis as the probable diagnosis. Herein, we describe a case of biopsy-proven splenic sarcoidosis mimicking metastasis at the time of ovarian adenocarcinoma. Imaging studies including positron-emission tomography, computed tomography were not useful for differentiating sarcoidosis from malignancy. Thus, our case highlights the importance of histopathological examination to rule out nonmalignant conditions before the diagnosis of metastatic disease is made.

목적: Tumor microenvironment reportedly plays critical roles in drug resistance and proliferation of CSCs; however, the functional role of CSC-secreted factors has not been clarified. 방법: We isolated cancer stem cells (CSCs) through sphere culture of A2780 epithelial ovarian cancer (EOC) cells and primary human EOC cells, and identified lysophosphatidic acid (LPA) as a CSC-derived autocrine factor using mass spectrometry. 결과: LPA treatment stimulated CSC-like properties, including drug resistance and high tumorigenic potential. CSCs were enriched in aldehyde dehydrogenase- high subpopulation, in which autotaxin, an LPA-producing enzyme, was highly expressed. LPA signaling was mediated through LPA receptor 1-dependent activation of the PI3K-AKT pathway. Abrogation of autotaxin-LPA-LPA receptor 1-AKT signaling axis significantly suppressed CSC characteristics, which provides a potential target for CSC treatment. EOC shows high mortality due to development of resistance to chemotherapy and relapse. 결론: CSCs have been implicated in drug resistance and relapse of ovarian cancer. Our findings suggested that ATX-LPA-LPAR1-AKT1 signaling axis is critical for maintenance of cancer stem-like characteristics in an ovarian CSC subpopulation through an autocrine loop. Inhibiting ATX-LPALPAR1- AKT1 signaling axis by chemical inhibitor or knockdown of gene expression increased the sensitivity of CSCs to chemotherapeutic reagents. Our findings provide therapeutic opportunities for development of relapse-free treatment of EOC.
