<P>Recent studies have demonstrated that activated microglia play an important role in dopamine (DA) neuronal degeneration in Parkinson disease (PD) by generating NADPH-oxidase (NADPHO)-derived superoxide. However, the molecular mechanisms that underlie microglial activation in DA cell death are still disputed. We report here that matrix metalloproteinase-3 (MMP-3) was newly induced and activated in stressed DA cells, and the active form of MMP-3 (actMMP-3) was released into the medium. The released actMMP-3, as well as catalytically active recombinant MMP-3 (cMMP-3) led to microglial activation and superoxide generation in microglia and enhanced DA cell death. cMMP-3 caused DA cell death in mesencephalic neuron-glia mixed culture of wild-type (WT) mice, but this was attenuated in the culture of NADPHO subunit null mice (gp91(phox-/-)), suggesting that NADPHO mediated the cMMP-3-induced microglial production of superoxide and DA cell death. Furthermore, in the N-methyl-4-phenyl-1,2,3,6-tetrahydropyridine (MPTP)-injected animal model of PD, nigrostriatal DA neuronal degeneration, microglial activation, and superoxide generation were largely attenuated in MMP-3-/- mice. These results indicate that actMMP-3 released from stressed DA neurons is responsible for microglial activation and generation of NADPHO-derived superoxide and eventually enhances nigrostriatal DA neuronal degeneration. Our results could lead to a novel therapeutic approach to PD.</P>
주름관 내부 유동과 압력강하에 대한 D형 그루브의 영향에 관한 수치해석

주름관은 자체의 신축성으로 변형에 의한 파손에 대응하고 설비의 편리성으로 인해 소방장비, 스프링클러 배관 등에 널리 사용된다. 그러나 벽의 주름진 형상으로 인해 복잡한 내부 난류유동이 발생하게 되며, 파이프 유동에 중요한 설계인자인 압력강하의 예측이 어렵다. 주름관 내의 압력강하에는 그루브 높이, 길이, 피치 등 파이프 벽의 형상 인자가 영향을 준다. 기존의 연구에는 관 내의 사각형 그루브의 피치(P)와 높이(K)의 비가 5 보다 작은 D형관의 경우에 대하여 길이 변화에 따른 압력 강하에 관하여 연구한 사례가 있다. 본 연구에서는 길이와 높이 변화를 고려한 P/K가 2.8, 3.5, 4.67의 경우 유동의 Re 수가 55,000, 70,000, 85,000인 경우에 대해 압력강하에 대한 수치해석 연구를 수행하였고, 주름관 내의 압력강하는 P/K 가 작을수록 감소하는 것으로 해석되어 압력 강하가 그루브 가로 세로 비율의 변화에 영향을 받고, 그루브 높이가 증가하면 재순환 면적이 증가하기 때문에 영향을 받는다는 것을 보여주었으며, 레이놀즈 수가 클수록 압력 강하가 증가하는 결과를 얻었다. A corrugated pipe is widely used in firefighting equipment and sprinkler pipes because of its elasticity, which is less damaged by deformation and convenient facilities. However, the corrugated shape of the wall results in complex internal turbulent flow, and it is difficult to predict the pressure drop, which is an important design factor for pipe flow. The pressure drop in the corrugated tube is a function of the shape factors of the pipe wall, such as groove height, length, and pitch. Existing studies have only shown a study of pressure drop due to length changes in the case of D-shaped tubes with less than 5 pitch (P) and height (K) of the rectangular grooves in the tube. In this work, we conduct a numerical study of pressure drop for P/Ks with length and height changes of 2.8, 3.5 and 4.67 with Re Numbers of 55,000, 70,000 and 85,000. The pressure drop in the corrugated tube was interpreted to decrease with smaller P/K. We show that the pressure drop is affected by the change in the groove aspect ratio, and the increase in the height of the groove increases the recirculation area, and the larger the Reynolds number, the greater the pressure drop.
Robust H\ulcorner Control for Delayed System with Time-Varying Norm-Bounded Parameter Uncertainty

In this paper, we present a robust H\ulcorner control design method for parameter uncertain systems that have delay in both state and control input. Through a certain algebraic Riccati inequality approach, a state feedback controller is obtained. The proposed state feedback controller stabilizes parameter uncertain delay systems and guarantees disturbance attenuation within a prescribed level. An illustrative example is given to demonstrate the results of the proposed method.
Morphine 과량복용 후 중독증상을 보인 환자의 지연된 Naloxone 치료 1례

Opioids are the one of the most commonly used drugs to control cancer pain all over the world. But, we should not overlook the potential risk of opioid intoxication because they have well-known detrimental side effects. The opioid intoxication can be diagnosed thorough various clinical manifestations. The altered mental status, respiratory depression, and miosis is very representative clinical features although these symptoms don’t always appear together. Unfortunately the opioid-toxidrome can be varied. A 42 years old man came to our emergency room after taking about 900 mg morphine sulfate per oral. He was nearly alert and his respiration was normal. Even though his symptoms didn’t deteriorated clinically, serial arterial blood gas analysis showed increase in PaCO2. So we decided to use intravenous naloxone. Soon, he was fully awaked and his pupils size was increased. After a continuous infusion of intravenous naloxone for 2 hours, PaCO2 decreased to normal range and his pupil size also returned to normal after 12 hours. Though the levels of serum amylase and lipase increased slightly, his pancreas was normal according to the abdominal computed tomography. He had nausea, vomit, and whole body itching after naloxone continuous infusion, but conservatively treated. We stopped the continuos infusion after 1 day because his laboratory results and physical examinations showed normal. As this case shows, it is very important to prescribe naloxone initially. If you suspect opioid intoxication, we recommend the initial use of naloxone even though a patient has atypical clinical features. In addition, we suggest intranasal administration of naloxone as safe and effective alternative and it’s necessary to consider nalmefene that has a longer duration for opioid intoxication.
Lamotrigine 단독 과량복용으로 발생한 횡문근융해증

Lamotrigine is a newer anti-epileptic drug for adjunctive treatment of refractory epilepsy, partial seizures, generalized tonic-clonic seizures, and bipolar disorder. Lamotrigine overdose causes serious central nervous and cardiovascular problems, but reports are uncommon. Few lamotrigine overdoses have been described because anti-epileptic drug use is limited and usually used with combination of other anti-epileptic drugs. In addition, most patients visit emergency departments with multi-drug overdoses, so few cases of lamotrigine poisoning alone exist. We had a female patient visit our emergency department a couple of hours after a lamotrigine overdose treated with intravenous hydration and urine alkalization by NaHCO3. She recovered successfully without any evidence of renal injury. However, she developed profound rhabdomyolysis, a previously unreported complication of this medication. We suggest that serial creatine kinase levels should be measured after lamotrigine poisoning.
