이중차이분석 통해 본 산정특례제도의 의료비부담 완화효과 = Impacts of the Benefit Extension Policy on Financial Burden and Catastrophic health care expenditure

In 2006, the government implemented the policy of extending the health insurance benefit coverage to lower the out-of-pocket(OOP) share of patients of serious case. This study proposes to confirm effectiveness of the benefit extension policy by identifying changes in ‘OOP as a share of the ability to pay`` and ``incidence rate of catastrophic health care expenditure`` of each individual household as manifested before and after the benefit extension policy was implemented. The 1st and 4th year data from the Korea Welfare Panel Study (KoWePS), conducted by the Korea Institute for Health and Social Affairs (KIHASA), were used for the analysis. We used a difference in difference model with propensity score matching to assess the effect of the benefit expansion policy. We compared intervention changes in ``OOP as a share of the ability to pay`` and ``incidence rate of catastrophic health care expenditure`` between group and control group. The intervention group consisted of beneficiary of policy, and control group consisted of non-beneficiary of policy. According to the results, ``OOP as a share of the ability to pay`` and ``incidence rate of catastrophic health care expenditure`` decreased. The rates decreased overall between 2005 and 2008, while those of control group case rapidly increased rather than intervention group. The benefit extension policy was not associated with significant changes in financial burden and catastrophic health care expenditure. The policies alleviating catastrophic health payments need to be more developed.
