Endovascular repair for thoracic aortic aneurysm is widespread and recently, the advent of new device-fenestrated stent graft enables endovascular repair for aortic aneurysm to be close or involved in the orifice of left subclavian artery. However, fenestrated stent graft is not available in Korea. We report herein a case in which endovascular repair for thoracic aortic aneurysm closed to the orifice of left subclavian artery using custom made fenestrated stent graft. The aneurysm was successfully repaired by a total endovascular approach without open surgical repair.

A hyperglycemic hyperosmolar state is usually associated with type 2 diabetes. It has significant mortality and morbidity and is rare in the pediatric population. We describe a rare case of a 15-year-old boy with type 2 diabetes who presented to the emergency department with a mixed hyperglycemic hyperosmolar state and diabetic ketoacidosis. Excessive consumption of high-sugar carbonated drinks may have worsening the initial presentation. The patient recovered without any complications. We highlight the fact that gradual correction of osmolarity and sodium is important to avoid cerebral edema despite severe dehydration.

Hemichorea-hemiballism is a rare complication of nonketotic hyperglycemia in type 2 diabetes mellitus (T2DM). It can be complicated in long-standing type 1 diabetes mellitus or T2DM, and has been described as a presenting symptom of new-onset diabetes. Rapid correction of diabetic ketoacidosis may also cause the delayed hemichorea. Although hyperglycemic hemiballism rarely causes generalized chorea due to bilateral basal ganglia involvement, patients typically present with hemichorea developing over days to months in the setting of elevated serum glucose. On T1-weighted brain magnetic resonance imaging and computed tomography scan a high signal intensity lesion at the basal ganglia is characteristic. After the correction of hyperglycemia, the movements generally disappear within hours, but atypical cases with delayed onset after the resolution of hyperglycemia, unremitting severe movements, and late recurrence are also reported. We report two cases of female T2DM patients who presented with hemichorea. One patient presented with hemichorea in nonketotic hyperglycemia, and the other with delayed onset hemichorea after the resolution of hyperglycemia.

We report one case of implant plate exposure after Ahmed valve implantation, treated with preserved fascia lata transplantation. Exposure of implant occurred in 47-year-old patient with Ahmed glaucoma valve implantation. She was treated with preserved fascia lata graft and amniotic membrane transplantation. No recurrent implant plate exposure has been found since then. Intraocular pressure has been maintained well for 3 months of follow-up. In cases of implant plate exposure, additional preserved fascia lata graft and amniotic membrane transplantation can improve surgical success.

Evans syndrome is an uncommon condition defined by the combination of immune thrombocytopenia and autoimmune hemolytic anemia in the absence of known underlying cause. Association of Evans syndrome with pregnancy is very rare, and only a few cases have been published in the medical literature. We experienced a case of Evans syndrome in pregnancy. Cesarean section was performed under general anesthesia without hemorrhagic complications at the 37th week of pregnancy following treatment with steroid, intravenous immunoglobulin, and transfusion of platelet concentrates. We have also briefly discussed the pathophysiology, the possible treatment options during pregnancy and prenatal outcome of Evans syndrome.
Burned-Out Testicular Choriocarcinoma with Retroperitoneal and Liver Metastases

Spontaneous regression or the ‘burned-out’ phenomenon of testicular germ cell tumors is well-recognized but a rare clinical entity of which few reports exist. Tumors with the ‘burned-out’ phenomenon are found in metastatic sites, not in the testis. We report a 33-year-old patient who presented with liver choriocarcinoma and a retroperitoneal mass with the ‘burned-out’ phenomenon in the testis.

Morbidity and mortality rates associated with acute myocardial infarction accompanying chronic total occlusion are comparatively high. European guidelines recommend primary intervention for the causative lesion in patient with acute myocardial infarction. Therefore, it is important to identify the culprit lesion. We report two cases of myocardial infarction with concurrent chronic total occlusion in an emergency setting.
Uterine Cervix Metastasis in Lung Adenocarcinoma with Anaplastic Lymphoma Kinase Rearrangement

The importance of anaplastic lymphoma kinase (ALK) as an oncogene in non-small cell lung cancer (NSCLC) has emerged as a major concern due to a dramatic clinical effect of targeted therapy. As compared with the research for targeted therapy, the study about clinicopathological characteristics for ALK positive NSCLC hasn’t been worked enough. Here, we describe a 35-year-old woman diagnosed with stage IVb NSCLC with ALK rearrangement. During evaluating her disease, a metastatic lesion to uterine cervix was found. Although lung cancer metastasis to female genital tract is rare, we also present case series that show a metastasis to the female genital tract in NSCLC with ALK rearrangement. These case series could suggest that ALK positive NSCLC has distinct metastatic pattern.

Hypoglossal nerve palsy is a rare complication of endotracheal intubation. The mechanism of nerve palsy is mainly attributed to stretching or compression of the nerve during airway manipulation. The cuff pressure can also contribute to the occurrence of hypoglossal nerve palsy. Since it is often accompanied by other cranial nerve palsies, meticulous overall cranial nerve examination is necessary. The main treatment is supportive with respiratory monitoring. The prognosis is favorable. Majority of patients achieve nearly full recovery of nerve function. Here, we report a case of unilateral hypoglossal nerve palsy following usual, uneventful endotracheal intubation and review the literature.

Objective Prostate-specific antigen (PSA) is widely used as a screening tool of prostate cancer, although there has been a controversy about efficacy. In Korea, rapid growth of obesity prevalence in adult men is a one of the biggest health problems. The objective of this study was to evaluate the potential association between body mass index (BMI), waist circumference (WC) and PSA. Methods We calculated BMI and WC of a total of 25,603 males who visited the Soonchunhyang University Cheonan Hospital Health Promotion Center from January 2016 to December 2020. This was categorized as follows: BMI < 23 kg/m2 (normal), 23 kg/m2 ≤ BMI < 25 kg/m2 (overweight), BMI ≥ 25 kg/m2 (obesity) and WC < 90 cm (normal), WC ≥ 90 cm (abdominal obesity). We analyzed the association between BMI, WC and PSA. Results The mean PSA value was 1.08 ng/mL, 1.03 ng/mL, and 0.97 ng/mL at normal, overweight, and obesity group, respectively. The mean PSA value was 1.04 ng/mL and 0.96 ng/mL at normal and abdominal obesity group. Thus, the mean PSA value decreased with an increase in BMI and WC category (P < 0.001). The correlation coefficient between BMI, WC and PSA was -0.055, -0.044 and they were statistically significant (P < 0.001). Conclusion We observed a significant negative correlation between BMI, WC and PSA value. Therefore, PSA value should be considered in relation to BMI and WC.
