        The Navigated Oblique Lumbar Interbody Fusion: Accuracy Rate, Effect on Surgical Time, and Complications

        Objective: The oblique lumbar interbody fusion (OLIF) can be done with either fluoroscopy or navigation. However, it is unclear how navigation affects the overall flow of the procedure. We wished to report on the accuracy of this technique using navigation and on how navigation affects surgical time and complications. Methods: A retrospective review was undertaken to evaluate patients who underwent OLIF using spinal navigation at University of California San Francisco. Data collected were demographic variables, perioperative variables, and radiographic images. Postoperative lateral radiographs were analyzed for accuracy of cage placement. The disc space was divided into 4 quadrants from anterior to posterior, zone 1 being anterior, and zone 4 being posterior. The accuracy of cage placement was assessed by placement. Results: There were 214 patients who met the inclusion criteria. A total of 350 levels were instrumented from L1 to L5 using navigation. The mean follow-up time was 17.42 months. The mean surgical time was 211 minutes, and the average surgical time per level was 129.01 minutes. After radiographic analysis, 94.86% of cages were placed within quartiles 1 to 3. One patient (0.47%) underwent revision surgery because of suboptimal cage placement. For approach-related complications, transient neurological symptoms were 10.28%, there was no vascular injury. Conclusion: The use of navigation to perform OLIF from L1 to L5 resulted in a cage placement accuracy rate of 94.86% in 214 patients.

        Effect of Sub-Structure and Precipitation Behavior on Mechanical Properties of Al–xCu–Li Alloys

        The tensile properties and microstructure of extruded and cold rolled 2A55 Al–Li alloys with different Cu content werestudied. After solution treatment, the strength of extruded sheets is slightly higher than that of cold rolled sheets due to strongdeformation texture and high density of substructure. However, the strength change order is different with the increase of Cucontent with T6-24 h. The extruded sample with higher Cu content have higher strength due to many dense T1precipitates. The δ′/θ′/δ′ composite precipitates and θ′ precipitates suppress the formation of T1precipitates, which cause cold rolled samplewith higher Cu content shows lower strength. Additionally, many fine T1precipitates nucleate at the sub-grain boundariesformed in the extrusion process, which have a significant effect on the mechanical property.

      • Prognostic Model Built on Blood-based Biomarkers in Patients with Metastatic Colorectal Cancer

        Background: We had previously showed that the neutrophil lymphocyte ratio (NLR), ${\gamma}$-glutamyl transpeptidase (GGT) and carcinoembryonic antigen (CEA) are prognostic factors for metastatic colorectal cancer (mCRC) patients. In this study we developed a prognostic model based on these three indices. Materials and Methods: A total of 243 patients who were initially diagnosed as mCRC between 2005 and 2010 in the Sun Yat-sen University Cancer Center were studied. The endpoint was overall survival (OS). Results: NLR>3, elevated GGT and elevated CEA were confirmed as independent risk factors which could predict poor prognosis. Patients could be divided into three groups according to the number of risk factors they had. Those with two or three were defined as the high risk group, individuals with one risk factor as the modest risk group and patients without risk factor as the low risk group. The OS values for these three groups were 16.2 months (2.80~68.8), 24.2 months (4.07~79.0), and 37.2 months (12.6~87.8), respectively (p<0.001). Conclusions: We developed a simple but useful model based on NLR, GGT and CEA to provide prognostic information to clinical practice in highly selected mCRC patients. Further prospective and multi-center studies are warranted to test our model.

      • Differential Expression of IQGAP1/2 in Hepatocellular Carcinoma and its Relationship with Clinical Outcomes

        Purpose: To investigate IQGAP1 and IQGAP2 expression in hepatocellular carcinoma (HCC) and itsassociation with HCC clinicopathological characteristics and survival outcomes. Methods: IQGAP1 and IQGAP2 mRNA and protein were measured in HCC tissues, para-tumor tissues and normal tissues by RT-PCR and Western blotting. We further examined 150 HCC samples with adjacent para-tumor tissues and 11 normal specimens by immunohistochemistry to evaluate the correlation of IQGAP1 and IQGAP2 with clinicopathological features and prognosis. Results: IQGAP1 mRNA and protein were up-regulated while IQGAP2 mRNA and protein were down-regulated in human HCC tissues compared with para-tumor and normal liver tissues (p<0.05). IQGAP1 expression was higher in primary HCC (122/150, 81.3%) than matched adjacent tissues (30/150, 20%, p<0.001), whereas IQGAP2 was lower (31/150, 20.7% as compared to 112/150, 74.7%, P<0.001). Positive IQGAP1 expression correlated with larger tumor size (p=0.002), advanced TNM stage (p=0.002) and tumor differentiation (III and IV, p=0.034). Negative IQGAP2 expression was significantly associated with larger tumor size (p=0.009), multicentric tumor occurrence (p=0.01), advanced TNM stage (0.009) and tumor differentiation (III and IV, p=0.020). Survival analysis revealed that patients with either IQGAP1+ or IQGAP2-tumors had significantly reduced disease-free survival (p<0.001 and 0.006 respectively) and overall survival (p<0.001 for both). Multivariate analysis showed that IQGAP1/2 switch was an independent prognosis factor for disease-free survival (HR=2.824) and overall survival (HR=2.189). Conclusion: Positive IQGAP1 and negative IQGAP2 expression were closely correlated with tumor progression and could be used as adjunctive biomarkers to improve prognostication for HCC patients.

        Investigation of the Mechanism of Period-doubling Bifurcation in Voltage Mode Controlled Buck-Boost Converter

        An investigation of the mechanism of period-doubling bifurcation in a voltage mode controlled buck-boost converter operating in discontinuous conduction mode is conducted from the viewpoint of nonlinear dynamical systems. The discrete iterative model describing the dynamics of the close-loop is derived. Period-doubling bifurcation occurs at certain values of the feedback factor. Results from numerical simulations and experiments are provided to verify the evolution of perioddoubling bifurcation, and the results are consistent with the theoretical analysis. These results show that the buck-boost converters exhibit a wide range of nonlinear behavior, and the system exhibits a typical period-doubling bifurcation route to chaos under particular operating conditions.

        Soluble expression, purification and the role of C-terminal glycine residues in scorpion toxin BmK AGP-SYPU2

        The existence of glycine residues in long-chain scorpion toxins has been well documented. However, their role as analgesics has not been evaluated. To address this issue, we investigated the functional role of glycines in the C-terminal end of Chinese-scorpion toxin from Buthus martensii Karsch (BmK AGP-SYPU2) using site-directed mutagenesis and analgesic activity assays. Recombinant BmK AGP-SYPU2 and its mutants were efficiently expressed in E. coli and purified to homogeneity using immobilized metal ion affinity chromatography (IMAC) and cation exchange chromatography. The mouse-twisting test was used to detect the analgesic activity of BmK AGP-SYPU2 and its mutants. As a result, we identified glycines at the C-terminal end that, when altered, significantly affected analgesic activity. Also, Mut6566 was significantly decreased compared to BmK AGP-SYPU2. These data indicate that the glycines at the C-terminal end are important for the analgesic activity of BmK AGP-SYPU2. [BMB reports 2010; 43(12): 801-806]

      • Experiments and theory for progressive collapse resistance of ECC-concrete composite beam-column substructures

        To explore the effect of Engineered Cementitious Composite (ECC) on improving the progressive collapse resistance of reinforced concrete frames under a middle column removal scenario, six beam-column substructures were tested by quasistatic vertical loading. Among the six specimens, four were ECC-concrete composite specimens consisting of different depth of ECC at the bottom or top of the beam and concrete in the rest of the beam, while the other two are ordinary reinforced concrete specimens with different concrete strength grades for comparison. The experimental results demonstrated that ECC-concrete composite specimens can improve the bearing capacity of a beam-column substructure at the stages of compressive arch action (CAA) and catenary action in comparison with ordinary concrete specimen. Under the same depth of ECC, the progressive collapse resistance of a specimen with ECC at the beam bottom was superior to that at the beam top. With the increase of the proportion of ECC arranged at the beam bottom, the bearing capacity of a composite substructure was increased, but the increase rate slows down with the proportion. Meanwhile, the nonlinear numerical analysis software MSC Marc was used to simulate the whole loading process of the six specimens. Theoretical formulas to calculate the capacities of ECC-concrete composite specimens at the stages of flexural action, CAA and catenary action are proposed. Based on the research results, this study suggests that ECC should be laid out at the beam bottom and the layout depth should be within 25% of the total beam depth.

        A Two-DNA Methylation Signature to Improve Prognosis Prediction of Clear Cell Renal Cell Carcinoma

        Purpose: Effective biomarkers and models are needed to improve the prognostic prospects of clear cell renal cell carcinoma(ccRCC). The purpose of this work was to identify DNA methylation biomarkers and to evaluate the utility of DNA methylationanalysis for ccRCC prognosis. Materials and Methods: An overview of genome-wide methylation of ccRCC tissues derived from The Cancer Genome Atlas(TCGA) database was download for analysis. DNA methylation signatures were identified using Cox regression methods. The potentialclinical significance of methylation biomarkers acting as a novel prognostic markers was analyzed using the Kaplan–Meiermethod and receiver operating characteristic (ROC) curves. Results: This study analyzed data for 215 patients with information on 23171 DNA methylation sites and identified a two-DNAmethylation signature (cg18034859, cg24199834) with the help of a step-wise multivariable Cox regression model. The area underthe curve of ROCs for the two-DNA methylation signature was 0.819. The study samples were stratified into low- and high-risk classificationsbased on an optimal threshold, and the two groups showed markedly different survival rates. Moreover, the two-DNAmethylation marker was suitable for patients of varying ages, sex, stages (I and IV), and histologic grade (G2). Conclusion: The two-DNA methylation signature was deemed to be a potential novel prognostic biomarker of use in increasingthe accuracy of predicting overall survival of ccRCC patients.

      • Corroded and loosened bolt detection of steel bolted joint based on improved you only look once network and line segment detector

        Steel bolted joint is an important part of steel structure, and its damage directly affects the bearing capacity and durability of steel structure. Currently, the existing research mainly focuses on the identification of corroded bolts and corroded bolts respectively, and there are few studies on multiple states. A detection framework of corroded and loosened bolts is proposed in this study, and the innovations can be summarized as follows: (i) Vision Transformer (ViT) is introduced to replace the third and fourth C3 module of you-only-look-once version 5s (YOLOv5s) algorithm, which increases the attention weights of feature channels and the feature extraction capability. (ii) Three states of the steel bolts are considered, including corroded bolt, bolt missing and clean bolt. (iii) Line segment detector (LSD) is introduced for bolt rotation angle calculation, which realizes bolt looseness detection. The improved YOLOv5s model was validated on the dataset, and the mean average precision (mAP) was increased from 0.902 to 0.952. In terms of a lab-scale joint, the performance of the LSD algorithm and the Hough transform was compared from different perspective angles. The error value of bolt loosening angle of the LSD algorithm is controlled within 1.09%, less than 8.91% of the Hough transform. Furthermore, the proposed framework was applied to fullscale joints of a steel bridge in China. Synthetic images of loosened bolts were successfully identified and the multiple states were well detected. Therefore, the proposed framework can be alternative of monitoring steel bolted joints for management department.
