      • Influence of Clinically Significant Portal Hypertension on Hepatectomy for Hepatocellular Carcinoma: a Meta-analysis

        Background: Clinically significant portal hypertension (PHT) is considered as a contraindication for hepatectomy according to the guidelines of the European Association for Study of Liver and the American Association for Study of Liver Diseases. However, this issue remains controversial. Here we performed a metaanalysis to evaluate the impact of PHT on the results of hepatectomy for hepatocellular carcinoma (HCC). Methods: Cohort studies evaluating the impact of clinically significant PHT, defined as oesophageal varices and/or splenomegaly associated with thrombocytopenia, on the results of hepatectomy for HCC were identified using a predefined search strategy. Summary risk ratios (RRs) and 95% confidence intervals (95% CIs) for PHT and outcomes after hepatectomy for HCC were calculated. Results: Seven cohort studies which including 574 cases with PHT and 1,354 cases without PHT were considered eligible for inclusion. The meta-analysis showed that, in all patients, pooled RRs of post-operative liver failure, post-operative ascites, peri-operative blood transfusion, operative mortality, 3- and 5-year overall survival associated with PHT were 2.23 (95% CI: 1.48-3.34, P=0.0001), 1.77 (95% CI: 1.19-2.64, P=0.005), 1.23 (95% CI: 1.03-1.49, P=0.03), 2.58 (95% CI: 1.12-5.96, P=0.03), 0.82 (95% CI: 0.75-0.88, P<0.00001) and 0.76 (95% CI: 0.69-0.85, P<0.00001), respectively. In subgroup analysis, similar results were found in Child-Pugh class A patients. Conclusion: This meta-analysis suggests that presence of oesophageal varices and/or splenomegaly associated with thrombocytopenia is associated with higher rates of post-operative complications and poor long-term survival after hepatectomy for HCC.

        Monosaccharide as a Central Scaffold Toward the Construction of Salicylate-Based Bidentate PTP1B Inhibitors via Click Chemistry

        The discovery of carbohydrate-based bioactive compounds has recently received considerable interest in the drug development. This paper stresses on the application of 1-methoxy-O-glucoside as the central scaffold, whereas salicylic pharmacophores were introduced with diverse spatial orientations probing into the structural preference of an enzymatic target, i.e. protein tyrosine phosphatase 1B (PTP1B). By employing regioselective protection and deprotection strategy, 2,6-, 3,4-, 4,6- and 2,3-di-O-propynyl 1-methoxy-O-glucosides were previously synthesized and then coupled with azido salicylate via click chemistry in forming the desired bidentate salicylic glucosides with high yields. The inhibitory assay of the obtained triazolyl derivatives leads to the identification of the 2,3-disubstituted salicylic 1-methoxy-O-glucoside as the structurally privileged PTP1B inhibitor among this bidentate compound series with micromole-ranged $IC_{50}$ value and reasonable selectivity over other homologous PTPs tested. In addition, docking simulation was conducted to propose a plausible binding mode of this authorized inhibitor with PTP1B. This research might furnish new insight toward the construction of structurally different bioactive compounds based on the monosaccharide scaffold.

      • 중국 고령사회의 노인복지서비스 확충을 위한 정책과제: 중국 청더시를 중심으로

        Since the 21st century, the world has faced a crisis of aging, and the issue of pension has become a world issue. At present, China’s aging population has entered a phase of rapid development and the current social pension service system is facing severe challenges. Relying solely on traditional family pensions is difficult to meet the elderly‘s need for pension security. Therefore, it is urgent to address the issue of welfare and security for the elderly and establish a sound service system for the elderly. This article introduced the current situation and trends of the aging population in China, the welfare policies of the elderly and the current situation and characteristics of welfare for the elderly. In addition, 100 civil servants in Chengde City carried out surveys on the needs and perceptions of the welfare of the elderly, and the contents of the survey consisted of income, health, living, and convalescence. According to the characteristics of respondents, the survey results were described and analyzed. Through the above-mentioned investigation and study, we can see that China is about to enter the aging society, but there are still some deficiencies in the elderly welfare policy. At present, as the ageing population continues to deepen, not only the number of elderly people is increasing, but also the number of elderly people and elderly people who are disabled is becoming more and more prominent. In addition to pure material needs, the needs of elderly people in terms of care, culture and entertainment have also become Growth trend. Only by digging out the welfare needs of the elderly, developing a diversity of welfare provision methods, and increasing welfare policies for the elderly can we meet the diversified welfare needs of the elderly. 21세기 이래 세계는 고령화의 위기에 직면해 있으며 연금 문제가 세계적인 이슈로 부상했다. 현재 중국의 노령 인구는 급속한 발전 단계에 접어 들었고 현재의 사회 연금 서비스 시스템은 심각한 도전에 직면해 있다. 전통적인 가족 연금에만 의존하는 것은 노령자의 연금 보장 필요성을 충족시키기가 어렵다. 따라서 고령자의 복지와 안전 문제를 해결하고 노인을 위한 건전한 서비스 체제를 수립하는 것이 시급하다. 이 연구에서는 중국의 노령 인구 현황, 노인 복지 정책, 노인 복지 현황 및 특성을 소개했다. 또한 청더(承德)시 공무원 100 여명이 노인 복지의 필요성과 인식에 대한 설문 조사를 실시했으며, 조사 내용은 소득, 건강, 생활 및 요양으로 구성되었다. 응답자의 특성에 따라 조사 결과를 설명하고 분석하였다. 위에서 언급한 조사와 연구를 통해 우리는 중국이 고령화 사회로 진입하려고 하고 있음을 알 수 있었지만 여전히 노인 복지 정책에는 몇 가지 결함이 있다. 현재 고령화가 심화됨에 따라 노령 인구가 증가하고 있을 뿐만 아니라 장애가 있는 노령자 및 노인 인구가 점차 커지고 있다. 순수한 물질적 필요에 더하여, 간병, 문화 및 오락 측면에서 고령자의 요구 또한 성장 추세가 되었다. 고령자의 복지 요구를 파헤치고, 복지 제도의 다양성을 개발하고, 노인 복지 정책을 향상시킴으로써 노인 복지 필요성을 충족시킬 수 있다.

      • EXPLORING STORYTELLING ELEMENTS IN MARKETING

        The present research is seeking to enhance their customers’ brand attitudes, especially in terms of which type of products should be their focus for customers with varying storytelling elements. Storytelling in marketing is critical for business, because it is a cornerstone for building strong relationship in the modern world. Storytelling elements also influence consumers’ emotional responses to brand. Besides, organization of information in an advertising narrative might influence the consumer's choice of an appropriate information-processing strategy (Mattila 2000), the narrative message of a story will affect consumer’s preference for a brand, thereby affects consumer’s purchase intention. However, despite some elements have been discussed in previous studies related to narratives (Chatman 1978; Papadatos 2006), we do not understand exactly why or when customers respond to stories. Therefore, to examine the storytelling effectives of different types of reversal factors, this research investigates the influence of different types of stories on brand attitudes in marketing. In sum, by studying the elements of reversal stories, this expected outcomes of this research intends to provide researchers and practitioners implications: 1. design a successful brand story can enhance corporate image and attract more consumers. 2. In practice, marketers need to combine storytelling factors with different products, and manipulate them with different weights to design the optimal advertisements or marketing campaign. 3. This study would be aroused the interesting to investigate other story elements and how these elements to affect brand attitudes.

      • The Design of an Intelligent Aquaculture Systsm Based on Iot and Lora Technologies

        In Taiwan, the export-oriented economy of traditional aquaculture is faced with the harshest conditions, such as environmental pollution, diseases and drug overdose. In addition, the traditional aquaculture fields are usually open-air for costing a lot on employing labors. Consequently, it is necessary to set up an intelligent aquaculture system for solving the above problems effectively. This study which was based on these technologies of internet of things (IoT), cloud computing and Long Range Low Power Wide Area Network (LoRa LPWAN), was designed a daily management system for auto-controlling on production planning, energy conservation and reducing labor costs. The users can operate this system which can both monitor the water quality and control these real-time aquaculture parameters (temperature, pH, illuminance and electrical conductivity) by simply using the remote user interface. This system is a modular design that is user-friendly and portable. After all, we expect that it will be great helpful to construct an automatic control platform for improving aquaculture efficiency in future.

        A Pan-Dengue Virus Reverse Transcription-Insulated Isothermal PCR Assay Intended for Point-of-Need Diagnosis of Dengue Virus Infection by Use of the POCKIT Nucleic Acid Analyzer

        <P>Dengue virus (DENV) infection is considered a major public health problem in developing tropical countries where the virus is endemic and continues to cause major disease outbreaks every year. Here, we describe the development of a novel, inexpensive, and user-friendly diagnostic assay based on a reverse transcription-insulated isothermal PCR (RT-iiPCR) method for the detection of all four serotypes of DENV in clinical samples. The diagnostic performance of the newly established pan-DENV RT-iiPCR assay targeting a conserved 3' untranslated region of the viral genome was evaluated. The limit of detection with a 95% confidence was estimated to be 10 copies of in vitro-transcribed (IVT) RNA. Sensitivity analysis using RNA prepared from 10-fold serial dilutions of tissue culture fluid containing DENVs suggested that the RT-iiPCR assay was comparable to the multiplex real-time quantitative RT-PCR (qRT-PCR) assay for DENV-1, -3, and -4 detection but 10-fold less sensitive for DENV-2 detection. Subsequently, plasma collected from patients suspected of dengue virus infection (n = 220) and individuals not suspected of dengue virus infection (n = 45) were tested by the RT-iiPCR and compared to original test results using a DENV NS1 antigen rapid test and the qRT-PCR. The diagnostic agreement of the pan-DENV RT-iiPCR, NS1 antigen rapid test, and qRT-PCR tests was 93.9%, 84.5%, and 97.4%, respectively, compared to the composite reference results. This new RT-iiPCR assay along with the portable POCKIT nucleic acid analyzer could provide a highly reliable, sensitive, and specific point-of-need diagnostic assay for the diagnosis of DENV in clinics and hospitals in developing countries.</P>
