미파열 뇌동맥류를 동반한 고혈압성 뇌내출혈 3례를 보고한다. 3례 모두 초기병변은 피각부출혈로서, 전대뇌동맥의 형성부전이 있는 측으로 출혈이 있었다. 뇌동맥류는 뇌내출혈 반대측을 공급하는 뇌혈관을 따라 위치했다. 뇌동맥류 발생 원인이 혈행역학적 긴장(hemodynamic stress)으로서, 연령 및 고혈압과의 관계를 고려해 볼때, 고혈압성 뇌내출혈 환자에서 뇌동맥류 동반은 흔히 예상될 수 있는 병변이나, 아직 이에 대한 보고가 없다. 고혈압성 뇌내출혈 환자에서 미파열 뇌동맥류 동반에 대한 주의 및 이에 대한 추적조사가 요망된다.

두부외상후 뇌전산화단층촬영상(CT) 종괴병변이 없는 소아환자 연속 100례를 대상으로 예후를 분석하였다. 대상 환자는 모두 15세 이하로 외상후 6시간이상 글라스고우 혼수계수가 8이하였다. 예후는 회복 27%, 중등도장애 17% , 중중장애 15%, 식물상태 2 % , 사망 39% 였다. 내원당시 글라스고우 혼수계수 5이하인 환자의 대부분은 사망하였다(81.9%). 내원당시 CT상 미만성뇌종창 소견은 53례에서 보였으며, 이중 27례(50.9%)에서 사망하였다. 예후에 나쁜 영향을 미치는 요인들로는 : 1) 내원당시 글라스고우혼수계수 5이하(p<0.05), 2) 저산소증(PaCO₂<60 ㎜Hg, p<0.05), 3) 뇌기저골절(p<0.005), 4) 폐 혹은 복부손상을 동반한 경우(p<0.005), 5) 미만성뇌종창(p<0.01), 6) 내원당시 CT상 뇌지주막하출혈, 뇌실내출혈 혹은 미량의 뇌경막하혈종이 있는 경우(p<0.05)였다. 연령, 성별, 두개골원개(skull vault) 골절, 두부를 제외한 다른 신체부위 골절 등은 예후에 영향을 미치지 못했다. 외상후간질이나 추적 CT상 나타난 지연성뇌병변은 불량한 예후를 나타내는 소견은 아니었다. 뇌량, 뇌기저핵부, 시상부, 혹은 상소뇌각 부위에 소량의 출혈 소견, 소위 말해 미만성뇌측색손상의 전형적인 CT 소견은 23례에서 나타났으나, 소아의 중증미만성뇌손상군에서는 불량한 예후를 반영하는 소견은 아니었다

<P>Cell–cell interactions between muscle precursors are required for myogenic differentiation; however, underlying mechanisms are largely unknown. Promyogenic cell surface protein Cdo functions as a component of multiprotein complexes containing other cell adhesion molecules, Boc, Neogenin and N-cadherin, and mediates some of signals triggered by cell–cell interactions between muscle precursors. Cdo activates p38MAPK via interaction with two scaffold proteins JLP and Bnip-2 to promote myogenesis. p38MAPK and Akt signaling are required for myogenic differentiation and activation of both signaling pathways is crucial for efficient myogenic differentiation. We report here that APPL1, an interacting partner of Akt, forms complexes with Cdo and Boc in differentiating myoblasts. Both Cdo and APPL1 are required for efficient Akt activation during myoblast differentiation. The defective differentiation of Cdo-depleted cells is fully rescued by overexpression of a constitutively active form of Akt, whereas overexpression of APPL1 fails to do so. Taken together, Cdo activates Akt through association with APPL1 during myoblast differentiation, and this complex likely mediates some of the promyogenic effect of cell–cell interaction. The promyogenic function of Cdo involves a coordinated activation of p38MAPK and Akt via association with scaffold proteins, JLP and Bnip-2 for p38MAPK and APPL1 for Akt.</P>

Background: There were few studies that investigated the differences in incidence of anxiety and depression according to individual diseases constituting ILD. We evaluated the differences in the incidence of anxiety and depression between idiopathic pulmonary fibrosis (IPF) and non-IPF ILD, and the changes to it at one year follow-up. Methods: Patients included those in the prospective ILD cohort of Seoul National University Hospital between March 2013 and August 2018. The Hospital Anxiety and Depression Scale (HADS) questionnaire was used to assess patients’ anxiety and depression. Results: There were 224 patients with ILD. The patients with IPF numbered 154 (68.8%), while those with non-IPF ILD numbered 70 (31.2%). Totally, 111 patients completed a follow-up HADS questionnaire one year later (77 in the IPF group, 34 in the non-IPF group). In the IPF group, 20.8% (32/154) and 27.3% (21/77) (p=0.347) of patients suffered from anxiety and 29.9% (46/154) and 31.2% (24/77) (p=0.957) suffered from depression at baseline and one year follow-up, respectively. In the non-IPF group, 14.3% (10/70) and 23.5% (8/34) (p=0.372) of patients suffered from anxiety and 24.3% (17/70) and 17.6% (6/34) (p=0.608) suffered from depression at baseline and one year follow-up, respectively. Conclusion: The proportion of patients with anxiety and depression at baseline and one year follow-up was not significantly different between the two groups. At one year follow-up, the proportion of those with anxiety and depression increased among the IPF patients but it was not statistically significant.

<P>Modulus-tunable composite micropillars are presented by combining replica molding and selective inking for skin adhesive patch in 'ubiquitous'-health diagnostic devices. Inspired from hierarchical hairs in the gecko's toe pad, a simple method is presented to form composite polydimethylsiloxane (PDMS) micropillars that are highly adhesive (1.8 N cm(-2) ) and mechanically robust (30 cycles).</P>

<P>We present a simple method for fabricating robust dry adhesives by coating a soft polydimethyl siloxane (PDMS) thin layer on rigid backbone micropillars of polyurethane acrylate (PUA). These core–shell type micropillars demonstrated enhanced durability both in normal and shear adhesion over more than 100 cycles of attachment and detachment. Relatively strong normal (∼11.4 N cm<SUP>−2</SUP>) and shear (∼15.3 N cm<SUP>−2</SUP>) adhesion forces were observed, which were similar to or even larger than those of homogeneous PDMS micropillars. A simple theoretical model based on beam deflection theory was used to explain the experimental results.</P> <P>Graphic Abstract</P><P>Robust dry adhesives with core–shell micropillars are described by combining replica molding and thin film coating. Objective:This study was conducted in order to demonstrate the initial experience of the Solitaire AB stent in mechanical intracranial thrombectomy. Methods:We conducted a retrospective review of 40 consecutive patients who underwent intra-arterial Solitaire AB stent thrombectomy for treatment of acute ischemic strokes between October 2010 and November 2011. Demographic, clinical, and radiological presentations and outcomes were studied. Results:Twenty six men and 14 women with a mean initial National Institutes of Health Stroke Scale (NIHSS) score of 14.1 (range, 8-26) and a mean age of 65.4 (range, 32-89) years were included in this study. Occlusion sites were as follows: internal carotid artery (n = 11), middle cerebral artery M1 (n = 22), M2 (n = 5), and basilar artery (n = 2). Successful revascularization was achieved in 36 (90%) patients. The mean NIHSS score was 11.6 (range, 1-23) at 24 hours after the procedure, and 42.5% of patients showed a modified Rankin scale score of ≤ 2 at 90 days. New occlusion by migrated emboli was observed in one (2.5%) case. Post-procedural intracerebral hemorrhage occurred in only one case (2.5%), with an all-cause mortality of two (5%). Conclusion:The Solitaire AB device is a relatively safe and effective tool for use in performance of mechanical thrombectomy in patients with acute ischemic stroke.
