Trigeminal Neurinomas : Report of 8 Cases with special Reference to Operative Approach

저자들은 최근 13년동안 총 8예의 삼차신경초종 환자에서 가능한한 1회의 수술에 의한 종양전적출의 원칙아래 수술을 시행하고 다음과 같은 결과를 얻었다. 남녀의 비는 3 : 5였고 연령 분포는 25세에서 56세까지 다양하였다. 종양의 위치 별 분포는 주로 중두개와 또는 후두개와에 국한된 경우가 각각 2예, 중·후두개와 양측에 걸쳐 장구형 모양을 정하고 있었던 경우가 4예였다. 주 증상으로는 안면부 동통과 청력장해가 가장 많았고 그밖에 다양한 증상이 출현하였다. 수술 접근법은 종양의 위치에 따라서 transpetrosal-transtentorial 혹은 orbitozygomatic-infratemporal approach 방법을 사용하였으며 모든예에서 종양을 전적출하였다. 1예에서만 수술 5년후에 종양이 재발하여 재수술을 시행하였으며 수술후 사망한 예는 없었으나 대부분의 예에서 수술중 삼차신경 분지의 손상으로 발생되는 부분적인 삼차신경 기능장애는 불가피하였다. 술후 결과는 3예에서 우수하였고 4예에서 양호했으며 1예에서는 수술전에 비해 악화되지 않는 좋은 결과를 보였다. The authors have treated eight cases of trigeminal neurinomas for the past thirteen years. We have been treating these tumors in accordance with the principle of radical resection through one-staged operation. Three were male and five were female. The age ranged from 25 to 56 years with the mean age of 41.5 years. One was a case of von Recklinghausen's disease, Topographically, the tumors were mainly located within the middle fossa in two cases, within the posterior fossa in two, and extended both supra- and infratentorially in four cases. Facial pain and hearing disturbance were the main symptoms and various other symptoms such as focal seizure, hemiparesis, gait disturbance, symptoms of increased intracrainal pressure and visual disturbance were also noted. Surgical operations were performed in all cases. As a surgical approach, either transpetrosal-transtentorial or orbitozygomatic-infratemporal route was employed. The choice of the better approach depends on the topography of the tumor. Total removal was performed in all cases. Only one case, which was an early one in this series required second operation to remove the tumor completely. In one case, the tumor recurred five years after the operation. There has been no operative mortality, but the injury or permanent damage to the trigeminal branches was inevitable in many cases. Surgical results were excellent in three, good in four, and fair in one case.

Medical ultrasound has benefits in mobility and safety than any other medical techniques such as X-ray, CT and MRI but has speckle noise which decrease the ability of an observer to distinguish the fine details in diagnostic examination. But simple removing of speckle often causes losing boundary information. Then, in this paper, we presented a novel neuro-fuzzy method which could remove speckle efficiently without loss of boundary information. Proposed method consists of image clustering by fuzzy algorithm and image processingby neural networks which was learned by back propagation. From the experiments for simulation image and real ultrasound image, we could verify the proposed method. 의용 초음파 장비는 X-Ray, CT, MRI 등 다른 의료 장비보다 휴대성과 안전성면에서는 장점이 있지만 진단 시 해상도와 화질 저하를 유발하는 스펙클이 생기는 단점이 있다. 그러나 단순한 스펙클 잡음의 제거는 경계선 정보의 손실을 발생시킬 수도 있다. 이에 본 논문에서는 효과적인 스펙클 제거와 손실 없는 경계선 검출을 위해 뉴럴네트워크와 퍼지 클러스터링을 이용한 뉴로-퍼지 스펙클 제거 방법을 제안하였다. 제안된 방법은 입력된 의용 초음파 영상에 대해 먼저 퍼지 클러스터링을 적용하여 세 영역으로 나누고 이후 각 영역별로 별도의 뉴럴 네트워크를 적용하는 방법이다. 실제 실험 및 기존 방법들과의 정성적?정량적 비교 분석을 통해 제안 방법의 유용성을 검증하였다.
