Microcystic adnexal carcinoma (MAC) is a locally aggressive malignant adnexal tumor displaying sweat duct and follicular differentiation. MAC usually presents as a slow-growing, yellow or erythematous plaque or nodule on central face. Typically, MAC is seen as a solitary lesion and case reports regarding multiple MACs are rarely been reported. A 43-year-old woman presented with five erythematous plaques and masses on scalp. According to the patient, about 10 years ago she underwent excisional biopsy for her occipital lesion and heard that there was cutaneous malignancy. There was no other combined systemic disease in our patient. Surgical resections were performed for five lesions on scalp. Excised specimens showed dual follicular and eccrine gland differentiation. Deeply infiltrating tumors were found and showed epidermal connection with follicular differentiations. Formation of ductal structures and intracytoplasmic lumina were also seen. Area showing ductal differentiation stained positively for carcinoembryonic antigen. The subcutaneous fat was affected and perineural invasion was observed. Histologic findings of the five lesions shared these various findings in a same spectrum. She was diagnosed as having multiple MACs and is currently on follow-up with no recurrence for a year. Herein, we present a rare case of multiple MACs on scalp in immunocompetent patient.

Eccrine poroma arises most often on the sole or side of the foot as a solitary nodule. There are few reports of the occurrence of multiple eccrine poroma, defined as eccrine poromatosis. The pathophysiology is unclear, but many of the published cases have a history of immunosuppression, either with chemotherapy or radiation. Eccrine poromatosis associated with chemotherapy or immunosuppression is a rare condition, and there is no reported case in Korean dermatologic literature to date. A 36-year-old male presented with multiple red, dome-shaped nodular lesions on his feet which have been for 6 months. 5 years ago he underwent chemotherapy and peripheral blood stem cell transplantation for his acute myeloid leukemia (AML). Graft-versus-host disease developed one year after transplantation and he was managed by taking azathioprine, imatinib, ruxolitinib and prednisolone. Apart from this, he has been in remission from AML until now. Two punch biopsied specimens were obtained and histopathologic examination showed marked acanthosis and tumor growing down into the dermis in broad anastomosing bands. Tumor cells were slightly smaller than keratinocytes, and sharp demarcation between the keratinocytes were seen. On the basis of clinical and histologic findings, he was diagnosed as eccrine poromatosis. Parathyroid Carcinoma: Outcome Analysis of 16 Cases from a Single Center

Purpose: Parathyroid carcinoma is an extremely rare malignancy, but there is still a lack of understanding of the natural course and prognostic implications of this disease. In this study, we reviewed the clinicopathological characteristics of patients with parathyroid carcinoma treated at our institution. Methods: Patients with parathyroid carcinoma were included in this study. We reviewed the clinicopathological characteristics of patients with parathyroid carcinoma treated at our institution, and evaluated patient outcomes and prognostic factors. Results: A total of 16 patients with parathyroid carcinoma were included in this study. The median follow-up duration was 61.5 months (range, 7–215 months), and seven patients developed recurrence (7/16, 43.8%). Four patients had died from parathyroid carcinoma (4/16, 25.0%) at the time of the analysis. Increased serum intact parathyroid hormone (iPTH) level during follow-up was an independent risk factor for recurrence after making adjustments for multivariate analysis. Trends of increasing serum calcium and iPTH levels after surgery were associated with the development of recurrence. Conclusion: We observed a good survival rate after initial surgery for parathyroid carcinoma. However, the rate of recurrence was relatively high and early detection of recurrence is critical. Close monitoring during follow-up is necessary, and a serial check for serum calcium and iPTH level may prove useful. When steadily increasing serum calcium and iPTH results are observed, careful evaluation for recurrence is needed.
