Purpose: This observational study was aimed to determine the influence of the Hawthorne effect on the adherence to hand hygiene (HH) among healthcare workers (HCWs) in South Korea. Methods: HCWs were monitored in 2 periods regarding adherence to HH when there were indications for HH. In first period, HCWs recognized that their behavior of hand hygiene being observed (overt observation), and did not recognize in second period (covert observation). Results: The overall difference in HH rate between two periods was 45.0% point (77.8% vs 32.8%). There were significant differences between profession but in nurse aids. The differences in HH rate between two periods were 46.1% point in nurses, 29.9% point in physicians, 64.0% in radiologists, 62.5% point in laboratory technicians, 36.4% point in physio-therapist, and 1.0% point in nurse aids. The Hawthorne effect on the adherence to HH lasted more than 3 months. Conclusion: The Hawthorne effect markedly influence on the adherence to HH regardless of profession except nurse aids. Therefore, Hawthorne effect can be useful tool to improve and sustain the adherence to HH among HCWs in South Korea.
신생아중환자실에서의 Extended Spectrum Beta Lactamase (ESBL) 생성 Klebsiella pneumoniae 유행의 성공적인 관리

Purpose: To describe successful control of outbreak of ESBL producing K. pneumoniae in neonatal intensive care unit. Methods: The 9 K. pneumoniae isolated from blood (7), urine (1), eye discharge (1) culture. The 5 cases with blood isolated were all occurred in infants with less than 1,500 g body weight. Intervention activities included hand hygiene, using maximal sterile barrier precaution during central venous catheter insertion, monitoring of medical staff`s performance of the contact precaution guidelines, and regular environmental cleaning and incubator disinfection. Especially, our study found the serious problem that the omnibedTM incubators are not replaced regularly to minimize intervention for very low birth weight infants. Results: It was recommended to replace a used omnibedTM incubator with an antiseptic one every week. Before the implementation of infection control, K. pneumoniae infection rate 6.29 per 1,000 patient-days. After the implementation, 2.02 per 1,000 patient-days January 2011 and 0 infection after February 2011. Conclusion: Our study demonstrates the importance of hand hygiene, aseptic technique, isolation precaution and environmental cleaning and disinfection of medical equipment. Especially, this outbreak was controlled by emphasizing the control of incubator as environmental reservoirs using for premature baby.

on nursing students. Methods: Cross-sectional survey was done with a self-administered questionnaire from 350 nursing students in 8 college from December 19, 2011 to February 21, 2012. Results: Overall correct answer to 13 knowledge-type questions was 70.8% percent, which wasn`t significantly different ever taken clinical practice (p=.174). 236 nursing students had taken an infection control training, which was significantly different (p=.012). The question ``the most important vehicle in pathogen transmission`` showed the highest correct answer (94.9%), followed by ``risk-guided application of a preventive strategy`` (92.0%). The question ``the main purpose of glove use`` showed the lowest correct answer (28.3%), followed by the basic concept of standard precautions` (34.6%). Preferred single prevention measure was ``hand hygiene`` (98.6%). 47.5% of the respondents answer ``know vaguely`` on the question about ``self assessment of knowledge on the concept of isolation precaution``. Preferred source of information on isolation precautions was searching for guideline (62.6%) followed by calling the hotline (24.8%). The most important obstacle to compliance with isolation precaution guideline was forgetfulness (65.4%), followed by lack of time (42.9%), lack of knowledge (34.3%), and lack of needs (2.6%). Conclusion: The knowledge on the isolation precautions among nursing students was low, therefore nursing students require fundamental and comprehensive courses in infection control precautions prior to taking part in the clinical practice. Considering forgetfulness was the most important obstacle to adhere to isolation precaution guideline, repeated education in the schools or hospitals are also recommended.

Purpose: This study identified the epidemiological characteristics, including the size and major strains, of carbapenemase-producing carbapenem-resistant Enterobacteriaceae (CP-CRE) and CP-CRE-related factors by comparing the characteristics of patients in the CP-CRE and non-CP-CRE groups and the CP-CRE and non-CRE groups. Methods: This secondary data analysis study included 24 patients in the CP-CRE group, 113 patients in the non-CP-CRE group, and 113 in the non-CRE group. The size and type of CP-CRE were analyzed in terms of frequency and percentage, and CP-CRE risk factors were identified using multiple logistic regression analysis. Results: The rate of CP-CRE positivity among patients with CRE was 17.5%, and the most common causative organism in the CP-CRE group was Klebsiella pneumoniae (81.8%). There were no significant differences between patients in the CP-CRE and non-CP-CRE groups. When compared with the non-CRE group, the isolation of multidrug-resistant organisms except for CRE, particularly vancomycin-resistant Enterococcus, was confirmed as a major risk factor. Conclusion: To prevent CP-CRE acquisition, patients with multidrug-resistant organisms require treatment with more thorough adherence to CRE prevention and management guidelines.

The "Sound Classification System on Floor Impact Sound Insulation Performance" is being studied in Korea. For that study,<br/> vocabularies appropriate for evaluation and reporting are needed.<br/> In this study, the extracted words such as annoyance, loudness, noisiness, sirritate, disagreeable, strident, disturbed, and dissonant are given to subjects in lab. psycho-acoustic experiment. And then, correlation analysis between the words and floor impact noise rating method are accomplished.<br/> As a result of this study, followings are suggested 'Irritate' is the word most accurately expressing the subjects' unpleasant feeling of domestic floor impact noise. The results of this study could be basic materials for psycho acoustic experiments for criteria on floor impact noise and Sound Classification on Floor Impact Sound Insulation Performance"

Purpose: This study aimed to identify the risk factors of carbapenem-resistant Enterobacteriaceae (CRE) acquisition to build a nomogram for CRE acquisition risk prediction and evaluate its performance. Methods: This unmatched case-control study included 352 adult patients (55 patients and 297 controls) admitted to the intensive care unit (ICU) of a 453-bed secondary referral hospital between January 1, 2018, and September 31, 2019, in Busan, South Korea. The nomogram was built with the identified risk factors using multiple logistic regression analysis. Its performance was analyzed using calibration-in-thelarge, the slope of the calibration plot, concordance statistic (c-statistic), and the sensitivity and specificity of the training set, subsets, and a new test set. Results: The risk factors of CRE acquisition among ICU patients at a secondary referral hospital were Acute Physiology and Chronic Health Evaluation II score at the time of admission, use of a central venous catheter and a nasogastric tube, as well as use of cephalosporin antibiotics. At 20.0% of the predicted CRE acquisition risk in the training set, the calibration-in-the-large was 0, slope of the calibration plot was 1, c-statistic was .93, sensitivity was 85.5%, and specificity was 84.8%. The performance was relatively good in the subsets and new test set. Conclusion: The nomogram can be used to monitor the CRE acquisition risk for ICU patients who have a similar case mix to patients in the study hospitals. Future studies need to involve more rigorous methodology and larger samples.
