Background and Objective We aimed to investigate whether viral anxiety affects insomnia severity in patients with insomnia disorder during the coronavirus disease 2019 (COVID-19) pandemic. In addition, we examined whether viral anxiety mediates the influence of dysfunctional beliefs about sleep on insomnia severity.Methods The medical records of 111 patients who visit the sleep clinic from September 2021 to May 2022 were reviewed. Patients’ symptoms were rated with rating scales including Insomnia Severity Index (ISI), Stress and Anxiety to Viral Epidemics-6 items (SAVE-6), Dysfunctional Beliefs and Attitudes about Sleep-16 items (DBAS-16), Patient Health Questionnaire-9 items (PHQ-9), Epworth Sleepiness Scale, and discrepancy between desired time in bed and desired total sleep time (the DBST index).Results The SAVE-6 score was not significantly correlated with ISI score among insomnia patients. Linear regression analysis showed that the ISI score was expected by PHQ-9 (β = 0.21, p = 0.038) and DBAS-16 (β = 0.42, p < 0.001). Mediation analysis showed that viral anxiety did not mediate the influence of dysfunctional beliefs about sleep on insomnia severity.Conclusions We could not observe the mediating effect of viral anxiety on the association between dysfunctional beliefs about sleep on insomnia severity. Though COVID-somnia was one of big issues in this COVID-19 pandemic, the influence of virus on the severity of insomnia among insomnia patients needs to be interpreted cautiously.

Ischiofemoral impingement syndrome is a rare clinical entity characterized by chronic groin, buttock or hip pain associated with radiographic evidence of narrowing of the space between the lesser femoral trochanter and the ischial tuberosity. Introduction of magnetic resonance imaging to the clinical practice as well as the establishment of the radiological definition of the abnormal ischiofemoral distance has led to an increasing interest in this condition. Ischiofemoral impingement syndrome is a poorly understood disorder of chronic pain, especially regarding its treatment. The authors present two cases of primary ischiofemoral impingement syndrome successfully treated with a minimally invasive surgical technique. With this endoscopic technique, it was possible to resect the lesser trochanter and restore the ischiofemoral space. Immediate clinical and functional improvement was reported by both patients.

In this study, the efficacy of strengthening of reinforced concrete (RC) beams in shear by utilizing high-performance fiber-reinforced concrete (HPFRC) was explored. The shear strengthening was achieved by epoxy bonding of prefabricated HPFRC strips or plates onto the beams. The beams were strengthened utilizing two different strengthening schemes: (i) plates side strengthening (ii) vertical strips applied at shear critical sections. The behavior of the two configurations was compared to the behavior of non-shear reinforced and shear-reinforced RC beams. The high-performance concrete (HPC) utilized contains 1.5% of steel fibers per volume of HPC mortar and is known as HPFRC. Parameters determined were the flexural strength and compressive strength of HPFRC mortar. The obtained results revealed that HPFRC realized a 28-day flexural strength of 20 MPa and a compressive strength of 108 MPa. Moreover, HPFRC strengthened RC beams experienced an increased in strength capacity of about 50% for plates and 36% for vertical strips compared to the RC beams with no stirrups. The results for HPFRC strengthened beams with plates were superior compared to those of the stirrup-reinforced beams, whereas the results of HPFRC strips strengthened beams were almost identical to the stirrup-reinforced beams. Also observed, was an improvement in the ductility of the beams with the best results achieved when employing HPFRC plates and strips.
Response of Double Angle and Shear Endplate Connections at Elevated Temperatures

Double angle and shear endplate connections are considered simple-beam end framing connections designed to carry gravity loads only. However, during a fire event, additional thermal induced axial forces might develop in the beam and cause failure of the connection. This paper presents the results of finite element (FE) simulations on double angle and shear endplate connections assemblies in fire. FE simulations are validated against experimental results of double angle and shear endplate connections at elevated temperature. A detailed comparison of the variation of the axial force during the heating and cooling phases of a fire is made between the two connections covering the following parameters: load ratio, beam length, and double angle and shear endplate thickness and location. The results can help future design guidelines to account for the thermal induced forces and deformations in double angle and shear endplate connections during a fire.

The association of left ventricular pseudoaneurysm (LVPA) formation to polycythemia vera (PV) is unknown and, to our knowledge, has never been reported. We describe the case of a 58- year-old man with PV who was admitted for acute dyspnea following an inferior myocardial infarction that occurred seven weeks prior to presentation. Multimodality imaging disclosed the presence of an LVPA. The patient refused surgical treatment and was readmitted three months later with acute decompensation. Follow-up imaging revealed increased LVPA. Review of the literature however showed no report of PV presenting as an LVPA that worsened in a follow-up admission. This rare association prompted a definitive diagnosis. In this case, multimodality imaging was crucial to establishing a definitive diagnosis and guiding therapy.

Eating behaviour disorder during early childhood is a common pediatric problem. Many terminologies have been used interchangeably to describe this condition, hindering implementation of therapy and confusing a common problem. The definition suggests an eating behaviour which has consequences for family harmony and growth. The recent Diagnostic and Statistical Manual of Mental Disorders, Fifth Edition does not cover the entire spectrum seen by pediatricians. Publications are substantive but level of evidence is most of the time low. This purpose of this review is to clarify terminology of eating behaviour problems during early childhood; including benign picky eating, limited diets, sensory food aversion, selective eating, food avoidance emotional disorder, pervasive refusal syndrome, tac-tile defensiveness, functional dysphagia, neophobia and toddler anorexia. This tool is proposed only to ease the clinical management for child care providers. Diagnostic criteria are set and management tools are suggested. The role of dietary counselling and, where necessary, behavioural therapy is clarified. It is hoped that the condition will make its way into mainstream pediatrics to allow these children, and their families, to receive the help they deserve.
