        The Longitudinal Trend of Cardiac Surgery in Korea from 2003 to 2013

        Background: The purpose of this study was to investigate longitudinal changes of the utilization of operational and surgical medical care inside and outside a metropolitan area over 10 years, analyzing the residential areas of patients and the locations of medical facilities for major cardiovascular surgery. Methods: Data analysis was conducted by classifying the addresses of patients and the locations of medical care facilities of metropolitan cities and provinces, using data from the National Health Insurance Corporation from January 2003 to December 2013. Results: There is serious concentration of major heart surgery to medical facilities in Seoul; this problem has not improved over time. There were differences in percentages of surgical procedures performed in the metropolitan areas according to major diseases. In the case of Busan and Daegu provinces, at least 50% of the patients underwent surgery in medical facilities in the city, but there are other regions where the percentage is less than 50%. In the case of provinces, the percentage of surgical procedures performed in medical facilities in Seoul or nearby metropolitan cities is very high. Conclusion: Policies to strengthen the regional capabilities of heart surgery and to secure human resources are required to mitigate the concentration of patients in the capital area. Many regional multi-centers must be designated to minimize unnecessary competition among regional university hospitals and activate a win-win partnership model for medical services.

      • Production of Cloned Mice by Aggregation of Tetraploid Em

        This study was investigated to optimize the efficiency of cloning and to produce the clone mice. Somatic cell nucleus transfer (SCNT) embryos were produced by injecting cumulus cell nucleus into enucleated B6D2F matured oocyte. Mouse chimeras can be also successfully produced with tetraploid host embryos. The fusion of mouse 2- cell embryos can be produced tetraploid embryos. Thus, we utilized the tetraploid embryos to increase the clone efficiency by aggregation with SCNT embryos of adult cumulus cells. SCNT embryos were found to be optimized for 6 hours activation with strontium (SrCl2). The cytochalasin B (CB) concentration (5 ug/ml) in the enucleation was evaluated in the efficiency of implantation sites and fetus offspring. Trichostatin A (TSA), histonedeacetylase inhibitor (HDACi), is recommended to production of clone mice, continuously exposed in 5~50 nM for 10 hours. And we also improved the SCNT implantation rate/ live offspring by co-transfer with parthenogenic embryo in uterus of the other site. Next, the aggregated SCNT were constructed by aggregation of SCNT embryo with tetraploid embryos to reduce epigenetic error in placenta. The pregnancy and implantation rates of aggregated SCNT were significantly higher than SCNT alone. The full term developmental rate in aggregated embryo was slightly higher than that of SCNT (3.57 vs 1.16). The placental weight of SCNT clone was significantly higher than that of in vitro fertilization as shown in previously reported. However, the placenta weight was almost reduced to IVF group in the aggregated SCNT. It was shown to be typical hyperplastic histology of mouse clones. But the aggregated SCNT method was useful to significantly reduce the placental weight known as general problems in cloned mice.

        Ride-through of PMSG Wind Power System Under the Distorted and Unbalanced Grid Voltage Dips

        This paper presents a ride-through skill of PMSG wind turbine system under the distorted and unbalanced grid voltage dips. When voltage dips occur in the grid, pitch control and generator speed control as well as a parallel resistor of DC-link help to keep the turbine's safety. Modern grid code requires a wind turbine to supply reactive currents to help voltage recovery after grid faults clearance. In order to supply reactive currents to the grid in case of the distortedly unbalanced grid voltage dips, a special PLL is needed to control the grid side converter and to regulate the grid voltages symmetrically. The proposed method is applied to 2MW multi-pole PMSG wind turbine system, and verified by simulation.

      • Hollow Abutment Screw Design for Easy Retrieval in Case of Screw Fracture in Dental Implant System

        <P>The prosthetic component of dental implant is attached on the abutment which is connected to the fixture with an abutment screw. The abutment screw fracture is not frequent; however, the retrieval of the fractured screw is not easy, and it poses complications. A retrieval kit was developed which utilizes screw removal drills to make a hole on the fractured screw that provides an engaging drill to unscrew it. To minimize this process, the abutment screw is modified with a prefabricated access hole for easy retrieval. This study aimed to introduce this modified design of the abutment screw, the concept of easy retrieval, and to compare the mechanical strengths of the conventional and hollow abutment screws by finite element analysis (FEA) and mechanical test. In the FEA results, both types of abutment screws showed similar stress distribution in the single artificial tooth system. A maximum load difference of about 2% occurred in the vertical load by a mechanical test. This study showed that the hollow abutment screw may be an alternative to the conventional abutment screws because this is designed for easy retrieval and that both abutment screws showed no significant difference in the mechanical tests and in the FEA.</P>

        Enhancing the Biological Control of Rice Seedling Disease by Adding Specific Carbon Sources into the Bacillus cereus D324 Formulation in Water-Seeded Rice

        Utilization of carbon sources by Bacillus cereus D324, a biological control agent, and Pythium species, which causes rice seedling disease, was studied with the objective of increasing the efficacy of biological control by providing the biological control agent with specific beneficial carbon sources. D-galactose, D-sorbitol, and D-mannitol were poor carbon sources for Pythium spp. growth but were good for B. cereus D324 growth. Growth in a growth chamber of rice seeds coated with B. cereus D324 amended with specific carbon sources, such as D-galactose and D-sorbitol, showed significantly enhanced seedling emergence compared to seeds coated only with B. cereus D324. Field trials showed that both seedling emergence and yield increased, when the above specific carbon sources were added to B. cereus D324 in seed coating formulations. This result indicated that amending seed coating formulations with specific carbon sources could significantly increase seedling emergence and yield in the field.

      • Spatio-temporal Changes of Sediment Environment in the Taehwa River Estuary, Ulsan of Korea.

        <P>The Taehwa River is a successful case for restoration of ecological system in Korea through the restoration project conducted during 1995-2005. To sustain this restored ecosystem, seasonal surveys for surface sediment quality were conducted in the entire area of the river estuary during the year 2012. It is noteworthy that the areas with pollutant source inflow exhibited higher organic carbon and acid volatile sulphide concentrations than the pollutant standard indices (20 mg C.g(-1) and 0.2 mg S.g(-1)) in the spring and summer. Trace metal concentrations (As: 13.2; Cu: 64.2; Zn: 185.7; and Pb: 48.3; mg.kg(-1)) also showed a considerable increment from their respective Effect Range Low standards. The observed sediment oxygen demand (avg. 41.13 mmol O(2)m(-2).d(-1)) in the prime habitat of Ruditapes philippinarum was substantially higher in this study than the average value (16 +/- 6 mmol O(2)m(-2).d(-1)) in Southern coastal area of Korea. The results from materials flux study suggest that high phosphate release from sediment into water column in summer and low sedimentation rate of nitrogenous nutrients during fall season could influence prolonging algal blooms in the Taehwa estuary. A further study is needed for a long-term of sediment monitoring on environment quality changes to maintain the restored ecological system and a safe consume of seafood.</P>

      • Performance Estimation of a Louver Dust Collector Attached to the Bottom of a Subway Train Running in a Tunnel

        <P> In underground tunnels, friction between the wheels and rails of subway trains creates particles, which are spread by the wind generated when trains pass by. A louver dust collector was attached to the bottom of a T-car of Seoul Subway Line 5 train in an effort to remove PM<SUB>10</SUB> inside tunnels, and obtain data when it was in actual operation. It made several round trips during which differential pressure of the louver dust collector was measured in relation to train speed. By comparing and verifying the differential pressure estimated by simulation and that actually measured, it was possible to estimate average flow of air that went into the louver dust collector. Furthermore, by comparing and verifying the measurement results on collection efficiency of a lab-scale louver dust collector in a wind tunnel, along with the results of numerical analysis, it was possible to estimate the collection efficiency in relation to subway train speed. As a result, it was confirmed that higher running speeds of subway trains increased the flow of air going into the louver dust collector and subsequently decreased the particle size corresponding to 50% collection efficiency. In other words, the cut-off size was estimated to be 9.7 μm at the lowest speed of 5 km h<SUP>-1</SUP>, and 4.9 μm at the top speed of 65 km h<SUP>-1</SUP>, in normal speed range for the Seoul Subway Line 5 trains. </P>

        Novel-1,3-Alternate Calix[4]thiacrown Ethers

        A series of novel 1,3-alternate calix[4]-mono-thiacrown and -bis-thiacrowns were synhesized within good yields. The three dimensional 1,3-alternate conformation was confimed by X-ray crystal structure. From the results of two phase extraction and NMR studies on the ligand-metal complex, one can conclude that the calix[4]thiacrown ethers showed the very high selectivity for silver ion over other metal ions by an electrostatic interaction between sulfur atom and silver ion and by a $\pi-metal$ complexation.

      • FCP 6 : Sentinel lymph node biopsy of the 104 malignant melanoma of KNUH over 9 years (2006~2014)

        Background: In these days, Sentinel lymph node biopsy (SLNB) is used as the most sensitive and specific staging tool available for determination of disease status of regional lymph node and it also developed as a technique to surgically assess the regional lymph nodes i.e. sentinel lymph node(SLN) and allowed to spare node-negative patients unnecessary and potentially morbid complete lymphadenectomies. Objectives: To examine the clinical analysis of SLNB cases in Kyungpook national university hospital (KNUH) over 9 years. Methods: In the period from January 2006 to January 2014, a SLNB was performed in 104 patients under cooperation of the department of dermatology, general surgery or orthopedic surgery in KNUH. Data were collected from medical records and the melanoma registry of KNUH. Results: Of the 104 melanoma patients who underwent SLNB, 49 were males (47.1%), 55 were females (52.9%) and their ages ranged from 34~93 years (average: 60.9). Detection rate of SLN was 98.1% (102/104). Twenty nine patients had a tumor-positive SLNB (27.8%) and underwent completion regional lymph node dissection. The mean Breslow thickness of the primary tumors was 3.76 mm, and the proportion of ulcerated melanomas was 51.4%. Two side effects had been observed such as seroma (1case) and lymphocele (1case). Conclusion: Herein, we reviewed the importance of SLNB as prognostic and staging tools with minimal morbidity through our experience of 104 malignant melanoma cases during past 9 years.
