Introduction: After surgery of liver or biliary tract, bile congestion is easily happened because of anatomical change of bile duct. We experienced relatively rare case of intrahepatic bile duct stones caused by foreign body left behind bile duct after previous bile duct surgery. Case report: A 84-year-old male patient came to our hospital due to two weeks of fever and chill. He underwent left lobectomy with hepaticojejunostomy for treatment of cholangiocarcinoma 10 years ago. Abdominal CT revealed multiple intrahepatic duct stones complicated with cholangitis and long, wire-like materials lied from IHBD to jejunum. Cholangitis were improved after PTBD and antibiotic treatment. Percutaneous transhepatic cholangioscopy was performed for treatment of bile duct stones. Endoscopic fi nding revealed numerous black stones occupying intrahepatic bile ducts and two black colored, long tube-like foreign bodies along the bile duct and jejunum. After removing stones with PTCS directed electrohydraulic lithotripsy, the foreign body was extracted with dormia basket through PTBD tract. The foreign body was identifi ed long plastic catheter. Conclusion: Foreign body in bile duct can induce abnormal fiow of bile juice and consequently the gallstone or sludge can be made. In this case, plastic catheter, the responsible cause of IHBD stone, thought to be placed from IHBD to jejunum for bile drainage after hepaticojejunostomy. But after clogging of the stent, the catheter caused the bile stasis and stone formation. So we report a rare case of foreign body induced intrahepatic bile duct stone treated by percutaneous transhepatic cholangioscopy.
Water Saving by Shallow Intermittent Irrigation and Growth of Rice

<P>To reduce water requirement and improve water productivity (the grain yield per unit volume of water irrigated) by water-saving irrigation techniques, we examined the effects of very shallow intermittent irrigation (VSII, 2cm), shallow intermittent irrigation (SII, 4cm) and traditional deep water irrigation (DWI, 10cm) on rice growth and yield in the field for two years. The amount of water irrigation during the rice-growing period (average of two years) was 318, 391 and 469 mm in VSII, SII and DWI, respectively. Rice growth and grain yield were not significantly influenced by the treatments. As the irrigation water input decreased, the water productivity increased. The water productivity increased by 46 % in VSII and 20 % in SII on the average as compared with DWI. The shallower the irrigation depth, the lower the breaking weight and the higher the lodging resistance, and the deeper the roots in the paddy soil. In DWI, the percentage of head rice was lower and the protein content was higher, suggesting deterioration in the palatability of cooked rice due to the increase of chalky rice. The water-saving rate was 32.9 % in VSII and 17.2 % in SII as compared with typical deep water irrigation in Korea.</P>

Background/Aims: Small rectal neuroendocrine tumors (NETs) can be treated with modified endoscopic mucosal resection (EMR). However, an optimal EMR method remains to be established. We aimed to assess the non-inferiority of Tip-in EMR versus precut EMR (EMR-P) for treating rectal NETs. Methods: This prospective, multicenter, randomized controlled trial enrolled patients with rectal NETs of < 10 mm in diameter. The patients were randomly assigned to EMR-P and Tip-in EMR groups in a 1:1 ratio. Primary outcome was margin-negative (R0) resection rate between the two methods, with a noninferiority margin of 10%. Results: Seventy-five NETs in 73 patients, including 64 eligible lesions (32 lesions in each, EMR-P and Tip-in EMR groups), were evaluated. In a modified intention-to-treat analysis, R0 resection rates of the EMR-P and Tip-in EMR groups were 96.9% and 90.6%, respectively, which did not demonstrate non-inferiority (risk difference, -6.3 [95% confidence interval: -18.0 to 5.5]). Resection time in the EMR-P group was longer than that in the Tip-in EMR group (p < 0.001). One case of intraprocedural bleeding was reported in each group. Conclusions: We did not demonstrate the non-inferiority of Tip-in EMR compared to EMR-P for treating small rectal NETs. However, the R0 resection rates for both techniques were high enough for clinical application.

<P>We propose a control methodology to precisely manipulate a self-positioning and rolling magnetic microrobot (SPRMM) along a complex moving path on a 3D thin surface. We introduce a closed-loop algorithm to control an external rotating magnetic field in order to precisely manipulate the SPRMM along a predefined arbitrary moving path. We also construct a biplane, vision-based image processing procedure to obtain the real-time 3D position of the SPRMM. We then construct an experimental setup to demonstrate precise rolling locomotion of the SPRMM on a 3D, complex thin surface.</P>

35 years-old male admitted in ED with stabbing on his anterior chest with knife. Thoracic surgery team undergone emergency operation which revealed cardiac tamponade with hematoma, pleural laceration and RVOT laceration that was close to LAD, but couldn`t be checked exactly because of blood pumping . So direct closure was done, but cardiac enzyme was markedly elevated after that. Follow-up echocardiography showed a VSD at RVOT, chest CT angiography confi rmed that is not ischemic VSD but a cardiac penetrating wound. So operation of VSD closure was done. We experienced a traumatic VSD on the unusual location after cardiac penetrating stab wound. But through mutually compensated imaging approach by echocardiography and CT scan, we could found that. So, when the stab wound injury especially on the chest around the heart, it need to perform serial echocardiographic exams, and then should be compensated by CT scan or MR imaging.
한외여과의 활성탄 흡착에 의한 분뇨처리 방류수의 고도처리

Hybrid process coupling ultrafiltration and adsorption was demonstrated to treat effluents from the night soil treatment to high quality product. The product water treated by hybrid process could be within the reinforced basis of effluent qualities. Removal efficiencies for COD, SS, T-N, T-P, TOC, and turbidity were 13.3%, 100%, 18.5%, 18.0%, 21.3%, and 80.7% by ultrafiltration, and 37.3%, 100%, 35.8%, 37.0%, 81.3%, and 84.9% by hybrid process. The performance for removal efficiencies was not nearly affected by run time. The permeate flux at the transmembrane pressure difference of 2.0Kg_f/㎠ was 49.8L/㎡/hr at initial operation, and decreased to 26.2L/㎡/hr after 7days, but it was restored to 99% of initial flux by chemical cleaning of organic foulants. It is expected that hybrid process coupling ultrafiltration and adsorption can be applied to treat effluents from the night soil treatment.
