        The Impact of Trimetazidine Treatment on Left Ventricular Functions and Plasma Brain Natriuretic Peptide Levels in Patients with Non-ST Segment Elevation Myocardial Infarction Undergoing Percutaneous Coronary Intervention

        Background and Objectives: The aim of this study was to investigate the impact of treatment with oral trimetazidine (TMZ) applied before and after percutaneous coronary interventions (PCI) on short-term left ventricular functions and plasma brain natriuretic peptide (BNP) levels in patients with non-ST segment elevation myocardial infarction (NSTEMI) undergoing PCI. Subjects and Methods: The study included 45 patients who were undergoing PCI with the diagnosis of NSTEMI. The patients were ran-domized into two groups. The first group (n=22) of the patients hospitalized with the diagnosis of NSTEMI was given conventional therapy plus 60 mg TMZ just prior to PCI. Treatment with TMZ was continued for one month after the procedure. TMZ treatment was not given to the second group (n=23). Echocardiography images were recorded and plasma BNP levels were measured just prior to the PCI and on the 1st and 30th days after PCI. Results: The myocardial performance index (MPI) was greater in the second group (p=0.02). In the comparison of BNP levels, they signifi-cantly decreased in both of the groups during the 30-day follow-up period (29.0±8 and 50.6±33, p<0.01 respectively). However, decreasing of BNP levels was higher in the group administered with TMZ. The decrease of left ventriclular end-diastolic volume was observed in all groups at 30 days after intervention, but was higher in the group administered with TMZ (p=0.01). Conclusion: Trimetazidine treatment commencing prior to PCI and continued after PCI in patients with NSTEMI provides improvements in MPI, left ventricular end diastolic volume and a decrease in BNP levels.

        A Voltage Regulation System for Independent Load Operation of Stand Alone Self-Excited Induction Generators

        In recent years, some converter structures and analyzing methods for the voltage regulation of stand-alone self-excited induction generators (SEIGs) have been introduced. However, all of them are concerned with the three-phase voltage control of three-phase SEIGs or the single-phase voltage control of single-phase SEIGs for the operation of these machines under balanced load conditions. In this paper, each phase voltage is controlled separately through separated converters, which consist of a full-bridge diode rectifier and one-IGBT. For this purpose, the principle of the electronic load controllers supported by fuzzy logic is employed in the two-different proposed converter structures. While changing single phase consumer loads that are independent from each other, the output voltages of the generator are controlled independently by three-number of separated electronic load controllers (SELCs) in two different mode operations. The aim is to obtain a rated power from the SEIG via the switching of the dump loads to be the complement of consumer load variations. The transient and steady state behaviors of the whole system are investigated by simulation studies from the point of getting the design parameters, and experiments are carried out for validation of the results. The results illustrate that the proposed SELC system is capable of coping with independent consumer load variations to keep output voltage at a desired value for each phase. It is also available for unbalanced consumer load conditions. In addition, it is concluded that the proposed converter without a filter capacitor has less harmonics on the currents.

      • Thyroid Nodules with Atypia or Follicular Lesions of Undetermined Significance (AUS/FLUS): Analysis of Variables Associated with Outcome

        Background: The Bethesda System for Reporting Thyroid Cytopathology is one of the main classification systems for thyroid nodules. It expects that 7% of all fine needle aspiration biopsies will be reported as atypia or follicular lesions of undetermined significance, and 5-15% of these undetermined nodules are malignant. Our study is a retrospective analysis of variables that may be associated with outcome in patients with indeterminate thyroid nodules. Materials and Methods: Patients who underwent thyroidectomy in our institution between 2010 and 2014 were retrieved from the institutional records database. Patient demographics and medical histories were recorded. All ultrasonography reports were examined for nodule features and biochemical blood levels, hormone levels and complete blood counts were recorded. Results: A total of 103 patient cytopathology reports were regarded as belonging to the undetermined category. Some 35% of patients had malignant nodules. Median preoperative red cell distribution width (RDW) level was 13.6 inthe benign group, while it was 14.3 in patients with malignancy, demonstrating a significant correlation (p=0.003). Only calcification presence was significantly different between benign and malignant groups on ultrasonography (p=0.034). Conclusions: Ultrasonography is one of the primary tools for this matter. RDW levels may become another promising tool to predict malignancy.

        3D Magnetic Analysis of Permanent Magnets in Spherical Configuration

        The present study aims to increase the amount of surface flux by changing the magnetic directions of a spherical magnet (NdFeB) consisting of four poles. For this purpose, the magnetic directions of quartile spherical slices constituting the spherical magnet are manipulated and their threedimensional analyses are carried out by using finite-element method via Maxwell environment. The analysis of the magnetic quartile spheres with four different magnetic directions are compared to the each other, and then the quartile sphere with the best surface flux distribution is suggested for rotor structure. It is clear emphasized that the induced torque of the spherical motor, in which such a rotor is used, will be improved as well.

        Traumatic Coronary Artery Dissection in a Young Woman after a Kick to Her Back

        We present the case of a 38-year-old woman admitted to our outpatient clinic with accelerating back pain and fatigue following a kick to her back by her husband. Upon arrival, we detected ST segment elevation in the D1, aVL, and V2 leads and accelerated idioventricular rhythm. She had pallor and hypotension consistent with cardiogenic shock. We immediately performed coronary angiography and found a long dissection starting from the mid-left main coronary artery and progressing into the mid-left anterior descending (LAD) and circumflex arteries. She was then transferred to the operating room for surgery. A saphenous vein was grafted to the distal LAD. Since the patient was hypotensive under noradrenaline and dopamine infusions, she was transferred to the cardiovascular surgery intensive care unit on an extracorporeal membrane oxygenator and intra-aortic balloon pump. During follow-up, her blood pressure remained low, at approximately 60/40 mmHg, despite aggressive inotropic and mechanical support. On the second postoperative day, asystole and cardiovascular arrest quickly developed, and despite aggressive cardiopulmonary resuscitation, she died.

        Corticosteroid Therapy Improves the Outcome of Semen Analysis in an Oligozoospermic Patient With Epididymal Sarcoidosis

        Sarcoidosis is a multisystem, inflammatory disorder characterized by the presence of noncaseating epithelioid granulomas. Sarcoidosis can involve the genitourinary system by affecting the kidney and genitals. Most characteristic genital lesions proceed to granuloma and can comprise the epididymis, testis, and vas deferens. Few case reports have been published on this rare entity. We report a case in which a man presented with bilateral epididymal sarcoidosis and severe oligospermia. Corticosteroid treatment, which was applied in gradually decreasing doses for 6 months, dissolved the testicular granuloma. Consequently, semen analysis demonstrated a significant increase in the sperm count to within normal limits.

        Braking performance and noise in excessive worn brake discs coated with HVOF thermal spray process

        In this paper, two excessive worn brake discs of a light commercial vehicle were coated with High velocity oxygen fuel (HVOF) thermal spray process for reuse. The coated discs were compared with uncoated Original equipment manufacturer (OEM) D1 disc in terms of braking performance and noise. The D2 disc was coated with tungsten-carbide-cobalt (88 %WC-12 %Co) powder and NiCr (80/20) binder, and 500 µm thick coating was obtained. The D3 disc was coated with Colmonoy-88 (Ni-W-Cr-B-Si) powder and NiCr (80/20) binder and 600 µm thick coating was obtained. Several test procedures (e.g. bedding, vibration, and structural-strength tests) was applied to three discs using inertia brake dynamometer. Results showed that coated D2 disc with tungsten carbide cobalt provided lower brake noise and higher brake performance compared with OEM disc. Considering the coefficient of friction and temperature, coated D3 disc has approximately equal braking performance with OEM D1 disc despite the high braking noise value.

        Integrated Model of Power Electronics, Electric Motor, and Gearbox for a Light EV

        This study presents a model of a drivetrain for an integrated design of a light electric vehicle (EV). For the drivetrain of each front wheel of the single-person, battery-powered EV tricycle consists of a battery, an inverter, and an outer rotor permanent magnet synchronous motor (PMSM), which is connected to an in-wheel gearbox. The efficiency of the inverter, motor, and gearbox is analyzed over the New European Driving Cycle. To calculate the losses and efficiency of the PMSM, the power electronics in the inverter and gearbox are used. The analytical models provide a fast, but less accurate result, useful for optimization purposes. To accurately predict the efficiency of the PMSM, a finite element model is used. The models are validated by test setups. Correspondingly, a good agreement between the measurements and the calculated results is achieved. A parameter study is performed to investigate the influence of the detailed component parameters (i.e., outer rotor radius, gear ratio, and number of pole pairs and stator slots) on the average efficiency of the drivetrain.
