      • Correlation of Preoperative Ki67 and Serum CA15.3 Levels with Outcome in Early Breast Cancers - a Multi Institutional Study

        Background: To investigate the association between preoperative pathological Ki-67 labeling index and serum tumor marker cancer antigen 15-3 (CA 15-3) with clinic-pathological parameters and treatment outcomes in early breast cancer. Materials and Methods: A retrospective study at 4 cancer centers in Saudi Arabia and Egypt was performed. Data were collected for female patients diagnosed with unilateral early breast cancer between March 2010 and October 2013. Cases treated with neoadjuvant chemotherapy (NACT) followed by surgery and radiotherapy were included. NACT included 6-8 cycles of anthracycline and taxane based regimens. Trastuzumab and hormonal treatments were added according to HER2 and hormone receptor status. Baseline serum CA15.3 and pathological Ki67 levels were evaluated and correlated with disease free survival (DFS) and overall survival (OS). Results: A total of 280 pts was included. The median age was 49 years (38-66 y) and median overall survival was 35 (20-38) months (mo). Estrogen receptors (ER), progesterone receptors (PR) and HER 2 receptors were positive in 233 (83.2%), 198 (70%) and 65 cases (23.2%), respectively. High preoperative Ki67 and CA15.3 were noted in 177 (63.2%) and 131 (46.8%). A total of 45 (16%) patients had distal or local recurrence and 24 (8.6%) died of their disease. Most of the relapsed cases had high preoperative Ki-67 (n=41, 91%) and CA15.3 (n=28, 62%) values. All of the patients who died had a high Ki-67 but CA15.3 was high in 9 (37%) only. Mean DFS/OS in patients with high preoperative Ki-67 was 32 months /32 months as compared to 37 months/35 months in those with normal Ki-67 (p<0.001). Correlation of preoperative CA15.3 and survival was statistically not significant. Conclusions:Preoperative Ki-67 can be a predictive and prognostic marker. Higher levels are associated with poor DFS and OS in patients with early BC.

      • Predictive Value of IHC4 Score for Pathological Response to Neoadjuvant Chemotherapy in Hormone Receptor-Positive Breast Cancer

        Purpose: This study aimed to explore the value of IHC4 in predicting pathological response after neoadjuvant chemotherapy in patients with hormonal receptor (HR)-positive breast cancer (BC). Materials and Methods: In this retrospective exploratory study, data for 68 HR-positive BC patients who received neoadjuvant chemotherapy were recorded. IHC4 scores were calculated based on estrogen receptors/progesterone receptors, Ki-67 and HER2 status. Logistic and ordinal regression analyses in addition to likelihood ratio test were used to explore associations of IHC4 scores and other clinico-pathological parameters with pathological complete response (pCR) and pathological stage. Results: Taking the 25th percentile as the cut-off, a lower IHC4 score was associated with an increased probability of pCR (low; 52.9% vs. High; 21.6%, OR=4.1, 95% CI=1.28-13.16, p=0.018) and a lower pathological stage (OR=3.9, 95% CI=1.34-11.33, p=0.012). When the IHC4 score was treated as a continuous variable, a lower score was again associated with an increased probability of pCR (OR=1.010, 95% CI=1.001-1.018, p=0.025) and lower pathological stage (OR=1.009, 95% CI=1.002-1.017, P=0.008). Lower clinical stage was associated with a better pCR rate that was of borderline significance (P=0.056). When clinical stage and IHC4 score were incorporated together in a logistic model, the likelihood ratio test gave a P-value of 0.004 after removal of the IHC4 score and 0.011 after removal of the stage, indicating a more significant predictive value of the IHC4 score for pCR. Conclusions: This study suggests that the IHC4 score can predict pathological response to neoadjuvant chemotherapy in HR-positive BC patients. This finding now needs to be validated in a larger cohort of patients.

        Micro-grid Monitoring and Supervision: Web-based SCADA Approach

        Owing to the widespread use of the micro-grid concept to serve many real life applications, the main concern of this paper is to monitor, evaluate and manage the operational performance of an existent, already installed micro-grid that consists of On & Off grid PV systems in addition to the main grid supply. With the aid of customized web based SCADA system fully designed by authors, the performance evaluation of the proposed micro-grid is carried out in terms of energy consumption, power quality indices and energy cost based on Egypt’s energy current tariff . The designed system helps in both micro-grid status monitoring as well as load sharing between the main grid supply and the off grid PV energy system. The data of the on grid PV energy system are accessed and collected online directly from the inverter web server instead of downloading them offl ine from the PV inverter. In this case, data are analyzed with a reasonable refresh rate, which saves time and eff ort. Remote access platform is developed securely through a multi-level security authentication and strong data encryption using the OpenVPN protocol under the Dynamic Domain Name System service. Smart wired and wireless technology—used as integrated sensing devices to the developed SCADA system—enables easy system installation and running cost saving. Finally, to validate the collected data from the newly SCADA designed monitoring system, a performance characterization for the on grid PV system following international standards of yields calculations is carried out for 3 months in 2019.

      • Improving Analysis Phase in Network Forensics By Using Attack Intention Analysis

        The increasing amount of cyber crimes has motivated network forensics researchers to develop new techniques to analyze and investigate these crimes. Reconstructing useful evidence of a cybercrime is difficult due to the vagueness of the analysis phase processes. The analysis phase is challenging because it provides detailed information on the intention and strategy of the attack. This paper aims to show the importance of reconstructing attack intentions in order to improve the analysis phase in network forensics. Intentions are identified through an algorithm called Attack Intention Analysis, which predicts cyber crime intentions by combining mathematical evidence theory and a probabilistic technique. In this paper, the attack intention model will be improved to present the motivation behind cyber crimes. The results of the comparison of the attack intention analysis methods prove that the AIA algorithm is more accurate.

        Social media has become a global means of communication in people's lives. Most people are using Twitter for communication purposes and its inappropriate use, which has negative effects on people's lives. One of the widely common misuses of Twitter is cyberbullying. As the resources of dialectal Arabic are rare, so for cyberbullying most people are using dialectal Arabic. For this reason, the ultimate goal of this study is to detect and classify cyberbullying on Twitter in the Arabic context in Saudi Arabia. To help in the detection and classification of tweets, Pointwise Mutual Information (PMI) to generate a lexicon, and Support Vector Machine (SVM) algorithms are used. The evaluation is performed on both methods in terms of the F1-score. However, the F1-score after applying the PMI is 50%, while after the SVM application on the resampling data it is 82%. The analysis of the results shows that the SVM algorithm outperforms better.

        Volumetric analysis of normal condyles and those with disc displacement with reduction in the Indonesian population: A CBCT study

        Purpose: Disc displacement can cause resorption of the head of the condyle and affect its volume. This study analysed the volume of normal condyles and those with disc displacement with reduction (DDR) in cone-beam computed tomography (CBCT) scans from the Indonesian population. Materials and Methods: This study analysed 56 condyles (26 normal and 30 with DDR) from patients who visited the Oral and Maxillofacial Radiology Unit after being referred from the Prosthodontics Unit at Dental Hospital Universitas Padjadjaran from December 2020 to February 2021. Samples were divided into 2 groups (normal and DDR left and right-side condyles) based on the DC/TMD Axis 1 form through the clinical examination results. Both sample groups were exposed to CBCT radiation. The CBCT imaging results in the Digital Imaging and Communications in Medicine format were exported to the open-source ITK-SNAP format to determine condyle volume. Volumetric data from the cortical and trabecular areas of the right or left side condyles were arranged by sex. The independent t-test was used to determine the significance of differences with IBM SPSS version 21.0. Intra- and inter-observer reliability and validity were tested before determining the volume of the condyles. Results: Normal condyles and DDR condyles showed significant differences in volume (P<0.05). Significant differences were also seen in cortical (P = 0.0007) and trabecular (P = 0.0045) volumes. There was a significant difference in condylar volume based on sex. Conclusion: The normal condyle volume was significantly different from the DDR condyle volume in both sexes.

        Costs and Operational Revenue, Loan to Deposit Ratio Against Return on Assets: A Case Study in Indonesia

        This study aims to examine the effect of Operating Costs and Income, Loan to Deposit Ratio on the Return on Asset (ROA) of Public-Private Foreign Exchange Banks listed on the Indonesia Stock Exchange (IDX) during the 2015–2018 period. This study is a quantitative study using financial reports of Public-Private Foreign Exchange Banks listed on the IDX as a data source. This study’s population is 25 Public-Private Foreign Exchange Banks listed on the IDX. This study uses purposive sampling to determine the sample to produce 21 banking companies. Data was analyzed using multiple linear regression methods and descriptive statistics. The F Test calculation results state that all the variables of free operating expenses, operating income, and the loan to deposit ratio simultaneously and significantly affect the return on assets (ROA) variable in Public-Private Foreign Exchange Banks listed on the IDX. This study’s results indicate that simultaneously Operational Costs, Operational Income, and Loan to Deposit Ratio have a significant effect on ROA. Operational Costs and Operational Income have a significant negative impact on Return on Assets. The third hypothesis shows that the Loan to Deposit Ratio has a positive and insignificant effect on Return on Assets.

        Effects of Resistant Starch Supplementation on Glucose Metabolism, Lipid Profile, Lipid Peroxidation Marker, and Oxidative Stress in Overweight and Obese Adults: Randomized, Double-Blind, Crossover Trial

        Obesity is a substantial public health challenge across the globe. The use of resistant starch has been proposed as a probable management strategy for complications of obesity. We investigated the effects of resistant starch intake on lipid profiles, glucose metabolism, antioxidant status, lipid peroxidation marker, blood pressure, and anthropometric variables in subjects with overweight or obesity. In this 12-week, randomized, double-blind, placebo-controlled, 2 × 2 crossover trial, 21 Participants (mean age, 35 ± 7.0 years; body mass index, 32.4 ± 3.5 kg/m2) were given 13.5 g Hi-Maize 260 or placebo daily for 4 weeks, separated by a 4-week washout period. Changes in total antioxidant status (p = 0.04) and serum concentrations of insulin in 52.4% participants with insulin levels above 16 μIU/mL at the baseline (p = 0.04) were significantly different in the three phases. In addition, the mean of serum high-density lipoprotein cholesterol after the intervention was significantly higher than after baseline value (p = 0.04). We found no significant differences in serum concentrations of total cholesterol, triacylglycerol, low-density lipoprotein cholesterol, fasting blood sugar, insulin, homeostatic model assessment of insulin resistance, quantitative insulin sensitivity check index, superoxide dismutase activity, malondialdehyde, blood pressure, and anthropometric variables in the three phases of baseline, after intervention with resistant starch and after placebo. Resistant starch consumption improved serum insulin concentrations, lipid profiles, and antioxidant status in subjects with overweight or obesity.         Potentiation of cefotaxime against methicillin-resistant Staphylococcus aureus (MRSA) with the application of a novel adjuvant

        One of the current strategies to overcome methicillin-resistant Staphylococcus aureus (MRSA) and to minimise the need for new antibiotics is the application of adjuvant in antibiotic therapy. In the present work, cefotaxime (CTX), a third-generation antibiotic was successfully incorporated into a nanofiber mat and applied in combination with an adjuvant including boric acid (BA), borax (BX), and phenylboronic acid (PBA). The antibacterial activity of the developed formulations against Staphylococcus aureus strains including methicillin-susceptible S. aureus MSSA (ATCC® 25923™), oxacillin-sensitive S. aureus OSSA (ATCC® 29213™) and methicillin-resistant S. aureus MRSA (ATCC® 43300™) were evaluated by Kirby–Bauer disc diffusion and minimum inhibitory concentration (MIC) methods. PBA markedly decreased the MIC of CTX from 16 µg ml-1 to 0.125 µg ml-1 leading to a 128-fold reduction in its MIC value. BA and BX demonstrated a 16-fold and an 8-fold reduction in the MIC value of CTX, respectively, when used for the treatment of MRSA ATCC 43300. The combination of CTX with any of the available commercial ß-lactamase inhibitors (clavulanic acid, sulbactam, tazobactam) did not produce an effect against MRSA ATCC 43300. Cytotoxicity was assessed using MTT assay on human umbilical vein endothelial cells (HUVECs) and the results showed no statistically significant toxic effect for the developed formulations. These novel findings suggest that PBA, a non-toxic and inexpensive compound, possesses structural similarities with β-lactam and may represent promising molecules to reverse resistance to CTX. This method can overcome the need to develop new antibiotics and render them more effective against MRSA.
