        Vibration upshot of operating mechanical sewing machine

        Background: Most of the people associated with tailoring occupation in Nepal are still using mechanical sewing machine as an alternative of new technology for tailoring. Common peroneal nerves of both right and left legs are exposed to strenuous and chronic stress exerted by vibration and paddling of mechanical sewing machine. Methods: The study included 30 healthy male tailors and 30 healthy male individuals. Anthropometric variables as well as cardio respiratory variables were determined for each subject. Standard Nerve Conduction Techniques using constant measured distances were applied to evaluate common peroneal nerve (motor) in both legs of each individual. Data were analyzed and compared between study and control groups using Man Whitney U test setting the significance level p ≤ 0.05. Results: Anthropometric and cardio respiratory variables were not significantly altered between the study and control groups. The Compound muscle action potential (CMAP) latency of common peroneal nerves of both right [(11.29 ± 1.25 vs. 10.03 ± 1.37), P < 0.001] and left [(11.28 ± 1.38 vs. 10.05 ± 1.37), P < 0.01] legs was found to be significantly prolonged in study group as compared to control group. The Amp-CMAP of common peroneal nerves of both right [(4.57 ± 1.21 vs. 6.22 ± 1.72), P < 0.001] and left [(4.31 ± 1.55 vs. 6.25 ± 1.70), P < 0.001] legs was found significantly reduced in study group as compared to control group. Similarly, the motor nerve conduction velocity (MNCV) of common peroneal nerves of both right [(43.72 ± 3.25 vs. 47.49 ± 4.17), P < 0.001] and left [(42.51 ± 3.82 vs. 46.76 ± 4.51), P < 0.001] legs was also found to be significantly reduced in study group in comparison to control group. Conclusion: Operating mechanical sewing machine by paddling chronically and arduously could have attributed to abnormal nerve conduction study parameters due to vibration effect of the machine on right and left common peroneal nerves. The results of present study follow the trend towards presymptomatic or asymptomatic neuropathy similar to subclinical neuropathy.

        Approximate solution of non-linear dynamic energy model for multiple effect evaporator using fourier series and metaheuristics

        This article presents the approximate solution of non-linear dynamic energy model of multiple effect evaporator (MEE) using Fourier series and metaheuristics. The dynamic model of MEE involves first-order simultaneous ordinary differential equations (SODEs). Prior to solving the dynamic model, the non-linear steady-state model is solved to obtain the optimum steady-state process parameters. These process parameters serve as the initial conditions (constraints) for the SODEs. The SODEs are exemplified as an optimization problem by the weighted residual function to produce their approximate solutions. The optimization task is to find the best estimates of unknown coefficients in the Fourier series expansion using two preeminent metaheuristic approaches: Particle swarm optimization and harmony search. Besides, the influence of the number of approximation terms in Fourier series expansion on the accuracy of the approximate solutions has been investigated. The solution of the dynamic model assists in the investigation of open-loop dynamics of the MEE. Moreover, the acquired results may assist in designing suitable controllers to ensure energy-efficient performance of MEE and to monitor the product quality. The optimization results reveal that both the metaheuristic approaches offer minimum violation of the constraints and, therefore, validate their efficiency in solving such complex non-linear energy models.

        “Targeted” prophylaxis: Impact of rectal swab culture-directed prophylaxis on infectious complications after transrectal ultrasound-guided prostate biopsy

        Purpose: To assess the prevalence of fluoroquinolone resistance among patients undergoing transrectal ultrasound (TRUS)-guided prostate biopsy and the impact of rectal swab culture-directed antibiotic prophylaxis on postbiopsy infectious complications. Materials and Methods: We prospectively analyzed all patients undergoing TRUS-guided prostate biopsy from April 2013 to February 2015. Antibiotic prophylaxis was tailored to the results of rectal swab cultures. If the organism was fluoroquinolone-sensitive, oral ciprofloxacin 500 mg with tinidazole 600 mg was prescribed. If the organism was fluoroquinolone-resistant, then a culture-directed antibiotic was prescribed. In both cases the antibiotic was continued for 3 days. All patients were followed for 14 days after biopsy to record infectious complications. Results: A total of 247 patients were included, and Escherichia coli was isolated on rectal swab cultures in 99.5% of the patients. Of these, 41.7% harbored fluoroquinolone-resistant E. coli. Piperacillin/tazobactam was the most common culture-directed antibiotic prescribed (59.3%), with amoxicillin/clavulanic being the second most common (25.5%) for the fluoroquinolone-resistant group. Only 2 patients (0.9%) developed postbiopsy fever and none had sepsis. Conclusions: Colonization of rectal flora with fluoroquinolone-resistant E. coli was seen in 40% of men undergoing prostate biopsy. Targeted prophylaxis, which uses the results of prebiopsy rectal swab culture to direct antibiotic prophylaxis, results in low rates of postbiopsy infections.

        Speech Recognition using Machine Learning

        Speech recognition is one of the fastest-growing engineering technologies. It has several applications in different areas, and provides many potential benefits. A lot of people are unable to communicate due to language barriers. We aim to reduce this barrier via our project, which was designed and developed to achieve systems in particular cases to provide significant help so people can share information by operating a computer using voice input. This project keeps that factor in mind, and an effort is made to ensure our project is able to recognize speech and convert input audio into text; it also enables a user to perform file operations like Save, Open, or Exit from voice-only input. We design a system that can recognize the human voice as well as audio clips, and translate between English and Hindi. The output is in text form, and we provide options to convert audio from one language to the other. Going forward, we expect to add functionality that provides dictionary meanings for Hindi and English words. Neural machine translation is the primary algorithm used in the industry to perform machine translation. Two recurrent neural networks used in tandem to construct an encoder-decoder structure are the architecture behind neural machine translation. This work on speech recognition starts with an introduction to the technology and the applications used in different sectors. Part of the report is based on software developments in speech recognition.

        Prognostic Factors in Patients Hospitalized with Diabetic Ketoacidosis

        Background: Diabetic ketoacidosis (DKA) is characterized by a biochemical triad of hyperglycemia, acidosis, and ketonemia. This condition is life-threatening despite improvements in diabetic care. The purpose of this study was to evaluate the clinical andbiochemical prognostic markers of DKA. We assessed correlations in prognostic markers with DKA-associated morbidity andmortality. Methods: Two hundred and seventy patients that were hospitalized with DKA over a period of 2 years were evaluated clinically andby laboratory tests. Serial assays of serum electrolytes, glucose, and blood pH were performed, and clinical outcome was noted aseither discharged to home or death. Results: The analysis indicated that significant predictors included sex, history of type 1 diabetes mellitus or type 2 diabetes mellitus,systolic blood pressure, diastolic blood pressure, total leukocyte count, Acute Physiology and Chronic Health Evaluation II(APACHE II) score, blood urea nitrogen, serum creatinine, serum magnesium, serum phosphate, serum osmolality, serum glutamicoxaloacetic transaminases, serum glutamic pyruvic transaminases, serum albumin, which were further regressed and subjected tomultivariate logistic regression (MLR) analysis. The MLR analysis indicated that males were 7.93 times more likely to have favorableoutcome compared with female patients (odds ratio, 7.93; 95% confidence interval, 3.99 to 13.51), while decreases in meanAPACHE II score (14.83) and serum phosphate (4.38) at presentation may lead to 2.86- and 2.71-fold better outcomes, respectively,compared with higher levels (APACHE II score, 25.00; serum phosphate, 6.04). Conclusion: Sex, baseline biochemical parameters such as APACHE II score, and phosphate level were important predictors of theDKA-associated mortality.
