        Ginsenosides as dietary supplements with immunomodulatory effects: a review

        Immune disorders have become one of the public health problems and imposes a serious economic and social burden worldwide. Ginsenosides, the main active constituents of ginseng, are regarded as a novel supplementary strategy for preventing and improving immune disorders and related diseases. This review summarized the recent research progress of ginsenosides in immunomodulation and proposed future directions to promote the development and application of ginsenosides. After critically reviewing the immunomodulatory potential of ginsenosides both in vitro and in vivo and even in clinical data of humans, we provided a perspective that ginsenosides regulated the immune system through activation of immune cells, cytokines, and signaling pathways such as MAPK, PI3K/ Akt, STAT, and AMPK, as well as positively affected immune organs, gut flora structure, and systemic inflammatory responses. However, the evidence for the safety and efficacy of ginsenosides is insufficient, and the immune pathways of ginsenosides remain incompletely characterized. We believe that this review will provide a valuable reference for further research on ginsenosides as dietary supplements with immunomodulatory effects.

      • Prognosis of Eight Chinese Cases of Primary Vaginal Yolk Sac Tumor with a Review of the Literature

        Background: Primary vaginal yolk sac tumor is a rare malignancy in the pediatric population, and a diagnostic challenge and appropriate initial treatment remains unsolved. The aim of this study was to investigate the clinicopathologic features, treatment and prognosis of this tumor. Materials and Methods: Eight cases of primary vaginal yolk sac tumor were reported with a literature review. Results: There were 4 pure yolk sac tumor cases and four mixed germ cell tumors containing yolk sac tumor element, including two cases with embryonal carcinoma and two cases with embryonal carcinoma and dysgerminoma. Partial vaginectomy was performed in four cases and all patients received chemotherapy. 85 cases in literatures were reviewed and 9 cases were misdiagnosed. Follow-up data was available in 77 cases and 5-year overall survival rate was 87.6%. 5-year survival rate of biopsy with chemotherapy, conservative surgery with chemotherapy and radical surgery with chemotherapy was 91.1%, 100% and 28.6%, respectively (p<0.001). Compared to cases without relapse or metastasis after initial treatment, patients with relapse or metastasis had a shorter overall survival (35.6% vs 96.6%, p<0.001). Conclusions: Mixed germ cell tumor containing yolk sac tumor element was not uncommon and partial vaginectomy may be a good choice for primary vaginal mixed yolk sac tumor type to eradicate local tumor cells and provide complete information for pathological diagnosis and postoperative adjuvant therapy.

        While the cloud computing benefits healthcare IT modernization, it introduces new challenges for data integration, which is crucial for information sharing, data mining and business intelligence applications. Data virtualization brings a new strategy and related technologies to cope with these challenges, particularly suitable for cloud data services and SaaS (Software-as-a-Service) applications. The paper discussed a unique architecture and related technologies to implement the data virtualization for cloud data. The paper also demonstrated how this technology can be used in the real-world applications, and a city-wide healthcare appointment system has been implemented for example.

        Effect of a Mangosteen Dietary Supplement on Human Immune Function: A Randomized, Double-Blind, Placebo-Controlled Trial

        The effect of a mangosteen product containing multivitamins and essential minerals was tested on immune function and well-being in healthy adults. A randomized, double blinded, placebo-controlled study was conducted in 59 healthy human subjects (40.60 years old). Changes from baseline immune function were measured after a 30-day consumption of the mangosteen product and the placebo. The subjects' self-appraisal of their health status was also surveyed. A xanthone-rich mangosteen product intake increased mean values for peripheral T-helper cell frequency (P=.020) and reduced the serum C-reactive protein concentration (P=.014). Increases in peripheral CD4/CD8 double-positive (DP) T-cell frequency and serum complement C3, C4, and interleukin (IL)-$1{\alpha}$ concentrations were significantly higher in the experimental group than in the placebo group (DP, P=.038; C3, P=.017; C4, P=.031; IL-$1{\alpha}$, P=.006). At the end of study, serum IL-$1{\alpha}$ and IL-$1{\beta}$ concentrations in the study group were significantly higher than that in the placebo group (IL-$1{\alpha}$, P=.033; IL-$1{\beta}$, P=.04). Furthermore, more participants in the experimental group reported greatly improved overall health status compared with participants receiving placebo (P=.001). The results indicated that the intake of an antioxidant-rich product significantly enhanced immune responses and improved the subject's self-appraisal on his or her overall health status.

        Inhibition of Dll4/Notch1 pathway promotes angiogenesis of Masquelet’s induced membrane in rats

        The Masquelet’s induced membrane technique for repairing bone defects has been demonstrated to be a promising treatment strategy. Previous studies have shown that the vessel density of induced membrane is decreased in the late stage of membrane formation, which consequently disrupts the bone healing process. However, relatively little is known about certain mechanisms of vessel degeneration in the induced membrane tissue and whether promotion of angiogenesis in induced membranes can improve bone regeneration. Here, we showed that the Delta-like ligand 4/ Notch homolog 1 (Dll4/Notch1) pathway was relatively activated in the late stage of induced membrane, especially at the subcutaneous site. Then, DAPT, a classical γ-secretase inhibitor, was applied to specifically inhibit Notch1 activation, followed by up-regulation of vascular endothelial growth factor receptor 2 (VEGFR2) and CD31 expression. DAPTmodified induced membranes were further confirmed to contribute to bone regeneration after autogenous bone grafting. Finally, in vitro experiments revealed that knocking down Notch1 contributed to the functional improvement of endothelial progenitor cells (EPCs) and that DAPT-treated induced membrane tissue was more favorable for angiogenesis of EPCs compared with the vehicle group. In conclusion, the present findings demonstrate that Dll4/ Notch1 signaling is negatively associated with the vessel density of induced membrane. Pharmacological inhibition of Notch1 attenuated the vessel degeneration of induced membrane both in vitro and in vivo, which consequently improved bone formation at the bone defect site and graft resorption at the subcutaneous site.

        The complete mitochondrial genome of fall armyworm Spodoptera frugiperda (Lepidoptera:Noctuidae)

        The mitochondrial genome (mitogenome) can provide important information for understanding molecular evolution and phylogenetic analyses. The complete mitogenome of Spodoptera frugiperda (Lepidoptera:Noctuidae) was determined to be 15,365 bp in length and has the typical gene order found in Noctuidae mitogenomes, it includes 13 protein-coding genes (PCGs), two rRNA genes, 22 tRNA genes and a A?T-rich region. The nucleotide composition was biased toward A?T nucleotides (81.09 %) and the AT skew of this mitogenome was slightly positive (0.004). All PCGs were initiated by ATN codons, except for cytochrome c oxidase subunit 1 (cox1) gene which was initiated by CGA. Eight of the 13 PCGs have the incomplete termination codon, T or TA. All the tRNA genes displayed the typical clover-leaf structure of mitochondrial tRNAs, with the exception of trnS1 (AGN). The A?T-rich region was 328 bp in length and consisted of several features common to the Noctuidae insects. Phylogenetic analysis showed that the S. frugiperda was within the Noctuidae.

        Investigation on the interactions of scutellarin and scutellarein with bovine serum albumin using spectroscopic and molecular docking techniques

        The binding abilities of scutellarin (Scu) andscutellarein (Scue) with bovine serum albumin (BSA) wereinvestigated using equilibrium dialysis, high performanceliquid chromatography, fluorescence spectroscopy, competitivesite marker and molecular docking. The resultsshowed that the average protein binding ratios of Scu andScue with BSA were (79.85 ± 1.83) and (85.49 ± 1.21) %respectively. Under simulated physiological conditions, thefluorescence data indicated that Scu and Scue bound withBSA through a static mechanism. The thermodynamicparameters indicated that the interactions of Scu-BSA andScue-BSA mainly occurred by van der Waals forces andhydrogen bonds and it was easier for Scue to bind withBSA than Scu, indicating that the glucuronic acid moleculein Scu decreased the binding affinity. Site competitivemarker experiments showed that the binding sites of Scuand Scue mainly located within the sub-domain IIA ofBSA. Furthermore, molecular docking studies indicatedthat one BSA could bind three Scue, while one BSA couldcarry only two Scu. All these results clearly indicated theinteractions of Scu and Scue with BSA, which will lay thefoundation for further research to determine the pharmacologyand pharmacodynamics of Scu and Scue for treatingischemic cerebrovascular disease.

        The Pattern of Time to Onset and Resolution of Immune-Related Adverse Events Caused by Immune Checkpoint Inhibitors in Cancer: A Pooled Analysis of 23 Clinical Trials and 8,436 Patients

        Purpose The occurrence pattern of immune-related adverse events (irAEs) induced by immune checkpoint inhibitor (ICI) in cancer treatment remains unclear. Materials and Methods Phase II-III clinical trials that evaluated ICI-based treatments in cancer and were published between January 2007 and December 2019 were retrieved from public electronic databases. The pooled median time to onset (PMT-O), resolution (PMT-R), and immune-modulation resolution (PMT-IMR) of irAEs were generated using the metamedian package of R software.Results Twenty-two eligible studies involving 23 clinical trials and 8,436 patients were included. The PMT-O of all-grade irAEs ranged from 2.2 to 14.8 weeks, with the longest in renal events. The PMT-O of grade ≥ 3 irAEs was significantly longer than that of all-grade irAEs induced by programmed cell death protein 1 (PD-1) and its ligand 1 (PD-L1) inhibitors (27.5 weeks vs. 8.4 weeks, p < 0.001) and treatment of nivolumab (NIV) plus ipilimumab (IPI) (7.9 weeks vs. 6.0 weeks, p < 0.001). The PMT-R of all-grade irAEs ranged from 0.1 to 54.3 weeks, with the shortest and longest in hypersensitivity/infusion reaction and endocrine events, respectively. The PMT-IMR of grade ≥ 3 irAEs was significantly shorter than that of all-grade irAEs caused by PD-1/PD-L1 blockade (6.9 weeks vs. 40.6 weeks, p=0.002) and NIV+IPI treatment (3.1 weeks vs. 5.9 weeks, p=0.031).Conclusion This study revealed the general and specific occurrence pattern of ICI-induced irAEs in pan-cancers, which was deemed to aid the comprehensive understanding, timely detection, and effective management of ICI-induced irAEs.

      • Spect-guidance to Reduce Radioactive Dose to Functioning Lung for Stage III Non-small Cell Lung Cancer

        Objective: To investigate the treatment effect of additional information obtained by single photon emission computed tomography (SPECT) lung perfusion imaging (LPI) in the radiotherapy planning process for patients with stage III non-small cell lung cancer (NSCLC). Methods: 39 patients with stage III NSCLC were enrolled. Gross tumor volume (GTV) was outlined by SPECT/CT images, SPECT-LPIs being used to define functional lung (FL) and non-functional lung (NFL) regions. Two sets of IMRT plans were designed to deliver 64Gy to PTV. One was a regular IMRT plan using CT images only (Plan 1), and the other was a corresponding IMRT plan using co-registered images (Plan 2). $FL_{Vx}$ (the % volume of functional lung receiving ${\geq}$x Gy) and $WL_{Vx}$ (% volume of whole lung to receive ${\geq}$x Gy) were compared by paired Student's t test. Kendalls correlation was used to analyze the factor (s) related with the FLV20 decrease. Results: Compared with plan 1, both $WL_{Vx}$ and $FL_{Vx}$ were decreased in plan 2. $WL_{V10}$, $WL_{V15}$, $WL_{V20}$, $WL_{V25}$, $WL_{V30}$ and $WL_{V35}$ decreased 9.7%, 13.8%, 17.2%, 12.9%, 9.8% and 9.8%, and $FL_{V10}$, $FL_{V15}$, $FL_{V20}$, $FL_{V25}$, $FL_{V30}$ and $FL_{V35}$ decreased 10.8%, 14.6%, 17.3%, 14.5%, 14.5% and 10.5%. $FL_{Vx}$ decreased significantly compared with $WL_{Vx}$. There were significant differences in $WL_{V10}$, $WL_{V15}$, $WL_{V20}$, $WL_{V25}$, $WL_{V3}$ and $FL_{V10}$, $FL_{V15}$, $FL_{V20}$, $FL_{V25}$, $FL_{V30}$ between plan 1 and plan 2 (P=0.002, 0.000, 0.000, 0.005, 0.027 and 0.002, 0.000, 0.000, 0.006, 0.010). According to Kendall correlation analysis, NFL had a negative relation with the percentage FLV20 decrease (r=-0.559, P<0.01), while the distance of PTV and NFL center had a significantly positive relation with the percentage of FLV20 decrease (r=0.768, P<0.01). Conclusion: Routine use of SPECT-LPI for patients undergoing radiotherapy planning for stage III NSCLC appears warranted.
