본 연구에서는 탐방로 안전성 평가 모델을 활용하여 21개 국립공원을 대상으로 안전지수와 등급분석을 수행하였다. 현장조사를 통해 탐방로 주변 위험요소 중 하나라도 확인되면 취약구간으로 판단하여 안전지수를 산출하였다. 취약구간은 총 180.4km로 조사되었으며 탐방로 총 연장 1,995.5km 대비 약 9.0%로 확인되었다. 안전지수는 총 200점 중 최소 42.3점에서 최대 167.0점 분포를 보이고 평균값은 101.5점으로 나타났다. 안전지수를 토대로 등급을 분석한 결과, ‘관심(Concern)’ 등급은 60.8km(34%), ‘주의(Caution)’ 등급은 57.8km(32%), ‘위험(Dangerous)’ 등급은 49.6km(27%), ‘매우위험(Very dangerous)’ 등급은 12.2km(7%)로 나타났다. 매우위험 구간은 북한산국립공원(6.0km), 지리산국립공원(3.3km), 오대산국립공원(1.6km), 설악산국립공원(0.9km), 월악산국립공원(0.4km), 속리산국립공원(0.01km) 등 6개 국립공원 탐방로에서 총 12.2km로 나타났다. 북한산국립공원은 Y계곡, 의상능선 등이 해당되며, 지리산국립공원은 칠선계곡, 구룡계곡에서 확인되었다. 오대산국립공원은 노인봉 일원, 설악산국립공원은 공룡능선, 흘림골 등에서 나타났다. 월악산국립공원은 금수산 일원, 속리산국립공원 막장봉 구간이 매우위험 구간으로 나타났다. 본 연구는 향후 재난위험 저감대책 수립과 탐방로 정비 우선순위 선정 등 국립공원 안전관리 정책을 수립하기 위한 기초자료로 활용될 수 있다. In this study, the safety indexing and grading analysis were conducted for 21 national parks, using the trail safety evaluation model. If any one of the risk factors around the trail was identified through a field survey, the safety index was calculated by judging it as a vulnerability zone. The vulnerability zone was 180.4km in total, and it was found to be about 9.0% compared to the total length of 1,995.5km. The safety index was distributed from the minimum of 42.3 to the maximum of 167.0, and the average value was 101.5 of total 200 scores. As a result of analyzing the grade based on the safety index, 60.8km (34%) was in the ‘Concern’ grade, 57.8km (32%) was in the ‘Caution’ grade, 49.6km (27%) was in the ‘Dangerous’ grade, and 12.2km (7%) was in the ‘Very dangerous’ grade. ‘Very dangerous’ sections were found to be a total of 12.2 km. including 6 national park trails: Bukhansan National Park (6.0km), Jirisan National Park (3.3km), Odaesan National Park (1.6km), Seoraksan National Park (0.9km), Woraksan National Park (0.4km), Songnisan National Park (0.01km). In terms of ‘Very dangerous’ section, Bukhansan National Park included Y Valley and Uisang Ridge, and Jirisan National Park included Chilseon Valley and Guryong Valley. The area of Noinbong in Odaesan National Park, and Gongnyong Ridge and Heulimgol in Seoraksan National Park were included. And the Geumsusan area in Wolaksan National Park and Makjangbong in Songnisan National Park were found to be ‘Very dangerous’. This study could be used as basic data to establish national park safety management policies, such as selecting priority of trail maintenance and establishing countermeasures to reduce disaster risk.

<P><B>Abstract</B></P> <P>Developing effective mucosal subunit vaccine for the <I>Streptococcus pneumoniae</I> has been unsuccessful mainly because of their poor immunogenicity with insufficient memory T and B cell responses. We thus address whether such limitation can be overcome by introducing effective adjuvants that can enhance immunity and show here that polysorbitol transporter (PST) serves as a mucosal adjuvant for a subunit vaccine against the <I>Streptococcus pneumoniae</I>. Pneumococcal surface protein A (PspA) with PST adjuvant induced protective immunity against <I>S. pneumoniae</I> challenge, especially long-term T and B cell immune responses. Moreover, we found that the PST preferentially induced T helper (Th) responses toward Th2 or T follicular helper (Tfh) cells and, importantly, that the responses were mediated through antigen-presenting cells via activating a peroxisome proliferator-activated receptor gamma (PPAR-γ) pathway. Thus, these data indicate that PST can be used as an effective and safe mucosal vaccine adjuvant against <I>S. pneumoniae</I> infection.</P> <P><B>State of Significance</B></P> <P>In this study, we suggested the nanoparticle forming adjuvant, PST works as an effective adjuvant for the pneumococcal vaccine, PspA. The PspA subunit vaccine together with PST adjuvant efficiently induced protective immunity, even in the long-term memory responses, against <I>Streptococcus pneumoniae</I> lethal challenge. We found that PspA with PST adjuvant induced dendritic cell activation followed by follicular helper T cell responses through PPAR-γ pathway resulting long-term memory antibody-producing cells. Consequently, in this paper, we suggest the mechanism for safe nanoparticle forming subunit vaccine adjuvant against pneumococcal infection.</P> <P><B>Graphical abstract</B></P> <P>[DISPLAY OMISSION]</P>

Multiple forms of tranglutaminases(TGase) are present in human epidermis and the cultured human keratinocytes. In an effort to understand the regulation of epidermal TGase in vivo, enzymes in human foreskin tissues and in the cultured human keratinocytes, derived from the same tissue and modified with oncogenic insertion, were examined. The cultured human kerationcytes were inserted or transfected with SV40 DNA virus, kirsten RNA virus or human papilloma viral genes, resulting in immortalization or in tumorigenic transformation. TGases from the tissue or cells were fractionated by elution with 0.03 M NaCl in DEAE-cellulose column for TGase E activity and by elution with 0.5 M NaCl in the same DEAE-column for TGase C activity, while TGase B activity was monitored in the particulated fraction. TGase E activity was the major extractable enzyme activity of the in vivo skin tissue. However, this TGase E activity was negligibly detectable in the immortalized cell lines regardless of the tumorigenicity under the varying medium conditions. And among the immortalized cell lines (RHEK-1, KHOS/RHEK-1, PSV2-ras/RHEK-1, Ki/HPA-IA), activities of TGase C and B were most highest in RHEK-1 cell line, while those activities were decreased with increase of tumorigenicity. From these results, it could be concluded that TGase E activity is deeply related with terminal differentiation of the skin tissues and TGase B and C activities are modulated by the cellular status of transformation.
Continuous separation of fungal spores in a microfluidic flow focusing device

<P>The research of fungi is of great importance in a number of fields, such as environmental and healthcare studies. While there are a large number of optical and molecular methods available for characterization and identification of fungi and their spores, their isolation is still conducted using slow and labor-intensive methods. Here, we develop a microfluidic device for the continuous separation of fungal spores from other eukaryotic cells. The spores were separated through the microfluidic device by expanding pinched flow fractionation (PFF) containing the spores, achieving a spatial separation perpendicular to the flow direction according to the spore size. Further branch flow fractionation (BFF) and co-flow of a Newtonian and viscoelastic fluid were used to enhance the separation performance. Using this microfluidic device, we demonstrated the separation of two different types of fungal spores and further separation of fungal spores from eukaryotic cells with a separation efficiency of above 90%. Compared to the existing conventional methods, our microfluidic flow focusing device requires little manual handling and uses small amounts of samples without any pre-treatment steps of the samples.</P>

<P><B>Abstract</B></P> <P><B>Background</B></P> <P>Hyponatremia is a well-known risk factor for worse outcomes in heart failure (HF) patients. The impact of hyponatremia according to the ejection fraction (EF) is unclear. We evaluated the prognostic value of hyponatremia according to HF type.</P> <P><B>Methods and results</B></P> <P>The Korea Acute Heart Failure (KorAHF) registry consecutively enrolled 5625 patients. Hyponatremia was defined as serum sodium level<135mmol/L at hospital admission. HF with preserved and reduced ejection fraction were defined as an LVEF ≥50% and LVEF ≤40%, respectively.</P> <P>Among 5103 patients with available EF, 2088 (60%) had HFrEF, and 1284 (25%) had HFpEF. There was no difference in serum sodium level between the groups (HFrEF: 137.4±4.7mmol/L vs. HFpEF: 137.5±5.0mmol/L, P=0.710).</P> <P>Hyponatremic patients had higher in-hospital mortality or urgent heart transplantation in all (11.3% vs. 4.5%, P<0.001), in HFrEF (13.1% vs. 4.9%, P<0.001), and in HFpEF (6.0% vs. 1.9%, P<0.001).</P> <P>After adjustment for significant covariates, hyponatremia was associated with 1.5 fold increased risk for 1-year post-discharge death in the HFrEF group (HR, 1.52; 95% CI, 1.24–1.86), but not in the HFpEF group (HR, 1.16; 95% CI, 0.84–1.61). During admission, the sodium status changed in 22% of the patients and the discharge sodium status had greater prognostic value.</P> <P><B>Conclusions</B></P> <P>Hyponatremia is independent of HF type. Hyponatremia is a significant risk factor for adverse in-hospital outcomes; however its long-term prognostic value is only limited to patients with HFrEF, but not for those with HFpEF.</P>

<P>Oculodentodigital dysplasia (ODDD) is a rare autosomal dominant inherited disease caused by mutations of the human gap junction alpha 1 gene, which encodes the protein Connexin-43. Patients with ODDD may present with neurological deficits with a typical pleiotropic combination of characteristic craniofacial, ophthalmological, phalangeal, and dental anomalies. In this report, we describe the first genetically confirmed Korean ODDD patient, who presented with spastic paraparesis. We will also review the neurological aspects of ODDD as reported in the literature.</P>

A case of basal cell carcinoma with ductal differentiation

Basal cell carcinoma (BCC) is the most common skin cancer, which mainly occurs on sun exposed areas of the head and neck. There are many clinical, histological subtypes of BCC although these subtypes share some common characteristics. BCCs can be clinically divided into nodular, pigmented, sclerosing, superficial and fibroepithelioma form. From histologic point of view, it can be classified as two groups: undifferentiated and differentiated. Neoplastic cells of BCC mainly differentiate toward follicular germinative cells, although there are other types such as BCCs which differentiate toward sebaceous, follicular, eccrine and appocrine apparatus. A 59-year-old woman presented with asymptomatic bluish black colored waxy plaque on her left parietal scalp area. She had no significant medical history. The lesion first appeared 16 months ago, increasing size without any symptom, and it was diagnosed as BCC with skin biopsy. The histopathology showed nodules and micronodules of basaloid cells, and unusually, had certain basaloid cells in deeper dermis showing ductal differentiation with mildly vacuolated cytoplasm developing duct-like structures. This suggests that BCC differentiated onto ductal apparatus. Herein, we report a rare case of BCC with ductal differentiation.
