        Gastric Outlet Obstruction: Current Status and Future Directions

        Gastric outlet obstruction (GOO) is a relatively common condition in which mechanical obstruction of the pylorus, distal stomach, or duodenum causes severe symptoms such as nausea, vomiting, abdominal pain, and early satiety. Its etiology includes both benign and malignant disorders. Currently, GOO has many treatment options, including initial conservative therapeutic protocols and more invasive procedures, such as surgical gastroenterostomy, stent placement and, the most recently implemented procedure, endoscopic ultrasound-guided gastroenterostomy (EUSGE). Each procedure has its merits, with surgery often prevailing in patients with longer life expectancy and stents being used most often in patients with malignant gastric outlet stenosis. The newly developed EUS-GE combines the immediate effect of stents and the long-term efficacy of gastroenterostomy. However, this novel method is a technically demanding process that requires expert experience and special facilities. Thus, the true clinical effectiveness, as well as the duration of the effects of EUS-GE, still need to be determined.

        Time-domain hybrid method for simulating large amplitude motions of ships advancing in waves

        Typical results obtained by a newly developed, nonlinear time domain hybrid method for simulating large amplitude motions of ships advancing with constant forward speed in waves are presented. The method is hybrid in the way of combining a time-domain transient Green function method and a Rankine source method. The present approach employs a simple double integration algorithm with respect to time to simulate the free-surface boundary condition. During the simulation, the diffraction and radiation forces are computed by pressure integration over the mean wetted surface, whereas the incident wave and hydrostatic restoring forces/moments are calculated on the instantaneously wetted surface of the hull. Typical numerical results of application of the method to the seakeeping performance of a standard containership, namely the ITTC S175, are herein presented. Comparisons have been made between the results from the present method, the frequency domain 3D panel method (NEWDRIFT) of NTUA-SDL and available experimental data and good agreement has been observed for all studied cases between the results of the present method and comparable other data.

        A Global-Local Approach for Estimating the Internet's Threat Level

        The Internet is a highly distributed and complex system consisting of billion devices and has become the field of various kinds of conflicts during the last two decades. As a matter of fact, various actors utilise the Internet for illicit purposes, such as for performing distributed denial of service attacks (DDoS) and for spreading various types of aggressive malware. Despite the fact that numerous services provide information regarding the threat level of the Internet, they are mostly based on information acquired by their sensors or on offline statistical sampling of various security applications (antivirus software, intrusion detection systems, etc.). This paper introduces proactive threat observatory system (PROTOS), an open-source early warning system that does not require a commercial license and is capable of estimating the threat level across the Internet. The proposed system utilises both a global and a local approach, and is thus able to determine whether a specific host is under an imminent threat, as well as to provide an estimation of the malicious activity across the Internet. Apart from these obvious advantages, PROTOS supports a large-scale installation and can be extended even further to improve the effectiveness by incorporating prediction and forecasting techniques.

        그리스 내셔널리즘 미술의 두 얼굴, 1950~1960

        As it is known, during the Second World War Greece has fought on the side of the allies and the end of the war found the country on the winners' side. However, the struggle for authority right after 1945 was merciless and extremely difficult, as well as dangerous for the course of the country to the future. The political powers were divided between the legal authorities that were represented by the king and formed the exiled government on the one hand and the part of the resistance teams and the rebels of the left that had a soviet friendly direction on the other. Thus, the start of a civil war was just a matter of time. It fin ally started in 1947 and lasted for more than two years. The consequences were disastrous for the country's economy and decisive for the future course of Greece. The national army prevailed with the help of, mostly, the English. Royal parliamentary democracy was established with a clear political turn to the west, as a completion and adaptation of the Agreement of the Great Powers at Yalta. Art had a 'similar' route. Dipolar, contradictory: conservative choices on the one side, and a will for pioneering inspiration and perspective on the other side. The 'dominate' trend was first evident in sculpture and mainly in the public monuments. Their construction aimed mostly at the public propaganda and at the promotion of the sovereign ideology. On the one side we have the public sculptures composed of faces of contemporary heroes or leading figures of the civic war and the national resistance. On the other side we have monumental statues mainly that appeal to a 'public' outside of the country's borders and mostly of the north borders, where there are countries with a communistic regime, like Bulgaria, Serbia and Albania. Their subject is derived from the heroic events of the Balkan Wars (1912-1913) and ancient historical figures like Alexander the Great as the Greek army leader, his father, Philippos II and Aristotle, who was of a north-Greek origin. The political message is twofold: on the one side the 'inner enemy' the communists that were defeated and the promotion of the new liberal social system and on the other side the north neighbours, which not only represent the East Block, but they also conspire the history and the culture of the Greeks. This is the way how the 'Cold War' was resulted in a full and totalitarian expression in art.

      • SDL: A New CAGD Library for Conputer-Aided Ship Design

        Software written in academic environments is rarely integrated and/or reused. Moreover, typical academic software products do not communicate with commercial CAD packages, consequently functionality that is readily available to industry is not available do academic researchers. The Surface Design Library (SDL) is a library serving as a framework under which, CAGD software, intended for research or educational use, can be written in such a manner that it becomes a reusable part of a useful CAGD system. Moreover SDL is equipped with an IGES translator so that it communicates with commercial CAD packages, This paper gives a brief overview to SDL and presents examples of using it for high quality fairing performed on a naval ship hull, before employing commercial CAD packages for further design and analysis.

        Undifferentiated embryonal sarcoma of the liver in a young female

        Undifferentiated Embryonal Sarcoma of the Liver (UESL) is a tumor highly malignant, of mesenchymal origin. It is a rare finding in adults, though less rare in children. The strategy to be followed and the therapeutic targets to be reached for this tumor, in adult cases, remain ambiguous and controversial. Herein we report the case of a 29 year old female patient with a massive UESL and we describe our therapeutic approach. A 29 year-old female patient was referred to our center with severe intermittent epigastric pain and fever due to a voluminous liver tumor: Needle biopsy was of no specific findings and surgical excision was decided. Right portal vein embolization and selective embolization of the segment’s IV branch was performed in order to achieve adequate future liver remnant (FLR). Right trisectonectomy was then performed, with uneventful post operative period and the patient was discharged at the 11<SUP>th</SUP> post operative day. UESL is a rare tumor that needs aggressive surgical approach and multidisciplinary team management is of paramount importance.
