        A novel prediction score for predicting the baseline risk of recurrence of stage I–II endometrial carcinoma

        Objective: To develop and validate a 3-year recurrence prediction score (RPS) system for predicting the baseline risk of recurrence of stage I–II endometrial carcinoma. Methods: We reviewed 427 patients with International Federation of Gynecology and Obstetrics staging I–II endometrial carcinoma underwent surgery without any adjuvant therapy from 2005 to 2013. The patients were divided into 2 groups: the test cohort (n=251) comprising those who underwent surgery in odd-numbered years, and the validation cohort (n=176) comprising those who underwent surgery in even-numbered years. Multivariate analysis was performed using 7 candidate predictors to identify the risk factors for 3-year recurrence-free interval (RFI) in the test cohort. Each risk factor was scored based on logistic regression analyses of the test data set, and the sum of the risk factor scores was defined as the RPS system. We then applied the system in the validation cohort. Results: Multivariate analysis revealed that the significant risk factors were age ≥60 years, pathological type II, positive cervical stromal invasion, and positive peritoneal cytology. In the test cohort, the 3-year RFI rates were 100%, 95.8%, 79.9%, and 33.3% for RPSs of 0, 1, 2, and 3, respectively. In the validation cohort, the 3-year RFI was significantly higher in the low-RPS group (RPS 0 or 1) than in the high-RPS group (RPS 2 or 3) (95.2% vs. 79.9%, p<0.01). Conclusions: The RPS system shows significant reproducibility for predicting the baseline risk of recurrence. The system could potentially impact the choice of adjuvant therapy for stage I–II endometrial carcinoma.

        Fabrication and Mechanical Properties of Porous Ti/HA Composites for Bone Fixation Devices

        <P>Porous Ti (pTi)/hydroxylapatite (HA) composites were fabricated by an infiltration method in a vacuum and sintering. The Young’s modulus of the composites was evaluated before and after the immersion in Hanks’ solution. The strength of the composites was also evaluated by four-point bending test. The Young’s modulus of the compacts was governed by the porosity and was comparable to the human bone in the porosity range from 24 to 34%. The Young’s modulus of sintered pTi/HA composites was larger than that of pTi. The Young’s modulus of the sintered pTi/HA composites decreased after immersion in Hanks’ solution. The proof and bending strengths of the sintered pTi/HA composites were larger than those of pTi. Solid solution hardening of Ti by oxygen contributed the increase of the proof strength.</P>

        Effects of Val-Pro-Pro and Ile-Pro-Pro on Nondipper Patients: A Preliminary Study

        Much clinical evidence on the antihypertensive effects of the milk-derived antihypertensive peptides Val-Pro-Pro and Ile-Pro-Pro (lactotripeptides) has been reported. However, circadian rhythm effects determined by ambulatory blood pressure monitoring (ABPM) to eliminate the confounding influence of the white-coat effect have not been fully studied. Twelve hypertensive patients not receiving antihypertensive medication (2 men, 10 women; mean age±standard deviation, 63.5±8.3 years) who had been visiting our clinic for more than 1 year participated in this study. Mean (±standard deviation) systolic blood pressure (SBP) and diastolic blood pressure (DBP) were 142.4±2.6 and 83.5±6.4 mm Hg, respectively, at the first office visit. After patients ingested a fermented milk product containing antihypertensive peptides (2.53 mg Val-Pro-Pro; 1.52 mg Ile-Pro-Pro) for more than 4 weeks, both office SBP and DBP were significantly reduced to a mean (±standard deviation) of 133.3±7.0 mm Hg and 76.5±8.4 mm Hg (P<.001 and P<.005 by paired t-test), respectively. The 24-hour SBP and DBP determined by ABPM were reduced from 127.3±2.4 and 78.7±2.3 mm Hg to 120.2±2.4 and 75.0±2.2 mm Hg (P<.001 and P<.05), respectively. Awake-time SBP (08:00–21:00), night-time SBP (0:00–05:00), and early-morning SBP (06:00–07:00) were reduced from 130.9±2.4 to 123.3±2.3 mm Hg, 118.7±2.9 to 113.2±3.4 mm Hg, and 132.8±4.3 to 122.4±3.9 mm Hg (by paired t-test: P<.001, P<.05, and P<.05), respectively. As seen with DBP measured by ABPM, 24-hour DBP and awake-time DBP were significantly reduced from 78.7±2.3 to 75.0±2.2 mm Hg and 82.1±2.5 to 77.3±2.2 mm Hg (P<.05 and P<.01), respectively. Office BP and 24-hour blood pressure did not significantly differ between the dipper and nondipper groups at baseline. However, after treatment, night-time and early-morning blood pressure were significantly reduced from baseline in the nondipper group (−8.5±2.5 and −15.6±3.7 mm Hg; P<.05 and P<.01, respectively) but not in the dipper group (−2.5±3.6 and −1.2±4.7 mm Hg; P not significant), and the reduction in early-morning blood pressure significantly differed between the groups (P<.05). These results suggest that Val-Pro-Pro and Ile-Pro-Pro decrease blood pressure in patients with stage I hypertension and result not only in lower blood pressure at night-time but also in lower early-morning SBP in nondipper patients.

      • Inhibition of heregulin signaling by N-glycan deleted soluble ErbB3

        ErbB signaling is implicated in the pathology of various types of carcinoma. Ligand binding to the extracellular domain of ErbB induces conformational changes that lead to homo- or heterodimerization of the receptors and subsequent signaling. It has been reported that N-glycans of the ErbB family regulate its function. We previously reported that N-glycans of EGFR are involved in receptor dimerization and endocytosis. N-glycans of ErbB3 are also involved in receptor dimerization and activation status; deletion of the N-glycan on Asn418 of ErbB3 leads to both spontaneous homodimerization of ErbB3 and heterodimerization of ErbB2-ErbB3. The promoted heterodimerization with ErbB2 leads to upregulation of receptor tyrosine phosphorylation, downstream signalings, and transforming activity. We recently found that N-glycans of ErbB4 also regulate ligand-induced signaling. Thus, N-glycans of the ErbB family seem to play an important role in receptor activation, especially in receptor dimerization. To examine the role of N-glycans of the ErbB in detail, we prepared a wild type and an N-glycan deleted mutant of sErbB3 (domains I, II, III and IV) and compared their properties. When added to culture cells expressing ErbB2 and ErbB3, the sErbB3 N418Q mutant downregulated the heregulin b signaling more effectively than the wild type sErbB3. Through mass spectrometry analysis, we were able to determine the structure of the N-glycan on Asn418 of sErbB3. This suggests that the specific N-glycan of sErbB3 regulates the binding status of sErbB3 to ligands or receptors. We consider that the sErbB3 N418Q mutant is a potent inhibitor of transforming activity of ErbB, and may have therapeutic applications in cancer.

        Variation in TP63 is associated with lung adenocarcinoma susceptibility in Japanese and Korean populations

        Lung cancer is the most common cause of death from cancer worldwide, and its incidence is increasing in East Asian and Western countries. To identify genetic factors that modify the risk of lung adenocarcinoma, we conducted a genome-wide association study in a Japanese cohort, with replication in two independent studies in Japanese and Korean individuals, in a total of 2,098 lung adenocarcinoma cases and 11,048 controls. The combined analyses identified two susceptibility loci for lung adenocarcinoma: TERT (rs2736100, combined P = 2.91 ? 10<SUP>??11</SUP>, odds ratio (OR) = 1.27) and TP63 (rs10937405, combined P = 7.26 ? 10<SUP>??12</SUP>, OR = 1.31). Fine mapping of the region containing TP63 showed that a SNP (rs4488809) in intron 1 of TP63 showed the most significant association. Our results suggest that genetic variation in TP63 may influence susceptibility to lung adenocarcinoma in East Asian populations.

      • Evaluation of Voltage Control Effect for Data Acquisition Period Length from SCADA with IT Switches

        Measured data from IT switches are utilized in order to control voltage in Japanese distribution systems with photovoltaic generation systems. However, length of period from the data measurement to the data acquisition from IT switches by SCADA affects voltage control ability in a distribution automation system. In this paper, the effect of the length of the data acquisition period for voltage control by LRT and SVR is evaluated through numerical simulations in a distribution system model. Furthermore, the optimal length of the data acquisition period is determined according to PV penetration rate.
