        Objectives: This study was performed to evaluate whether there is the difference of graft height changes in radiographic evaluation in single implants and multiple implants after maxillary sinus floor augmentation with an auto-xenograft mixture. Methods: The study group comprised 29 patients (16 men, 13 women; mean age, 56.5±12.4 years; range, 27 to 87 years) treated at the implant clinic of the Ewha Womans University Mokdong Hospital. They were treated with the sinus floor augmentation of lateral window technique and simultaneous placement of dental implants. Their implants have been evaluated for 42.4±10.5 months. To evaluate the height of grafted material, the real implant length was compared with that of panoramic radiography evaluated by a digital caliper (PACS program), and then the real height of grafted material was assumed. SPSS 19.0 was used in statistical analysis. Wilcoxon singed rank test was utilized for P value and Mann-Whitney for statistical significance in the changes of grafted height between single implants and multiple implants. The level of statistical significance was defined as P<0.001. Results: The evaluated graft height in single implants' group decreased to 0.60±0.54 mm at the time of delivery (after 7.5 months, P=0.001), in multi-unit implants' group to 0.96±0.84 mm (after 6.5 months, P<0.001). The evaluated graft height in single implants' group decreased to 1.11±0.75 mm at the last follow-up (after 43.5 months, P=0.001), in multi-unit implants' group to 1.52±0.99 mm (after 41.8 months, P<0.001). Mann-Whitney test was used for the difference between single implants and multi-unit implants. There was no difference in two groups at the time of delivery and the last follow-up (P>0.001). Conclusion: There was no statistical significance of radiographic graft height after maxillary sinus floor augmentation between single implants and multiple implants.

        턱기형의 악교정수술후 환자의 심리적변화

        This is to study the presurgical motivation, psychological changes and self satisfaction after the surgical treatment of dentofacial deformities. The subjects were 34 patients that the orthognathic surgeies were rendered and followed-up over 6 months. The results were as followings; 1. The analysis of MMPI profiles in clinical scale reveals that 8.8%(3 patients) of neurologic type, 2.9%(1 patient) of personality problem type and none of psychotic type, while 88.3% of showed no abnormal findings in psychologic status. 2. The significant differences of personality characteristics were noticed in the Depression(D), Hypochondriasis(Hs), Musculinity-Feminity(Mf) were changed significantly following the orthognathic surgeries.(P<0.05) 3. The self assesment of the esthetic results were satisfactory in 78.6%, while no changes in 14.3%.

        악교정 골성형술후 약관절 장애의 변화

        Sixty four patients were followed-up for over 6 months following the orthognathic surgeries and were examined for the Temporo-mandibular joint dysfunction. The results are as follows: 1. Of 64 patients, 9(14.1%) had been suffered from TMD preoperatively : 47% in the prognathism and 46.2% in the laterognathism. 2. Following the orthognathic surgeries, 7 of 64 patients(10.9%) complained TMD, and 55.6% of the TMD patients resulted in significant improvement. 3. By the type of deformities, laterognathism with TMD could be expected to be improved in 30.8%, while 7.7% in the prognathism. 4. Non-rigid fixation resulted in higher incidence of TMD than the Rigid fixation after the SSRO ; 4 of 15(26.5%) and 2 of 25(8.0%) respectively.

      • 신라대학교 2018년도 2학기 ‘부산의 하루’ 수강생 대상 설문조사 결과 분석

        In 2018 2nd semester, 134 students who were attending the ‘A Day of Busan’ were asked to take this survey. Among them, 78 students were from Busan. This paper analyzed these 78 students. According to the respondents' grades, 52.6% of the respondents were in the first grade, slightly more than half, and the second was 33.3%. It can be seen that the 'A Day of Busan' course are mainly in interested in lower grades. According to the year of birth of the respondents, the birth rate in 1999 was 43.6% of the total, including one missing value. In this questionnaire, we surveyed and analyzed the religion of the students, the parent's home, the typical food of Busan, representative places in Busan, and the personality, strengths and weaknesses of Busan people. We also surveyed students' lives, for example, the number of participating in Pusan International Film Festival, the number of visits to professional baseball games, the number of overseas trips, using mobile phones time per day, the number of close friends, sleeping hours. Also we surveyed the population size of Busan. The proportion of students who replied that population size is less than the actual population of 3.45 million in Busan was similar to that of students who replied that it is more. 2018년도 2학기 ‘부산의 하루’ 수강생 가운데 부산출신 학생 78명을 대상으로 분석을 하였다. 응답자의 학년별 구성을 보면 1학년이 52.6%로 절반을 조금 넘고, 그 다음은 2학년으로 33.3%이었다. ‘부산의 하루’ 과목은 주로 저학년이 수강하는 것을 알 수 있다. 응답자의 출생연도를 보면 1999년 출생자가 결측값 1명을 포함한 전체에서 43.6%를 점하여 가장 높은 비율을 보였다. 응답학생 78명의 종교를 보면 무교가 61.5%로 압도적인 비중을 점하고 있다. 그 다음으로 불교가 20.5%, 개신교가 12.8%, 가톨릭이 3.8% 그리고 기타종교가 1.3%였다. 아버지의 고향은 부산이 53.8%로 절반을 넘으며, 그 다음은 경남으로 24.4%였다. 부산과 경남을 합하면 78.2%로 절대적인 비중을 점하고 있다. 어머니의 고향은 부산이 56.4%, 경남이 11.5%로 부산과 경남을 합하면 67.9%였다. 부산의 대표음식으로는 돼지국밥이 36.2%, 21.2%의 어묵, 15.7%의 밀면, 8.7%의 씨앗호떡, 7.4%의 생선회 순이었다. 타지역에서 오는 친구들에게 보여주고 싶은 곳으로는 광안리가 17.9%, 해운대로 13.1%, 서면으로 10.3였다. 그 다음으로는 롯데야구 6.1%, 광안리불꽃놀이 5.3%, 센텀시티 5.1%, 광안대교 4.9% 순으로 나타나 주로 광안리와 해운대 그리고 서면 쪽으로 쏠리는 경향이 나타났다. 부산에서 새로 생기면 좋은 것에 대한 설문에서는 놀이공원이 13.3%, 워터파크10.8%, 공원 8.7%, 체육관 8.2%, 수목원과 박물관이 각각 5.8%로 나타나 학생들은 놀이시설을 가장 원하고 있었으며 그 다음으로 공원과 체육관을 선호하였다. 부산사람들의 성격에서 좋은 점이 무엇인가에 대해서는 ‘의리있다’가 17.8%로 가장 많았고, 나쁜 성격은 32.5%의 압도적인 비율이 성격이 ‘급하다’로 나타났다. 즐겨마시는 음료에는 커피가 33.3%로 가장 높았고, 그 다음은 26.9%의 콜라였다. 주중 술 마시는 횟수에 대한 설문에는 ‘전혀 마시지 않는다’는 학생이 38.5%로 약 40%에 해당하는 학생이 전혀 술을 마시지 않는 것으로 나타났다. 부산국제영화제에 참여한 횟수, 부산에서 열린 프로야구 경기에 참여한 경험, 해외 여행 횟수에 대한 설문에 대해서는 각각 전체의 64.1%, 35.9%, 33.3%의 학생들이 한번도 없는 것으로 나타났다. 하루 휴대전화 사용시간의 평균은 271분, 친한 친구수의 평균은 13명, 수면시간은 평균은 6.8시간, 한달 용돈은 평균 35만원이었다. 부산의 인구가 얼마나 되겠는가 하는 설문에 대한 응답은 매우 다양하였는데, 2만명에서부터 3,800만명까지 넓은 범위에 걸쳐 분포하였다. 그런데 부산의 실제인구 345만명 선보다 적다고 응답한 학생의 비율이 약 절반이었고 나머지 절반은 그보다 많다고 응답하였다.

        구강 내 디지털 인상채득을 통한 맞춤형 지대주와 시멘트 합착 후 나사형 임플란트 보철 수복 증례

        최근 디지털 인상채득 시스템이 개발되면서 이를 기존의 CAD/CAM 시스템과 접목한 인레이, 온레이, 크라운이나 간단한 계속가공의치 등의 고정성 보철물의 제작이 활발히 소개되고 있다. 그러나 임플란트 보철에서는 주로 기존의 아날로그식 인상채득 후 이를 스캔하여 제작하는 방법이 사용되어 왔으나, 구강 내 디지털 스캔한 데이터를 이용하여 나사형 보철물을 제작하는 술식은 아직 활발히 이루어지지 않았다. 이에 본 증례보고에서는 6명의 환자를 대상으로 구강 내 스캐너인 iTero를 사용하여 디지털 스캔을 시행한 후 그중 다섯명은 맞춤형 지대주와 시멘트 합착 후 나사형 임플란트 보철물(screw-retained implant prosthesis after cementation, or SCRP)을 제작하였고, 임상적으로 기능과 심미적인 측면으로 만족할 만한 결과를 얻었기에 이를 보고하고자 한다. Fixed dental prostheses such as inlay, onlay, crown, and bridge fabricated by CAD/CAM technique combined with digital impressions is getting popular due to the recent rapid progress of digital impression taking system. For the scope of implant prosthesis, however, digital intra-oral scan hasn’t been actively utilized for the fabrication of superstructures. In this case report, 6 cases of titanium-milled custom abutment based on the iTero intra-oral scan data were introduced, five of them were restored with screw-type prosthesis after cementation (SCRP) and the clinical results were satisfactory on restoring the function and esthetics.

        In order to characterize pharmacokinetic profiles according to route of a new enrofloxacin salt form (Enrotil®), it was given to 4 healthy pigs via oral (p.o.), intramuscular (i.m.) or intravenous (i.v.) administrations at a dose rate of 5 ㎎/㎏ body weight. Enrofloxacin (ENFX) in serum was detected by bioassay using E. coli BE1186 as a test organism. The biological elimination half-lives (t_1/2(β)) of ENFX were 6.76±0.99 h (i.v.), 7.16±2.30 h (i.m.) and 11.45±3.90 h (p.o.), Volume of distribution (Vd) of enrofloxacin was 2.20±0.31 L/㎏ (i.v.), 2.52±0.60 L/㎏ (i.m.) and 1.88±0.33 L/㎏ (i.m.). Mean residence time (MRT) was 8.77±1.26 h after i.v. injection and the maximal concentration time (Tmax) following p.o. and i.m. administration was 0.76±0.09 h and 0.60±0.12 h, indicating a rapid absorption from these routes. Bioavailibility (F) was calculated as 64.1% for p.o. administration and 59.71% for i.m. injections. In summary, the newly formulated enrofloxacin salt form has shown a high water solubility, rapid absorption and large tissue distribution, suggesting a potential antibacterials for oral application on a large scale in veterinary sectors.

        Three dimensional finite element analysis of the stress distribution around the mandibular posterior implant during non-working movement according to the amount of cantilever

        PURPOSE In case of large horizontal discrepancy of alveolar ridge due to severe resorption, cantilevered crown is usually an unavoidable treatment modality. The purpose of this study was to evaluate the clinical criteria for the placement of the aforementioned implant crown. MATERIALS AND METHODS The mandible model with 2 mm thick cortical bone and cancellous bone was fabricated from CT cross-section image. An external connection type implant was installed and cantilevered crowns with increasing offset of 3, 4, 5, 6, and 7 mm were connected. Vertical load and 30° oblique load of 300 N was applied and stress around bone and implant component was analyzed. A total of 14 cases were modeled and finite element analysis was performed using COSMOS Works (Solid works Inc, USA). RESULTS As for the location of the vertical load, the maximum stress generated on the lingual side of the implant became larger according to the increase of offset distance. When the oblique load was applied at 30°, the maximum stress was generated on the buccal side and its magnitude gradually decreased as the distance of the offset load increased to 5 mm. After that point, the magnitude of implant component's stress increased gradually. CONCLUSION The results of this study suggest that for the patient with atrophied alveolar ridge following the loss of molar teeth, von-Mises stress on implant components was the lowest under the 30° oblique load at the 5 mm offset point. Further studies for the various crown height and numbers of occusal points are needed to generalize the conclusion of present study.

        Gain of a New Exon by a Lineage-Specific Alu Element-Integration Event in the BCS1L Gene during Primate Evolution

        BCS1L gene encodes mitochondrial protein and is a member of conserved AAA protein family. This gene is involved in the incorporation of Rieske FeS and Qcr10p into complex III of respiratory chain. In our previous study, AluYRa2-derived alternative transcript in rhesus monkey genome was identified. However, this transcript has not been reported in human genome. In present study, we conducted evolutionary analysis of AluYRa2-exonized transcript with various primate genomic DNAs and cDNAs from humans, rhesus monkeys, and crabeating monkeys. Remarkably, our results show that AluYRa2 element has only been integrated into genomes of Macaca species. This Macaca lineage-specific integration of AluYRa2 element led to exonization event in the first intron region of BCS1L gene by producing a conserved 3' splice site. Intriguingly, in rhesus and crabeating monkeys, more diverse transcript variants by alternative splicing (AS) events, including exon skipping and different 5' splice sites from humans, were identified. Alignment of amino acid sequences revealed that AluYRa2-exonized transcript has short N-terminal peptides. Therefore, AS events play a major role in the generation of various transcripts and proteins during primate evolution. In particular, lineage-specific integration of Alu elements and species-specific Alu-derived exonization events could be important sources of gene diversification in primates.

        인공 고관절 치환술에서 대퇴주대 주위의 시멘트 두께 측정

        목적: 확강용 기구에 의해 만들어진 가상의 업체 공간 속에서 대퇴주대의 회전에 따른 시멘트의 두께 변화를 측정하였다. 대상 및 방법: C stem과 Versys stem을 사용하였고, 분석은 C250/400VZ (Steinbichler Co., Germany)와 S/W: Geomagic & CATLA V5를 이용하였다. 정중간에 대퇴주대를 정일치 시킨 후 확강용 기구를 이용하여 만들어진 가상의 입체 공간 내에서 실제 대퇴주대를 횡단면과 관상면을 기준으로 회전시켜 회전 변화에 따른 실제 주대의 바깥면과 확강용 기구의 바깥면 사이의 거리를 측정하였다. 결과: C stem은 대퇴주대가 3도 회전할 때에 1 ㎜ 미만으로 감소하였고 Versys stem의 경우는 5도에서 간격이 1 ㎜ 미만으로 감소하였다. 선회축 회전의 경우 C stem은 0.25도 내반된 상태에서 1 ㎜ 미만으로 감소하였고, 내반각이 0.8도에서 접촉하였으며, Versys stem의 경우는 1도에서 접촉하였다. 선회축 회전과 횡단면의 회전이 복합된 경우 전예에서 1 ㎜ 미만으로 나타났다. 결론: 적절한 시멘트 두께의 유지를 위하여 대퇴주대의 정확한 삽입을 위한 기구 개발, 또는 대퇴주대의 회전 변화에 견딜 수 있는 대퇴주대의 모형 변화가 필요할 수 있으리라 판단된다. Purpose: To measure the cement mantle thickness that developed from a rotation of the femoral stem in virtual space made by the broach. Materials and Methods: The C stem and Versys stem of the subjects enrolled in this study were examined. A C250/400VZ(Steinbichler Co. Germany) and S/W: Geomagic & CATIA V5 was used to examine the three-dimensional configurations. The axial images were acquired after neutral placement of the femoral stem, horizontal rotation, coronal tilting, and a combination of both. The thick-ness was measured from the distance between the outer surface of the femoral stem and the closest vertex in the outer contour of the broach in cross-sectional images. Results: The distance was < 1 ㎜ at the horizontal rotation of 3 degrees in the C stem, and at 5 degrees in the Versys stem. The distance was < 1 ㎜ at varus of 0.25 degrees in the C stem, and touched at 0.8 degrees. In the case of the Versys stem, the femoral stem touched at a varus of 1 degree. A combination of both horizontal and pivot rotation made the distance < 1 ㎜ at rotation of all angles. Conclusion: It might be necessary to develop a new instrument for the accurate insertion of the femoral stem along the broached space or to modify the stem design to stand against the change in the femoral stem position in the broached space.
