Effect of bosentan on the production of proinflammatorycytokines in a rat model of emphysema

Endothelin (ET) receptor antagonists have been developed to produce a reduction of ET related effects in various diseases, as well as in animal models of airway inflammation. We aimed to investigate the anti-inflammatory potential of bosentan on a rat model of emphysema. Thirty Wistar male rats were classified as control group (group 1), intratracheally (i.t.) instilled with saline, treated with vehicle solution; elastase group (group 2), i.t. instilled with porcine pancreatic elastase (PPE), treated with vehicle solution; and PPE+bosentan group (group 3), i.t. instilled with PPE, treated with bosentan. The levels of TNF-α, IL-1β, IL-6, and IL-8 in bronchoalveolar lavage fluid (BALF) and lung tissue, cell counts in BALF, and histologic analysis of all groups were evaluated. Neutrophile granulocytes (NG) and alveolar macrophages (AM) were increased more in group 2 than in group 1 (P < 0.001, P = 0.04, respectively). Compared with group 2, neutrophil granulocyte (NG) and alveolar macrophages (AM) counts were decreased in group 3 (P < 0.001). Histological examination confirmed a diffuse neutrophilic inflammation and irregular alveolar air space enlargement in group 2. Treatment with bosentan partially reduced the enlarged lung volumes. Compared with group 1, the BALF levels of TNF-α and IL-6, and the lung tissue levels of IL-1β, IL-6, and IL-8 were increased in group 2 (P = 0.028, P = 0.005, P = 0.001, P = 0.019, P < 0.001, respectively). The TNF-α and IL-8 levels of BALF (P = 0.007, P = 0.001, respectively), and the TNF-α, IL-1β, IL-6, and the IL-8 levels of lung tissue (P = 0.031, P = 0.017, P = 0.007, P < 0.001) were decreased in group 3 compared to group 2. In conclusion, bosentan decreased the inflammatory response by reducing numbers of inflammatory cells and proinflammatory cytokines.

Purpose The purpose of this study is to evaluate the prognostic role of preoperative neutrophil to lymphocyte ratio (NLR) and platelet to lymphocyte ratio (PLR) and the need for para-aortic lymphadectomy in patients with primary fallopian tube carcinoma (PFTC). Materials and Methods Ninety-one patients with a diagnosis of PFTC were identified through the gynecologic oncology service database of six academic centers. Clinicopathological, surgical, and complete blood count data were collected. Results In univariate analysis, advanced stage, suboptimal surgery, and NLR > 2.7 were significant prognostic factors for progression-free survival, whereas in multivariate analysis, only advanced stage and suboptimal surgery were significant. In addition, in univariate analysis, cancer antigen 125 ! 35 U/mL, ascites, advanced stage, suboptimal surgery, NLR > 2.7, PLR > 233.3, platelet count ! 400,000 cells/mm3, staging type, and histological subtype were significant prognostic factors for overall survival (OS); however, in multivariate analysis, only advanced stage, suboptimal surgery, NLR > 2.7, and staging type were significant. Inclusion of pelvic and para-aortic lymphadenectomy in surgery showed significant association with longer OS, with a mean and median OS of 42.0 months and 35.5 months (range, 22 to 78 months), respectively, vs. 33.5 months and 27.5 months (range, 14 to 76 months), respectively, for patients who underwent surgery without para-aortic lymphadenectomy (hazard ratio, 3.1; 95% confidence interval, 1.4 to 5.7; p=0.002). Conclusion NLR (in both univariate and multivariate analysis) and PLR (only in univariate analysis) were prognostic factors in PFTC. NLR and PLR are inexpensive and easy tests to perform. In addition, patients with PFTC who underwent bilateral pelvic and para-aortic lymphadenectomy had longer OS.

<P>We present our investigation of 319 Class II objects in Orion A observed by Spitzer/IRS. We also present the follow-up observations of 120 of these Class II objects in Orion A from the Infrared Telescope Facility/SpeX. We measure continuum spectral indices, equivalent widths, and integrated fluxes that pertain to disk structure and dust composition from IRS spectra of Class II objects in Orion A. We estimate mass accretion rates using hydrogen recombination lines in the SpeX spectra of our targets. Utilizing these properties, we compare the distributions of the disk and dust properties of Orion A disks with those of Taurus disks with respect to position within Orion A (Orion Nebular Cluster [ONC] and L1641) and with the subgroups by the inferred radial structures, such as transitional disks (TDs) versus radially continuous full disks (FDs). Our main findings are as follows. (1) Inner disks evolve faster than the outer disks. (2) The mass accretion rates of TDs and those of radially continuous FDs are statistically significantly displaced from each other. The median mass accretion rate of radially continuous disks in the ONC and L1641 is not very different from that in Taurus. (3) Less grain processing has occurred in the disks in the ONC compared to those in Taurus, based on analysis of the shape index of the 10 mu m silicate feature (F-11.3/F-9.8). (4) The 20-31 mu m continuum spectral index tracks the projected distance from the most luminous Trapezium star, theta(1) Ori C. A possible explanation is UV ablation of the outer parts of disks.</P>
