긴장성 요실금증 - 변형된 Stamey 술법을 이용한 25 예의 분석 -

본 가톨릭의과대학 산부인과학교실에서는 1985년부터 1989년까지 긴장성 요실금을 주소로 내원한 환자 25예에서 Stamey 방법을 변형한 내시경하 방광경부거상법을 이용하여 치료한 결과 다음과 같은 결론을 얻었다. 1. Modified Stamey 술은 다른 방법에 비해 수술시간이 단축되고, 수술조작 및 수술후의 처치도 간편하였다. 2. 수술의 성공율은 96%로 높았으며, 실리콘튜브 및 No. 2 monofilament 나일론을 이용하여 비교적 영구적으로 방광경부의 거상을 유지시킬 수 있었다. 3. 방광경부의 적정 거상정도를 판단하는데는 객관적인 기준이 없었고, 술자의 숙련을 요했다. 이상의 결과를 볼 때, 본 교실에서 시행한 변형된 Stamey 술법은 긴장성 요실금 환자의 치료에 있어서 권장할만한 방법이며 술자의 숙련에 의해 더 좋은 결과를 얻을 수 있을 것으로 생각된다. There are various surgical approached for the treatment of stress urinary incontinence up to now, many procedures have been utilized with varying degree of success. We had experienced 25 cases of stress urinary incontinence from 1985 to 1989 and treated all these cases with the method using modified Stamey procedure, so obtained the results as follows. 1. Mean age of the patients was 42.9 year and mean parity was 3.2. 2. In subdivision of 25 cases, Green type I was 52% and type II was 48% also, in the degree of severity, grade I was 40% and grade II was 60%. 3. Postoperatively, posterior urethrovesical angle was markedly reduced. 4. Postoperatively, posterior urethrovesical angle was markedly reduced. 5. The average length of Foley catheter drainage was 9 days. 6. Postoperative stress test & Boney test showed negative results in all the cases. 7. During the average 17 months follow up after operation, only 1 case was recurred. 8. Postoperative complications were minimal except for urinary retention in 9 out 25 cases(36%), of whom three were unable to void till 2 weeks after operation.
