        Accuracy of cardiac output measurements during off-pump coronary artery bypass grafting: according to the vessel anastomosis sites

        Background: During beating heart surgery, the accuracy of cardiac output (CO) measurement techniques may be influenced by several factors. This study was conducted to analyze the clinical agreement among stat CO mode (SCO), continuous CO mode (CCO), arterial pressure waveform-based CO estimation (APCO), and transesophageal Doppler ultrasound technique (UCCO) according to the vessel anastomosis sites. Methods: This study was prospectively performed in 25 patients who would be undergoing elective OPCAB. Hemodynamic variables were recorded at the following time points: during left anterior descending (LAD) anastomosis at 1 min and 5 min; during obtuse marginal (OM) anastomosis at 1 min and 5 min: and during right coronary artery (RCA) anastomosis at 1 min and 5 min. The variables measured including the SCO, CCO, APCO, and UCCO. Results: CO measurement techniques showed different correlations according to vessel anastomosis site. However, the percent error observed was higher than the value of 30% postulated by the criteria of Critchley and Critchley during all study periods for all CO measurement techniques. Conclusions: In the beating heart procedure, SCO, CCO and APCO showed different correlations according to the vessel anastomosis sites and did not agree with UCCO. CO values from the various measurement techniques should be interpreted with caution during OPCAB.       • The effects of secondary task on the control of vehicle under driving simulator: a study among 50s taxi drivers

        Objective: The aim of this study is to quantitatively evaluate the effects of the secondary task during simulated driving using the variable indicating control of vehicle. Background: There are so many researches for simulated driving and performance. For instance, some researches show that the effects of cell phone conversations on simulated driving. Cell phone conversations impaired explicit recognition memory. Method: Participants included 50s taxi drivers; 15 males and 15 females. All subjects were instructed to keep a distance (30m) from the car ahead and a constant speed (80km/hr or 100km/hr). Sending text message (STM) and Searching navigation (SN) were selected as the secondary task. Experiment consisted of rest for 3min., driving alone for 1min. and driving with secondary task for 1 min.. We were analyzed anterior-posterior coefficient of variation (APCV) and medial-lateral coefficient of variation (MLCV). Results: APCV was increased by 175.4% in STM and 223.8% in SN at 80km/hr. MLCV was increased by 242.3% in STM and 323.4% in SN at 100km/hr. There was no significant difference between genders. Conclusion: Consequently, performing secondary task during driving impaired the control of vehicle.

        Evolutionary dynamics of transposable elements during silkworm domestication

        Although there are some documented examples on population dynamics of transposable elements (TEs) in model organisms, the evolutionary dynamics of TEs in domesticated species has not been systematically investigated. The objective of this study is to understand population dynamics of TEs during silkworm domestication. In this work, using transposondisplay we examined the polymorphism of seven TE families [they represent about 59% of silkworm (Bombyx mori) total TE content] in four domesticated silkworm populations and one wild silkworm population. Maximum likelihood (ML) was used to estimate selection pressure. Population differentiation and structure were performed by using AMOVA analysis and program DISTRUCT, respectively. The results of transposon-display showed that significant differentiation occurred between the domesticated silkworm and wild silkworm. These TEs have experienced expansions and fixation in the domesticated silkworm but not in wild silkworm. Furthermore, the ML results indicated that purifying selection of TEs in the domesticated silkworm were significantly weaker than that in the wild silkworm. Interestingly, an adaptation insertion induced by BmMITE-2 was found, and this insertion can reduce the polymorphism of the flanking regions of its neighboring COQ7 gene. Our results suggested that TEs expanded and were fixed in the domesticated silkworm might result from demographic effects and artificial selection during domestication. We concluded that the data presented in this study have general implication in animal and crop improvements as well as in domestication of new species.

      • Ether-free polymeric anion exchange materials with extremely low vanadium ion permeability and outstanding cell performance for vanadium redox flow battery (VRFB) application

        <P><B>Abstract</B></P> <P>Ether-free poly(<I>p</I>-phenylene)-based anion exchange membrane materials with outstanding vanadium ion blocking properties are reported. The synthesized ionomers with a rigid backbone structure show small dimensional variations despite the high ion exchange capacities. Among the synthesized membranes, the QPPP-2 membrane with ion exchange capacity of 2.1 meq·g<SUP>−1</SUP> exhibits high anion (OH<SUP>−</SUP>, SO<SUB>4</SUB> <SUP>2−</SUP>, and Cl<SUP>−</SUP>) conductivity. The QPPP-2 membrane shows an extremely low diffusion coefficient (2.12 × 10<SUP>−9</SUP> cm<SUP>2</SUP> min<SUP>−1</SUP>) for VO<SUP>2+</SUP>, which is much lower than that of Nafion 115 (2.88 × 10<SUP>−6</SUP> cm<SUP>2</SUP> min<SUP>−1</SUP>), indicating the vanadium ion blocking property of QPPP-2. Moreover, the chemical and operational stabilities of the membranes are investigated <I>via</I> ex situ soaking tests in 0.1 M VO<SUB>2</SUB> <SUP>+</SUP> solution and in situ operation tests for 100 cycles, respectively. The vanadium redox flow battery single cell assembled with the QPPP-2 membrane exhibits a coulombic efficiency of 99%, voltage efficiency of 87%, and energy efficiency of 86% during 100 cycles at 80 mA cm<SUP>−2</SUP>. The excellent chemical, morphological, and electrochemical properties of the QPPP-2 membrane make it suitable for use in vanadium redox flow battery.</P> <P><B>Highlights</B></P> <P> <UL> <LI> AEMs are fabricated by a Ni(0)-catalyzed cross-coupling reaction. </LI> <LI> The synthesized ionomers indicated excellent dimensional and chemical stability. </LI> <LI> Ether-free polymeric AEMs showed extremely low vanadium ion cross over. </LI> <LI> The performance of developed AEM showed high EE (87%) with high capacity retention. </LI> </UL> </P> <P><B>Graphical abstract</B></P> <P>[DISPLAY OMISSION]</P>

        갈화 ( Puerariae flos ) 추출물이 Rat 혈중 Ethanol 농도에 미치는 영향

        Ethanol concentration in blood, brain and liver of rats was shown to be effectively lowered by arrowroot flower extract. The lowering effect for ethanol concentration in blood was maximum when measured after 1 hour from ethanol feeding. Hot water extract was more effective than 80% ethanol extract. The treatment of extract at 10 min. before ethanol feeding gave a better result than that at 10 min after or 1 hour before ethanol feeding. The ethanol concentration in brain and liver was lowered as found in the blood ethanol concentration. Acetaldehyde was not detected either in blood or the tissues. The optimal amount of the Puerariae flos was 55.7㎎/㎏·body weight. The newly developed analytical method using dichloromethane as extracting solvent was proven to be very effective in terms of speed and simplicity.

      • Chlorpropamide 2-hydroxylation is catalysed by CYP2C9 and CYP2C19 <i>in vitro</i>: chlorpropamide disposition is influenced by CYP2C9, but not by CYP2C19 genetic polymorphism

        <P>Aims</P><P>We evaluated the involvement of cytochrome P450 (CYP) isoforms 2C9 and 2C19 in chlorpropamide 2-hydroxylation <I>in vitro</I> and in chlorpropamide disposition <I>in vivo</I>.</P><P>Methods</P><P>To identify CYP isoforms(s) that catalyse 2-hydroxylation of chlorpropamide, the incubation studies were conducted using human liver microsomes and recombinant CYP isoforms. To evaluate whether genetic polymorphisms of CYP2C9 and/or CYP2C19 influence the disposition of chlorpropamide, a single oral dose of 250 mg chlorpropamide was administered to 21 healthy subjects pregenotyped for CYP2C9 and CYP2C19.</P><P>Results</P><P>In human liver microsomal incubation studies, the formation of 2-hydroxychlorpropamide (2-OH-chlorpropamide), a major chlorpropamide metabolite in human, has been best described by a one-enzyme model with estimated <I>K</I><SUB><I>m</I></SUB> and <I>V</I><SUB>max</SUB> of 121.7 ± 19.9 µ<SMALL>M</SMALL> and 16.1 ± 5.0 pmol min<SUP>−1</SUP> mg<SUP>−1</SUP> protein, respectively. In incubation studies using human recombinant CYP isoforms, however, 2-OH-chlorpropamide was formed by both CYP2C9 and CYP2C19 with similar intrinsic clearances (CYP2C9 <I>vs.</I> CYP2C19: 0.26 <I>vs.</I> 0.22 µl min<SUP>−1</SUP> nmol<SUP>−1</SUP> protein). Formation of 2-OH-chlorpropamide in human liver microsomes was significantly inhibited by sulfaphenazole, but not by <I>S</I>-mephenytoin, ketoconazole, quinidine, or furafylline. In <I>in vivo</I> clinical trials, eight subjects with the <I>CYP2C9</I>*<I>1/</I>*<I>3</I> genotype exhibited significantly lower nonrenal clearance [*<I>1/</I>*<I>3 vs.</I>*<I>1/</I>*<I>1</I>: 1.8 ± 0.2 <I>vs.</I> 2.4 ± 0.1 ml h<SUP>−1</SUP> kg<SUP>−1</SUP>, <I>P</I> < 0.05; 95% confidence interval (CI) on the difference 0.2, 1.0] and higher metabolic ratios (of chlorpropamide/2-OH-chlorpropamide in urine: *<I>1/</I>*<I>3 vs.</I>*<I>1/</I>*<I>1</I>: 1.01 ± 0.19 <I>vs.</I> 0.56 ± 0.08, <I>P</I> < 0.05; 95% CI on the difference − 0.9, − 0.1) than did 13 subjects with <I>CYP2C9</I>*<I>1/</I>*<I>1</I> genotype. In contrast, no differences in chlorpropamide pharmacokinetics were observed for subjects with the <I>CYP2C19</I> extensive metabolizer <I>vs.</I> poor metabolizer genotypes.</P><P>Conclusions</P><P>These results suggest that chlorpropamide disposition is principally determined by CYP2C9 activity <I>in vivo</I>, although both CYP2C9 and CYP2C19 have a catalysing activity of chlorpropamide 2-hydroxylation pathway.</P>

        Unexpected pulmonary edema and cardiac arrest following wedge resection of spontaneous pneumothorax -A case report-

        BackgroundReexpansion pulmonary edema is a rare but potentially lethal complication. We report a case of suspected reexpansion pulmonary edema that led to cardiac arrest.CaseA 16-year-old male patient underwent wedge resection due to right pneumothorax. The patient showed pink frothy sputum three hours following surgery, and a chest x-ray showed right unilateral pulmonary edema. Thirteen hours following surgery, the patient continuously showed pink frothy sputum and presented with severe hypoxemia, tachypnea, and tachycardia. After transferring to the intensive care unit (ICU), he developed ventricular tachycardia. Cardiopulmonary resuscitation was performed for 32 min. Chest X-ray showed diffuse bilateral pulmonary edema. Extracorporeal membrane oxygenation was performed. During the 65 days of ICU care, the patient became mentally alert. However, follow-up echocardiography revealed severe heart failure.ConclusionsRexpansion pulmonary edema can rapidly progress to diffuse bilateral pulmonary edema. Therefore, careful observation is required for the patients who show signs of pulmonary edema after reexpansion.

        The effect of FK506 (tacrolimus) loaded with collagen membrane and fibrin glue on promotion of nerve regeneration in a rat sciatic nerve traction injury model

        Background: Peripheral nerve injury is one of the most common injuries that might occur in oral and maxillofacialsurgery. The purpose of this study was to determine the effect of FK506 loaded with collagen membrane and fibringlue on the promotion of nerve regeneration after traction nerve injury in a rat model. Methods: Thirty male Sprague-Dawley rats were divided into three groups: group A (n = 10), a sham group whosesciatic nerve was exposed without any injury; and groups B (n = 10) and C (n = 10), which underwent tractionnerve injury using 200 g of traction force for 1 min. The injured nerve in group C was covered with a collagenmembrane soaked with FK506 (0.5 mg/0.1 mL) and fibrin glue. Functional analysis and microscopic evaluation wereperformed at 2 and 4 weeks after injury. Results: The sciatic function index was ? 5.78 ± 3.07 for group A, ? 20.69 ± 5.22 for group B, and ? 12.01 ± 4.20for group C at 2 weeks after injury. However, at 4 weeks, the sciatic function index was ? 5.58 ± 2.45 for group A, ?19.69 ± 4.81 for group B, and ? 11.95 ± 1.94 for group C. In both periods, statistically significant differences werefound among the groups (p<0.017). Histomorphometric evaluation revealed improved nerve regeneration in groupC compared to that in group B. However, no statistical differences in axonal density were found among the threegroups (p < 0.017). Conclusion: Localized FK506 with collagen membrane and fibrin glue could promote axonal regeneration in a ratmodel of traction nerve injury.
