        Advanced peri-implantitis cases with radical surgical treatment

        Purpose: Peri-implantitis, a clinical term describing the inflammatory process that affects the soft and hard tissues around an osseointegrated implant, may lead to peri-implant pocket formation and loss of supporting bone. However, this imprecise definition has resulted in a wide variation of the reported prevalence; ${\geq}10%$ of implants and 20% of patients over a 5- to 10-year period after implantation has been reported. The individual reporting of bone loss, bleeding on probing, pocket probing depth and inconsistent recording of results has led to this variation in the prevalence. Thus, a specific definition of peri-implantitis is needed. This paper describes the vast variation existing in the definition of peri-implantitis and suggests a logical way to record the degree and prevalence of the condition. The evaluation of bone loss must be made within the concept of natural physiological bony remodelling according to the initial peri-implant hard and soft tissue damage and actual definitive load of the implant. Therefore, the reason for bone loss must be determined as either a result of the individual osseous remodelling process or a response to infection. Methods: The most current Papers and Consensus of Opinion describing peri-implantitis are presented to illustrate the dilemma that periodontologists and implant surgeons are faced with when diagnosing the degree of the disease process and the necessary treatment regime that will be required. Results: The treatment of peri-implantitis should be determined by its severity. A case of advanced peri-implantitis is at risk of extreme implant exposure that results in a loss of soft tissue morphology and keratinized gingival tissue. Conclusions: Loss of bone at the implant surface may lead to loss of bone at any adjacent natural teeth or implants. Thus, if early detection of peri-implantitis has not occurred and the disease process progresses to advanced peri-implantitis, the compromised hard and soft tissues will require extensive, skill-sensitive regenerative procedures, including implantotomy, established periodontal regenerative techniques and alternative osteotomy sites.

        The Role of Organizational Slack and Human Capital in Managing Performance Shocks: An Analysis of US Nursing Homes and Hurricane Katrina

        Public service organizations employ buffers to ward off disruptive performance shocks such as disasters, budget cuts, and uncertain events. Existing applications on buffering focus on how slack human resources are reallocated to high-risk areas during times of uncertainty to dampen the impact these events have on core organizational processes. Besides slack, the quality of human capital is also important. Skilled personnel have the technical acumen and expertise to help managers navigate through treacherous times and still deliver public services. Yet, existing work does not disentangle the distinct impact of these two buffers. Using a panel dataset of nursing homes in Louisiana and Mississippi, I explore how personnel slack and human capital helped mitigate the devastation that Hurricane Katrina had on a very fragile, dependent, and sensitive segment of the population. The findings suggest that human capital buffers the impact of Hurricane Katrina on health deficiencies and physical dependency. In contrast, slack capacity amplified the impact of Katrina on health deficiencies.

      • Phosphorylation and isoform use in p120-catenin during development and tumorigenesis

        P120-catenin is essential to vertebrate development, modulating cadherin and small-GTPase functions, and growing evidence points also to roles in the nucleus. A complexity in addressing p120-catenin's functions is its many isoforms, including optional splicing events, alternative points of translational initiation, and secondary modifications. In this review, we focus upon how choices in the initiation of protein translation, or the earlier splicing of the RNA transcript, relates to primary sequences that harbor established or putative regulatory phosphorylation sites. While certain p120 phosphorylation events arise via known kinases/phosphatases and have defined outcomes, in most cases the functional consequences are still to be established. In this review, we provide examples of p120-isoforms as they relate to phosphorylation events, and thereby to isoform dependent protein-protein associations and downstream functions. We also provide a view of upstream pathways that determine p120's phosphorylation state, and that have an impact upon development and disease. Because other members of the p120 subfamily undergo similar processing and phosphorylation, as well as related catenins of the plakophilin subfamily, what is learned regarding p120 will by extension have wide relevance in vertebrates.

        Affinity Apheresis for Treatment of Bacteremia Caused by Staphylococcus aureus and/or Methicillin-Resistant S. aureus (MRSA)

        Staphylococcus aureus (SA) bacteremia is associated with high mortality, and often results in metastatic infections. The methicillin-resistant SA (MRSA) is an urgent health care issue, as nosocomial infections with these bacteria represent limited treatment alternatives. Samples of whole blood containing challenge inoculums of SA and MRSA strains were passed through columns packed with surfaceheparinized polyethylene beads. The bound bacteria were eluted and quantitatively determined by culturing and by real-time PCR. Significant amounts of both SA and MRSA adhered to the heparinized beads (more than 65% of inoculated bacteria). After rinsing with buffer at high ionic strength, viable bacteria or bacterial DNA were eluted from the columns, indicating that the binding was specific. The conclusions that can be made from these experiments are that, as earlier reported in the literature, the high affinity of SA to heparin is retained in whole blood, and MRSA in whole blood binds to heparin with similar or higher affinity than SA. It should be possible to lower the amount of SA and/or MRSA from the blood of infected patients to levels that could be taken care of by the immune system. In previous studies, we have shown that passing blood from septic patients over beads coated with end-point-attached, biologically active heparin is a useful technique for regulating the levels of heparinbinding cytokine. These findings in combination with the present findings indicate the possibility of creating an apheresis technology for treatment of sepsis caused by SA and/or MRSA.

      • Differential Expression of OATP1B3 Mediates Unconjugated Testosterone Influx

        <P>Castration-resistant prostate cancer (CRPC) has greater intra-tumoral testosterone concentrations than similar tumors from eugonadal men; simple diffusion does not account for this observation. This study was undertaken to ascertain the androgen uptake kinetics, functional, and clinical relevance of de novo expression of the steroid hormone transporter OATP1B3 (SLCO1B3). Experiments testing the cellular uptake of androgens suggest that testosterone is an excellent substrate of OATP1B3 (Km =23.2 mu mol/L; V-max = 321.6 pmol/mg/minute), and cells expressing a doxycycline-inducible SLCO1B3 construct had greater uptake of a clinically relevant concentration of 3H-testosterone (50 nmol/L; 1.6-fold, P = 0.0027). When compared with Slco1b2 (-/-) mice, Slco1b2 (-/-)/hSLCO1B3 knockins had greater hepatic uptake (15% greater AUC, P = 0.0040) and lower plasma exposure to 3H-testosterone (17% lower AUC, P = 0.0030). Of 82 transporters genes, SLCO1B3 is the second-most differentially expressed transporter in CRPC cell lines (116-fold vs. androgen-sensitive cells), with a differentially spliced cancer-type ct-SLCO1B3 making up the majority of SLCO1B3 expression. Overexpression of SLCO1B3 in androgen-responsive cells results in 1.5-to 2-fold greater testosterone uptake, whereas siRNA knockdown of SLCO1B3 in CRPC cells did not change intracellular testosterone concentration. Primary human prostate tumors express SLCO1B3 to a greater extent than ct-SLCO1B3 (26% of total SLCO1B3 expression vs. 0.08%), suggesting that androgen uptake in these tumor cells also is greater.         이 글은 테르툴리아Tertulia 연구에서 소리를 만들어내는 아이디어와 기술 방법에 대한 논의로 대략 구성되는데, 테르툴리아 프로젝트는 시각 예술가 에두아르도 몰리나리 Eduardo Molinari와 협동으로, 2009년 초여름 자그레브Zagreb의 미로고즈Mirogoj 묘지에서 열린 23번째 유로카즈 페스티발 Eurokaz Festival에서 발표된 예술 설치 작품이다. 스펙트랄 모델링 방법이 이 프로젝트의 크고 작은 구조 체계와 이들의 시적 가치와 함께 논의된다. 테르툴리아 프로젝트 맥락 속에서의 소리와 공간의 관계는, 시간의 흐름에 따라 변형하는 스펙트럼의 관점을 통해 다시 논의되기도 한다. This paper briefly comprises the research, ideas and techniques for creating the sound component of the ‘Tertulia’ project, an art installation composed in collaboration with visual artist Eduardo Molinari and produced by the 23<SUP>rd</SUP> Eurokaz Festival, realized at Mirogoj Cemetery in Zagreb during the early summer of 2009. Spectral Modeling techniques are discussed in relation to the micro-and macro-structure of the project, as well as to their poetic potency. The relations between sound and space within the context of ‘Tertulia’ are also revisited through the perspective of spectral transformations over time.
