개의 여러가지 질병상태에서 근복적인 치료나 보조적인 치료제로서의 flumethasone 투여는 아주 좋은 치료효과를 나타냈다. 우리가 병원에서 상용하는 flumethasone의 평균 투여량은 0.125$\~$0.5mg으로서 경구 또는 주사로 투여했다. 경구적으로 투여하는 주된 사용량은 1일 0.03125mg$\~$0.125mg이며 환축의 크기, 치유과정 그리고 병증의 심한 정도에 따라 양을 가감했다. 투여경로를 달리하여도 약효에 차이가 생기지는 않았다. flumethasone의 소염효과, 당신생효과 및 항독소적 치료효과에서 기존의 corticoids 투여시 나타나는 효과에 비해 정성적인 차이는 없었으나 실험실 결과와 병원에서 사용한 임상결과 flumethasone은 hydrocortisone보다 300$\~$400배 그리고 prednisone이나 prednisolone 보다는 60$\~$80배나 강한 약효를 발휘하는 것으로 나타났다. 임상적용에서 나타난 감동적인 효과에서 이러한 flumethasone의 약효에 대한 자료들이 입증되었다. 또한 치료된 동물들이 보여준 반응은 flumethasone의 약효 지속시간이 기존의 corticoids보다 더 긴 것으로 나타났다. 앞에서 언급했듯이 flumethasone 투여시는 전해질 불균형에 의해 유발되는 다음다갈증이나 다뇨증, 식욕촉진 등의 부작용이 전혀 나타나지 않았다. 이러한 flumethasone의 안전성은 기존의 corticoids로 비만동물이나 나이든 환축을 치료할때 자주 나타났던 부작용업이 장기간 치료할 수 있으므로 아주 중요하다.

Purpose: Newborn screening (NBS) methods to detect congenital hypothyroidism (CH) vary regarding whether total thyroxine (T4), thyroid stimulating hormone (TSH), or both are measured. Neonates with low T4 and normal or low TSH (lowT4/TSH) may only be detected by T4-inclusive methods or age-dependent repeat screens. Premature neonates and those with pituitary-hypothalamic disorders frequently manifest lowT4/TSH. Methods: This is a retrospective case-study of newborns who were screen-positive for lowT4/TSH in Alabama in 2009–2016 using a combined T4 and TSH method and 2 routine NBS. The clinical, laboratory, and final diagnosis after 3 years were determined. Results: Over 8 years, 225 infants were referred to our institution for evaluation and treatment of CH. Twelve infants were screen-positive for lowT4/TSH by first or second NBS. Four of the 12 infants had permanent CH (30%): 2 with primary and 2 with central etiologies. One infant with moderately severe central CH was only detected by the routine second NBS. Six of 7 premature infants had elevated TSH on serum confirmation labs consistent with a delay in hypothalamic-pituitary maturation, yet 2 of these patients were later established to have permanent primary CH. While most cases of lowT4/TSH resolved by 3 years of age, several neonates had extended periods of moderate to severe hypothyroxinemia prior to detection and treatment. Conclusion: One third of the infants with lowT4/TSH on NBS in this study had permanent CH. These results emphasize the importance of T4-based assay methods, subsequent (repeat) screens and long-term follow-up in the management of neonates with lowT4/TSH on newborn screen.
Internet search analytics for shoulder arthroplasty: what questions are patients asking?

Background: Common questions about shoulder arthroplasty (SA) searched online by patients and the quality of this content are unknown. The purpose of this study is to uncover questions SA patients search online and determine types and quality of webpages encountered. Methods: The “People also ask” section of Google Search was queried to return 900 questions and associated webpages for general, anatomic, and reverse SA. Questions and webpages were categorized using the Rothwell classification of questions and assessed for quality using the Journal of the American Medical Association (JAMA) benchmark criteria. Results: According to Rothwell classification, the composition of questions was fact (54.0%), value (24.7%), and policy (21.3%). The most common webpage categories were medical practice (24.6%), academic (23.2%), and medical information sites (14.4%). Journal articles represented 8.9% of results. The average JAMA score for all webpages was 1.69. Journals had the highest average JAMA score (3.91), while medical practice sites had the lowest (0.89). The most common question was, “How long does it take to recover from shoulder replacement?” Conclusions: The most common questions SA patients ask online involve specific postoperative activities and the timeline of recovery. Most information is from low-quality, non-peer-reviewed websites, highlighting the need for improvement in online resources. By understanding the questions patients are asking online, surgeons can tailor preoperative education to common patient concerns and improve postoperative outcomes.
