        Effect of Different Inclusion Level of Condensed Distillers Solubles Ratios and Oil Content on Amino Acid Digestibility of Corn Distillers Dried Grains with Solubles in Growing Pigs

        The purpose of this experiment was to determine and compare the digestibility of crude protein (CP) and amino acids (AA) in full-oil (no oil extracted) and de-oiled (oil extracted) corn distillers dried grains with solubles (DDGS) with different condensed distillers solubles (CDS) ratios. Six barrows ($29.6{\pm}2.3kg$) fitted with ileal T-cannula were allotted into a $6{\times}6$ Latin square design. Each period was comprised of a 5-d adaption period followed by a 2-d collection of ileal digesta. The five test diets contained 62% DDGS as the sole source of AA. A nitrogen-free diet was used to measure the basal endogenous losses of CP and AA. Chromic oxide (0.3%) was used as an index in each diet. The results showed that CP and AA were very similar in 5 DDGS, but the standardized ileal digestibility (SID) of lysine (from 56.16% to 71.15%) and tryptophan (from 54.90% to 68.38%) had the lowest values and largest variation within the essential AA, which suggests reduced availability of AA and different levels of Maillard reactions in the five DDGS. The apparent ileal digestibility and SID of CP and most of AA in full-oil DDGS (sources 1 and 2) were greater (p<0.05) than de-oiled DDGS (sources 3, 4, and 5). Comparing the AA SID in the 5 DDGS, full-oil with low CDS ratio DDGS (source 1) had non-significantly higher values (p>0.05) than full-oil with high CDS ratio DDGS (source 2); however, the SID of most AA of de-oiled with low CDS ratios DDGS (source 3) were non-significantly lower (p>0.05) than de-oiled with high CDS ratio DDGS (source 4); and the de-oiled DDGS with middle CDS ratio (source 5) but with different drying processing had the lowest SID AA values. In conclusion, de-oiled DDGS had lower SID of CP and AA than full-oil DDGS; a higher CDS ratio tended to decrease the SID of AA in full-oil DDGS but not in de-oiled DDGS; and compared with CDS ratio, processing, especially drying, may have more of an effect on AA digestibility of DDGS.

        한국인 성인남성 사체에서 시행한 인중 구륜근 섬유들의 효소-조직화학적 분석

        The orbicularis oris muscle (OOM) is a very important muscle that originate from the second branchial arch and is innervated by the facial nerve. The aim of this study was to elucidate distribution types of two muscle fibers that composing OOM by using enzyme‐ histochemical examinations and tried to make a basis for a clinical application. The fresh frozen tissues from the superior and inferior portions of the OOM were taken from post mortem 65‐year‐old Korean male adult. Total five different sagittal sections were used on the midline of the philtrum, the middle portion of lower lip, the mouth corner, and each midlateral side of upper and lower mouth. We used enzyme‐histochemical staining such as Periodic Acid‐Schiff (PAS), Succinic Dehydrogenase (SDHase), reduced Nicotinamide Adenine Dinucleotide‐Tetrazolium Reductase (NADH‐TR), Adenosine Triphosphatase (ATPase) in pH 9.4, 4.6 and 4.3, and Modified Gomori Trichrome. There were about 30.24 % type 1 muscle fiber and 65.40 % type 2 muscle fiber in the midline of the philtrum (p < 0.05). Enzyme‐histochemical staining is very useful and innovative method to elucidate characteristics of muscle fibers. We expect that chiloplasty and reconstruction of the lip portions for cleft lip patients, based on these results, are better to recovery function and aesthetic. However, we have some problems as an intramuscular variability and the inter‐individual variation etc. Therefore we have to make progress these studies continuously to overcome these problems.

        Laugier-Hunziker 증후군 1 예

        Laugier-Hunziker syndrome (LHS) is a rare acquired benign macular hyperpigmentation of the lips and oral mucosa which is often associated with longitudinal melanonychia. LHS is known to be an entirely benign condition with no systemic manifestations, which requires patient reassurance as the only intervention. The significance of this condition is due to its inclusion in differential diagnoses of pigmentary disorders of the oral mucosa, especially Peutz-Jeghers syndrome. We report a case of Laugier-Hunziker syndrome in a 66-year-old woman who presented with longitudinal pigmented bands on her fingernails and multiple, pigmented macules on the lip and tongue.         The Antioxidant Properties of Garlic Compounds: Allyl Cysteine, Alliin, Allicin, and Allyl Disulfide

        Garlic and garlic extracts, through their antioxidant activities, have been reported to provide protection againstfree radical damage in the body. This study investigated antioxidant properties of garlic compounds representing the four mainchemical classes, alliin, allyl cysteine, allyl disulfide, and allicin, prepared by chemical synthesis or purification. Alliin scav-enged superoxide, while allyl cysteine and allyl disulfide did not react with superoxide. Allicin suppressed the formation ofsuperoxide by the xanthine/xanthine oxidase system, probably via a thiol exchange mechanism. Alliin, allyl cysteine, and al-lyl disulfide all scavenged hydroxyl radicals; the rate constants calculated based on deoxyribose competitive assay were1.41.7. 1010, 2.12.2. 109, and 0.71.5. 1010 M. 1 second. 1, respectively. Contrary to previous reports, allicin did notexhibit hydroxyl radical scavenging activity in this study. Alliin, allicin, and allyl cysteine did not prevent induced microso-mal lipid peroxidation, but both alliin and allyl cysteine were hydroxyl scavengers, and allyl disulfide was a lipid peroxida-tion terminator. In summary, our findings indicated that allyl disulfide, alliin, allicin, and allyl cysteine exhibit different pat-terns of antioxidant activities as protective compounds against free radical damage.

      • Successful Treatment of Excessive Dynamic Airway Collapse with Noninvasive Positive Pressure Ventilation - A Case Report

        Introduction Excessive central airway collapse (ECAC) can present as unexplained dyspnea, wheezing or stridor. ECAC can easily be misdiagnosed as bronchial asthma, COPD and heart failure. ECAC can occur as a result of tracheobronchomalacia or excessive dynamic airway collapse (EDAC).There are various Methods of diagnosis and treatment of ECAC but it has to be tailored according patient’s circumstances. Case Report We present a 75-year-old lady with multiple comorbidities including vascular dementia who was brought in by her daughter after she noticed that her mother had wheezing, shortness of breath and reduced effort tolerance for the past 2 months. Her symptoms were worse on exertion and during sleep. She was given bronchodilator without any improvements. She was overweight and her STOPBANG score of 6. CT thorax showed posterior indentation of mid distal trachea with significant luminal narrowing. Unfortunately, she couldn’t comply to dynamic chest CT test due to vascular dementia. Flexible bronchoscopy done under conscious sedation confirmed almost total occlusion of both left and right main bronchus together with narrowing of tracheal diameter of more than 80% that were present only during expiration. Partial sleep study done showed evidence of OSA with an AHI of 58/hour, and a lowest oxygen saturation at 51%. Both her EDAC and OSA was treated successfully with a CPAP machine. On subsequent follow up, her daughter reported that her symptoms has resolved together with improvements in her effort tolerance. Discussion and Conclusion There are various Methods to diagnose and manage a patient with EDAC, some of which are invasive and thus are risky for elderly patients with multiple co-morbidities. Choices of investigations and treatment for this condition need to be tailored according to patient’s need and circumstances. CPAP in this case was given without real time titration under flexible bronchoscopy with success in her case.

      • Nose Stud Screw Aspiration- A Tiny Foreign Body with Potentially Huge Disaster: 2 Cases with Different Ways of Retrieval

        Nose piercing is traditionally a common practice among women of Indian ancestry. The practice of nose piercing has gained significant popularity in various communities worldwide for the past decade. The practice can potentially lead to aspiration of nose jewelleries such as screws of nose studs. Aspiration of small foreign bodies can be silent although potentially serious and life-threatening sequelae can happen in long term. Herein, we present 2 patients of Indian ancestry who aspirated screw of nose studs with different Methods of retrieval. Both of the patients are assymptomatic and foriegn body was found incidentally on chest radiograph. They could not recall the timing of aspiration of foreign body, which is nose stud screw in this case. Small foriegn body such the nose stud screw could potentially dislodge into subsegmental bronchi, rendering retrieval extermely difficult. The 2 cases are presented to show that the same type of foreign body can be retrieved easily if it remains in the main bronchus simply with flexible bronchoscope and grasping forceps, or require various method such as retrieval basket, magnet etc under rigid bronchoscopy. Small foriegn body could lead to serious complications such as severe infection, or massive hemoptysis if left in the airway. While aspiration of scarf pins among Muslim girls and ladies who wears a hijab, called the “hijab syndrome” have been extensively reported, there is a paucity of publications regarding “nosestud syndrome” among Indian communities. To the best of our knowledge, this is the first case report delineating aspiration of screw of nose studs and their retrieval Methods. The incidence and burden of nose stud aspiration remains unknown, and is likely underreported and underrecognized, as patients can remain asymptomatic. We hope that our case report can help to increase awareness and understanding of potential dangers of nose piercing.

        Stylocheiron abbreviatum (Euphausiacea, Crustacea): A New Record from the Korean Waters

        The euphausiid crustacean Stylocheiron abbreviatum G O. Sars, 1883 has been found in the southern waters of Jeju Island, Korea. This species is the third member of the genus Stylocheiron and the twelfth of the family Euphausiidae identified in Korean waters. Using descriptions of this species, a key to Korean euphausiids is presented.

      • Leaking Thoracic Aorta Aneurysm Causing Pleural Effusion: A Potential Disaster

        Introduction Thoracentesis is the first step in diagnosing a unilateral pleural effusion. Although malignancy remains the most common cause of a hemoserous effusion, we must be aware of other causes such as uremia, pulmonary infarction, post cardiac injury syndrome etc. We report a rare case of leaking thoracic aortic aneurysm masquerading as a unilateral hemoserous pleural effusion. Case Report A 79-year-old female with underlying hypertension, dyslipidemia and chronic kidney disease presented with a 2-weeks history of cough and dyspnea. She also reported a weight loss of 5 kilograms in 1 month. A Chest X-Ray (CXR) on presentation revealed a large left pleural effusion. She has exudative effusion based on Light's criteria. pleural fluid culture and cytology was negative. As her main differential diagnosis was malignancy, she was subjected to pleuroscopy to obtain a tissue diagnosis. During pleuroscopy, 1 liter of hemoserous fluid was drained. Apart from thickened parietal pleura, a large cystic mass was seen adhered to the parietal pleura posteriorly. Biopsies were taken from the parietal pleura and decision was made not to biopsy or aspirate the cystic mass during pleuroscopy. Computed tomography of the thorax done few weeks later showed a large thoracic aorta aneurysm with multiple penetrating atherosclerotic ulcers and mural thrombus. There was no pleural or lung mass detected. Patient was counseled for surgical repair but opted for conservative management. Discussion and conclusion Leaking thoracic aneurysm is an uncommon but potentially dangerous cause of hemoserous pleural effusion. A blind thoracentesis or biopsy of the cystic mass during pleuroscopy would have led to rupture of the aneurysm. Depending on the clinical context, it is safer to obtain a CT thorax prior to pleuroscopy when dealing with undiagnosed pleural effusion.
