Left ventricular outflow tract obstruction and systolic anterior motion of the mitral valve after myxomatous mitral valve repair is due to excess tissue of mitral valve among numerous other factors. A patient who had myxomatous mitral valve and acute mitral regurgitation due to chordae rupture was admitted for mitral valve repair. The hospital course of the patient was not significantly different compared to other patients. On the 40th postoperative day, the patient experienced syncope and came to ER. The patient showed severe left venthcular outflow tract obstruction and systolic anterior motion of mitral valve. The patient was treated medically and was discharged with normal echo findings. We report this case with brief review of related literatures.

Usual interstitial pneumonia(UIP) is relatively rare disease of lung parenchyme under the age of 50. Lung cancer is reported to occur as a complication of UIP, but has not been reported in young age patient with UIP in Korea. A 35-year-old man was admitted to the hospital because of dyspnea, cough and mild fever. He had been suffered from dyspnea during 3 years. The dyspnea worsened recent 1 month ago. He had been treated with pneumonia at another hospital during 4 months before this admission. Chest radiographs and High resolution CT showed subpleural and basal dominant reticular opacities in both lung with no change until now. He was smoker and his occupation was car engine-man dealing with benzene. UIP is suspected by history and radiologic findings. Operation for open lung biopsy was performed to obtain lung specimen via video-assisted thoracoscopic surgery. Patient's pathologic examination showed UIP combined with adenocarcinoma in RLL superior segment and posterobasal segment and RLL lobectomy was done. After operation, the patient has been in cancer free condition so far. We report a case of UIP combined with lung cancer in young man with brief review of the literature.

배경 및 목적: 본 연구는 표준화 과정을 거쳐서 개발된 ‘한국 실어증 감별진단 검사(Korean Test for Differential Diagnosis of Aphasia: KTDDA, 이하 KTDDA)’에 대한 공인타당도를 검증하는 것을 목적으로 한다. 방법: 공인타당도 검증을 위해 성인 언어장애 환자 10명을대상으로 최근에 개발된 KTDDA와 임상현장에서 많이 사용되고 있는 ‘파라다이스․한국판-웨스턴 실어증검사(Paradise․Korean version- Western Aphasia Battery: P․K-WAB,이하 P․K- WAB)’를 실시하였다. 결과: 분석 결과 KTDDA의 중증도와 P․K-WAB의 실어증 지수(Aphasia Qutient: AQ) 사이의 상관계수 0.863 (p < .01), 중증도와 언어지수(Language Quotient: LQ)의 상관계수 r = .980 (p < .01)로 유의미한 상관을 보였다. 각 하부 검사 사이에서도 따라말하기(r = .955, p < .01), 이름대기(r = .942, p < .01), 유창성(r = .807, p< .01), 읽기(r = .793, p < .01), 쓰기(r = .780, p < .01), 청각적이해력(r = .705, p < .05)에서도유의미한 상관을 보였다. 논의 및 결론: 본 연구의 결과 KTDDA가 실어증 환자를 진단하는공식적인 검사도구임을 재확인할 수 있었고, 기존의 P․K-WAB과의 공인타당도의 객관적 검증을 통해서 향후 실어증 진단에 유용하게 활용할 수 있을 것이다. Background & Objectives: The purpose of this study was to demonstrate the concurrent validity of the standardized “Korean Test for the Differential Diagnosis of Aphasia (KTDDA).” Methods: For concurrent validity, 10 aphasic patients were evaluated with the recently developed KTDDA and the Paradise․Korean type-Western Aphasia Battery (P․K-WAB). Results: The correlation between the severity of the KTDDA and the aphasia quotient (AQ) of the K-WAB was significant (r = .863, p < .01). The correlation between the severity of the KTDDA and the language quotient (LQ) of the K-WAB was also significant (r = .980, p < .01). The repetition subtest (r = .955, p < .01), the naming subtest (r = .942, p < .01), the fluency subtest (r = .807, p < .01), the reading subtest (r = .793, p < .01), the writing subtest (r = .780, p < .01), and the auditory comprehension subtest (r = .705, p < .05) between the KTDDA and the K-WAB also had statistically significant correlations. Discussion & Conclusion: We determined that the KTDDA is the official standardized tool in the assessment of aphasia, and its clinical usefulness is based on its concurrent validity with the K-WAB.

The present study was designed to find pharmacologically active compund against ariway in flammation from the roots of Asparagus cochinchinensis. The 70% ethanol exreact of the roots of A. cochinchinsis (ACE)was found to inhibit IL 6 production from IL_1B treated lung epithelial cells(A549)and the major constiuent, methyl protodiscin (MP), also stronglay inhibited the production dif IL-6,IL-8, and tumor neceosis factir (TNF) a from A549cellsat10-100uM. Thisdownregulating effect of proimflammatory cytikine pridyction was found to be mediated, at least in part,via inhibition of c-Jun N-terminal kinase(JNK) and C-Jun activatuib pathway. When examined in an in vivo mofrl of airway inflammation in mice, lipopolysaccharide-(LPS) induced acute lung injury, ACE, and MP significantly inhibited cell infiltration in the bronchialveolar lavage fluid by the oral treatment at doses fo 100-400mg/kg and 30-60mg/kg, respectively.MP also inhibited the production of Proinflammatory cytikines such as 1L-6, TNF-a, and 1L-1B in l ung tissue. All of these findings provide scientific evjdence supporting the role of A. cochinchinisi as a herbal remedy in treating ariway intlammation and also suggest a therapeutic balue of MP on airway inflammatory disriders.
Quinolone Alkaloids from Evodiae Fructus and Their Inhibitory Effects on Monoamine Oxidase

1-Methyl-2-undecyl-4(1H)-quinolone (1) was previously isolated as a selective MAO-B inhibitor from the Evodiae Fructus. Further bioassay-guided purification led to the identification of five known quinolone alkaloids, 1 -methyl-2-nonyl-4(1H)-quinolone (2), 1-methyl-2-[(Z)-6-unde-cenyl]-4(1 H)-quinolone (3), evocarpine (4), 1-methyl-2-[(6Z,9Z)-6,9-pentadecadienyl]-4(1H)-quinolone (5), and dihydroevocarpine (6). All the isolates showed more potent inhibitory effects against MAO-B compared to MAO-A. The most MAO-B selective compound 5 among the isolates inhibited MAO-B in a competitive manner, according to kinetic analyses by Lineweaver-Burk reciprocal plots.
Determination of Neutron Absorption Fraction Factor in Manganese Sulfate Bath System

The correction factor of neutron fraction absorbed by $^{55}$ Mn in the MnSO$_4$ bath was determined for the absolute measurement of neutron emission rate by using the solution circulation-type manganese sulfate bath system. For the determination of this correction factor, I/f, the atomic number desnsity and the effective neutron capture cross section data of Mn, S and impurity elements in the MnSO$_4$ solution were determined. For the atomic number density determination, the MnSO$_4$ solution concentration was determined by using the volumetric EDTA titration and gravimetric method. The impurity contents were analyzed by using the ICP method. For the calculation of effective neutron capture cross sections, a FORTRAN computer program EASCAL was developed in this study. in which Westcott's parameters and Axton's empirical relations are used.
