Background/AimsNatural history of chronic intestinal pseudo-obstruction (CIPO), a rare disease characterized by episodes of non-mechanical obstruction, is unclear in adults. This study evaluates the clinical course of CIPO and palliative care needs of patients. MethodsFrom October 2010 to September 2021, 74 patients who underwent cine MRI and had a definitive diagnosis of CIPO were prospectively included. We investigated disease etiology and outcomes, age at onset, nutritional status at consultation (body mass index and serum albumin), hydrogen breath test results, and total parenteral nutrition (TPN) during the disease course. ResultsForty-seven patients (64%) were women, with a mean age of 44 years at onset and 49 years at diagnosis. Primary CIPO was observed in 48 patients (65%). Secondary CIPO was observed in 26 cases (35%), of whom 18 (69%) had scleroderma. The mean body mass index, serum albumin level, and hydrogen breath test positivity rate were 17 kg/m2, 3.8 mg/dL, and 60%, respectively. TPN and invasive decompression therapy were required by 23 (31%) and 18 (24%) patients, respectively. Intestinal sterilization was performed in 51 (69%) patients and was effective in 33 (65%); of these, 28 (85%) were taking metronidazole. Seven (9%) patients used opioids. There were 9 deaths (12%), including 5 (56%) from infection and 2 (22%) from suicide. Of the deaths, 6 (67%) and 4 (44%) underwent TPN management and decompression therapy, respectively. Fifty-one patients (69%) wanted palliative care. ConclusionCIPO is a rare, severe, and under-recognized disease. Standardization of treatment strategies, including palliative care and psychiatric interventions, is desired.
Off-stoichiometry Effect on Network Structure and Tensile Properties of an Epoxy Resin

Epoxy resins, which have a network structure formed via curing reactions of epoxy and amine compounds, are often used as a composite with inorganic fillers. In general, either an epoxy or amine component was segregated at the filler interface, meaning that the reaction at the interface would proceed non- stoichiometrically. The objective of this study is to give a better understanding of how off-stoichiometry affects the network, thereby the material properties. The epoxy resins were obtained by the reaction of di-functional epoxy and amine compounds at various ratios (p). Higher p value corresponds to an excess of epoxy groups compared to amino ones. The glass transition temperature decreased with increasing p, suggesting a plasticizing effect of unreacted epoxies. Young’s modulus remained unchanged at a lower p, while it started to decrease at a higher p. Discussion on the correlation with the network will be given.

<P>Double-gate single-electron transistors (SETs) were fabricated by chemical assembling using electroless gold-plated nanogap electrodes and chemisorbed chemically synthesized gold nanoparticles. The fabricated SET showed periodic and stable Coulomb oscillations under application of voltages of both gates. The sole SET also exhibited all two-input logic operations?XOR, XNOR, NAND, OR, NOR, and AND?with an on/off ratio of 10<SUP>2</SUP>. This study was to investigate annual trends of physique and physical fitness of 4 and 5 years old boys from 1969 to 2009. Annual trends of physique were considered by height, weight, and BMI. In addition, annual trends of physical fitness were examined by 20m dash, tennis ball throw, standing long jump, one-leg hop and hanging from a horizontal bar. No further shifts toward earlier or larger growth were seen among parameters of physique. Among parameters of motor performance, declining or stagnation trends were seen. Though stagnation trend was seen in height, declining trend was seen in weight. In physical fitness, declining trends were seen especially in tennis ball throw and hanging from a horizontal bar. From these, decreased upper body muscle strength due to decrease in muscle mass was suspected. Also, stagnation or declining trend in other items of physical strength of preschool boys was seen. Considered with the findings in the other reports, the possibility of physical strength is lowered in the coming years was suggested.

A 91-year-old man with chronic cognitive impairment underwent shunt embolization for portosystemic encephalopathy (PSE). He experienced intermittent episodes of impaired consciousness and decreased cognitive function and activities of daily living (ADL), for which Alzheimer disease was suspected. On admission, he was in a coma and PSE was diagnosed based on his high ammonia level and the computed tomography findings. After shunt embolization, the patient fully recovered from the impaired consciousness and experienced no recurrence. The patient’s Revised Hasegawa Dementia Scale and Mini-Mental State Examination scores improved significantly from 12 and 17 to 30 and 29 points, respectively. The Barthel Index score also improved from 55/100 to 85/100, suggesting a marked improvement in ADL. PSE progresses slowly in very old patients and may mimic the clinical course of Alzheimer disease but without liver enzyme abnormalities. Therefore, it should be distinguished in every dementia case.

Conventional transarterial chemoembolization (TACE) using ethiodized oil and gelatin sponge (GS) particles is a standard treatment for unresectable BCLC-B stage hepatocellular carcinoma (HCC). Ethiodized oil can cause temporary embolic micro-interactions in tumor sinuses, portal veins, hepatic venous sinuses, and arteries as a temporary embolic material for the microvasculature. Using GS particles as an added embolic material, strong ischemic effects can be achieved not only in HCC, but also in the surrounding liver parenchyma. In recent years, various technical innovations in TACE using ethiodized oil have been made in Japan to improve the outcomes of TACE, such as a device for emulsifying ethiodized oil and water-soluble anticancer drugs, the use of intraoperative embolization guidance software to plan embolization during TACE, and the introduction of various microcatheters. This report examines some of the technical innovations that have been adopted to improve TACE outcomes.

<P>We demonstrate a broadband and long-distance intermediate frequency-over-fiber (IFoF) transmission scheme employing a transmitter composed of parallel intensity/phase (IM/PM) modulators with appropriate bandwidth allocations to IM and PM. Due to the proposed scheme, we can eliminate all the null frequencies caused by dispersion-induced RF power fading, which, in turn, enables us to significantly increase the available bandwidth. In addition, our system does not require any synchronization between IM and PM, which reduces complexity compared to conventional parallel transmitter architecture. We successfully transmitted 20 <TEX>$\times$</TEX> 360 MHz filtered orthogonal-frequency-division-multiplexed signals corresponding to a common public radio interface equivalent data rate of 524.28 Gbps over a 30- and 40-km single-mode fiber satisfying the 8% threshold for the error-vector magnitude values for all the subcarriers. These results show that our proposed IFoF transmission scheme is scalable to long-distance mobile fronthaul links for 5G and beyond.</P>
