        손마사지가 허혈성 심질환 환자의 불안에 미치는 효과

        The purpose of this study was to investigate the effect of a hand massage program on anxiety, vital sign and pain in clients with ischemic heart disease. The design utilized for this stuffy was quail-experimental with a nonequivalent control group non-synchronized design The subjects were fifty-four patients, twenty-eight for the experimental and twenty-six for the control group, who were admitted with ischemic heart diease at a cardiac intensive care unfit in K medical center of K university. This study was carried out from May, 1999 to March, 2000. The level of anxiety and pain measured by Visual Analogue Scale, systolic blood pressure, diastolic blood pressure and pulse rate were measured before and after hand massage, the state of Anxiety was measured by the Spielberger (1970) scale at admission and after hand massage for three days. The collected data were processed by using the SPSS PC program and analyzed using χ2-test and t-test. The result of this study are as follows : 1. The scores of VAS anxiety and State of anxiety of the experimental group were lower than those of the control group. 2. The degree of systolic blond pressure, diastolic blood pressure and pulse rate of the experimental group were lower than that of the control group. 3. The score of VAS pain of the experimental group was lower than that of the control group. The results suggested that hand massage can decrease VAS anxiety, State of anxiety, vital signs and VAS pain of patients who were admitted with ischemic heart disease at cardiac intensive care unit Therefore, It Is proposed that hand massage is an appropriate nursing intervention to relieve anxiety of the patients who were admitted with ischemic heart disease at a cardiac intensive care unit.

        노인전문간호사의 역할과 기능

        Purpose : The elderly population in the Republic of Korea was over 8.3% in 2003. This segment is projected to grow to 14.4% by 2019, with average life expectancy at 77.5 years for men and 84.1 year for women. According to significant increases in elderly population over 80 years of age, the demand of expert care for the bedridden and those suffering from dementia will be increased. The expanding role of nurses caring for the elderly requires that we properly define the role of the gerontological nurse practitioner (GNP). Method : The authors collected and analyzed the following sources available from nations where systems have already been in place for the gerontological nurse practitioner: educational manuals, textbooks, research papers, and Web sites for American Nurses Credentialing Center and Gerontological Nurse Practitioners. Result : The facilities and settings where GNPs serve are in-patient and out-patient acute care settings, long term care facilities, respite care facilities, day care centers, home-care agencies, community centers for the elderly, continuing care retirement communities. Although there are differences in role by setting of service, GNPs are nurse clinicians, educators, counselors, coordinators, facility administrators, researchers, advisors in ethical decision making, health services managers, self-care trainers, preventative-care administrators, environmental managers, advocates, program evaluators, information providers, and emergency responders. Conclusion : The role and functions of GNPs according to the facilities and settings in South Korea should be analyzed and developed.

        손마사지가 허혈성 심질환 환자의 수면에 미치는 효과

        The purpose of this study was to investigate the effect of hand massage program on total sleep hours at night and satisfaction of sleep in clients with ischemic heart disease. The design utilized for this study was quasi-experimental with a nonequivalent control group non-synchronized design. The subjects were fifty-four patients, twenty-eight for the experimental and twenty-six for the control group, who were admitted with ischemic heart diease at cardiac intensive care unit in K medical center of K university. This study was carried out from May, 1999 to March, 2000. Hand massage was carried out at 8∼9 PM, once a day for 3 days. Total sleep hours at night were measured from 9 PM through 6AM next morning. Sleep hours of subjects were observed 30 minutes interval. Satisfaction of sleep was measured by Visual Analogue Scale(VAS) at 7 AM next morning. The collected data were processed by using the SPSS PC program and analyzed using χ2-test and repeated measures of ANOVA. The result of this study are as follows: 1. The total sleep hours at night of the experimental group were not higher than those of the control group. 2. The satisfaction of sleep of the experimental group was not higher than that of the control group. In conclusion, hand massage did not promoted sleep in ICU clients with ischemic heart disease.

      • Molecular characterization of the autophagy-related gene Beclin-1 from the olive flounder (<i>Paralichthys olivaceus</i>)

        <P><B>Abstract</B></P><P>Autophagy is an important cellular response to starvation and stress, and plays critical roles in embryogenesis, development, cell death, cancer, and immunity. Beclin-1 is one of the central regulators of autophagy in mammals. In the present study, we isolated a PoBeclin-1 cDNA from the olive flounder (<I>Paralichthys olivaceus</I>) by screening a flounder gill cDNA library and rapid amplification of cDNA ends (RACE). The PoBeclin-1 cDNA we isolated encodes a 447-amino acid polypeptide containing a conserved Bcl-2-binding domain. The deduced amino acid sequence of PoBeclin-1 showed high degrees of sequence identity (80.5–95.3%) with Beclin-1 from human, frog, mouse, zebrafish, and pufferfish. PoBeclin-1 transcripts were detected from 1 day post-hatching and were found to be ubiquitously expressed in the healthy flounder. Expression of PoBeclin-1 mRNA was increased in the kidney and spleen of flounders challenged with viral hemorrhagic septicemia virus (VHSV). When infected with VHSV, PoBeclin-1-overexpressing HINAE cells had low level (about 26%) of VHSV G transcripts compared to control cells. Taken together, these results suggest that PoBeclin-1 may play a role in the innate immune response to viral infection in the flounder.</P> <P><B>Highlights</B></P><P>► PoBeclin-1 cDNA encodes a 447-aa polypeptide containing a conserved BH3 domain. ► Expression of PoBeclin-1 mRNA was increased in flounders challenged with VHSV. ► Overexpression of PoBeclin-1 decreased VHSV-G mRNA levels. ► PoBeclin-1 may play a role in the innate immunity to VHSV infection.</P>

        Gamma Knife Radiosurgery for Brain Metastases from Breast Cancer

        Objective : The authors conducted a retrospective cohort study to determine prognostic factors and treatment outcomes of brain metastases (BM) from breast cancer (BC) after Gamma Knife radiosurgery (GKS). Methods : Pathologic and clinical features, and outcomes were analyzed in a cohort of 62 patients with BM from BC treated by GKS. The Kaplan- Meier method, the log-rank test, and Cox's proportional hazards model were used to assess prognostic factors. Results : Median survival after GKS was 73.0 weeks (95% confidence interval, 46.0-100.1). HER2+ [hazard ratio (HR) 0.441; p=0.045], Karnofsky performance scale (KPS) ${\geq}70$ (RR 0.416; p=0.050) and systemic chemotherapy after GKS (RR 0.282; p=0.001) were found to be a favorable prognostic factor of overall survival. Actuarial local control (LC) rate were $89.5{\pm}4.5%$ and $70.5{\pm}6.9%$ at 6 and 12 months after GKS, respectively. No prognostic factors were found to affect LC rate. Uni- and multivariate analysis revealed that the distant control (DC) rate was higher in patients with; a small number (${\leq}3$) of metastasis (HR 0.300; p=0.045), no known extracranial metastasis (p=0.013, log-rank test), or the HER2+ subtype (HR 0.267; p=0.027). Additional whole brain radiation therapy and metastasis volume were not found to be significantly associated with LC, DC, or overall survival. Conclusion : The treatment outcomes of patients with newly diagnosed BM from BC treated with GKS could be affected primarily by intrinsic subtype, KPS, and systemic chemotherapy. Therapeutic strategy and prognosis scoring system should be individualized based on considerations of intrinsic subtype in addition to traditionally known parameters related to stereotactic radiosurgery.

        Optimum maintenance scenario generation for existing steel-girder bridges based on lifetime performance and cost

        This paper proposes a practical and realistic method to establish an optimal lifetime maintenance strategy for deteriorating bridges by considering the life-cycle performance as well as the life-cycle cost. The proposed method offers a set of optimal tradeoff maintenance scenarios among other conflicting objectives, such as minimizing cost and maximizing performance. A genetic algorithm is used to generate a set of maintenance scenarios that is a multi-objective combinatorial optimization problem related to the lifetime performance and the life-cycle cost as separate objective functions. A computer program, which generates optimal maintenance scenarios, was developed based on the proposed method using the life-cycle costs and the performance of bridges. The subordinate relation between bridge members has been considered to decide optimal maintenance sequence and a corresponding algorithm has been implemented into the program. The developed program has been used to present a procedure for finding an optimal maintenance scenario for steel-girder bridges on the Korean National Road. Through this bridge maintenance scenario analysis, it is expected that the developed method and program can be effectively used to allow bridge managers an optimal maintenance strategy satisfying various constraints and requirements.

      • Is Low-Lying Optic Chiasm an Obstacle to an Endoscopic Endonasal Approach for Retrochiasmatic Craniopharyngiomas? (Korean Society of Endoscopic Neurosurgery -003)

        <P><B>Objective</B></P> <P>Despite advances in endoscopic techniques, retrochiasmatic craniopharyngiomas (CPs) are difficult to remove completely, because the low-lying optic chiasm often provides an obstacle to an endoscopic endonasal approach. This study aimed to identify the endoscopic surgical outcomes of the retrochiasmatic CP and resolve the issues related to low-lying optic chiasm.</P> <P><B>Methods</B></P> <P>We reviewed 154 consecutive patients with CP who underwent endonasal endoscopic resection from February 2009 to April 2017 at 2 independent institutions. The topographic relationship of the tumor with the third ventricle, stalk, and optic chiasm and clinical outcomes were investigated.</P> <P><B>Results</B></P> <P>Retrochiasmatic CPs were found in 142 of 154 patients (92.2%). The median follow-up time was 25 months. Gross total resection and near-total resection were achieved in 113 patients (79.6%) and 21 patients (13.8%), respectively. Postoperative cerebrospinal fluid leaks were found in 16 patients (11.3%). Low-lying and high-lying chiasms were found in 44 patients (31.0%) and 98 patients (69.0%), respectively. Low-lying chiasm did not affect clinical outcomes including the extent of resection. Patients with low-lying chiasm showed a marginal trend for postoperative visual deterioration. The ventricular growth pattern representing the origin of the tumor and previous surgery were significantly associated with the position of the optic chiasm (<I>P</I> = 0.007 and 0.001, respectively).</P> <P><B>Conclusions</B></P> <P>An endoscopic endonasal approach is an effective surgical approach for retrochiasmatic CP, even in tumors with low-lying chiasm. However, a thorough and careful dissection is necessary to prevent visual deterioration.</P> <P><B>Highlights</B></P> <P> <UL> <LI> Low-lying chiasm can interfere with endoscopic approach for craniopharyngioma. </LI> <LI> The position of optic chiasm is affected by tumor origin previous surgery. </LI> <LI> Low-lying chiasm does not alter clinical outcomes including extent of resection. </LI> <LI> Endoscopic endonasal approach is feasible even in the presence of low-lying chiasm. </LI> </UL> </P>
