        뇌수술 시 과호흡과 만니톨의 투여가 뇌혈류와 뇌산소대사에 미치는 영향

        Background: There are therapies to lower intracranial pressure (ICP) including head elevation, hyperventilation, diuretics injection, intravenous mannitol, hypothermia, cerebrospinal fluid drainage, and cerebral resection in neurosurgical patients. However in recent reports, hyperventilation followed by mannitol administration may lead to cerebral ischemia. Therefore, we investigated the effect of 0.5-1.0 g/kg mannitol administration on jugular venous oxygen saturation (SjVO_2) and cerebral arterial-jugular venous oxygen content difference (AVDO_2) at PaCO_2 25-30 mmHg and 35-40 mmHg in patients undergoing neurosurgery. Methods: we studied 17 patients undergoing neurosurgery in the Ajou University Hospital. Anesthesia was induced with fentanyl, midazolam, thiopental, and vecuronium, and maintained with )_2-Air-Isoflorane, a continuous infusion of fentanyl, and vecuronium. Patients were divided into two groups. Group 1 (n = 10) which is PaCO_2 25-30 mmHg and Group 2 (n = 7) which is PaCO_2 35-40 mmHg by controlling ventilator. Measurements of SjVO_2 and AVDO_2 in following time intervals: Ⅰ = preinjection of mannitol, Ⅱ = postinjection 20 minutes of mannitol, Ⅲ = postinjection 40 minutes of mannitol were obtainde for each group. 0.5-1.0 g/kg mannitol was administered intravenously just at duramater opening. Results: Hemodynamics and hematologics were not significantly different among the two groups. SjVO_2 of each group are as follows; Group 1; Ⅰ (70.3 ± 8.1%), Ⅱ (66.3 ± 6.9%), Ⅲ (69.1 ± 7.9%) and Group 2; Ⅰ (78.6 ± 7.4%), Ⅱ (75.1 ± 8.1%), Ⅲ (76.0 ± 11.2%), Hyperventilation significantly decreased SjVO_2- AVDO_2 was not significantly different but SjVO_2 in Ⅱ was significantly decreased compared with Ⅰ and Ⅲ in Group 1 (20% patients). Conclusions: Mannitol produced a change of SjVO_2 during hyterventilation. Therefore, intravenous mannitol during hyperventilation should be given cautiously according to the patients status because it may cause cerebral ischemia in critical patients.         Abstract - in this study, Polymide(PI) Thin-films of MIS structure and MIM structure is deposited on Si wafer and AI Thin-films based on spincoating method. Using material is SE5291(PAA) of NISSAN CHEMICAL. Spincoating velocity is 4000[rpm] for 5[min] The uniform thin films of PI formed by curing at 150[℃] for 30[min](Soft baking) and 350[℃] for 60[min](Hard baking). Thickness of PI Thin-films is 150-170 [nm]. For Electrical characteristics of PI Thin-films measured Ⅰ-Ⅴ properties, C-V properties and f-C properties. The result analysis is like below: Ⅰ-Ⅴ characteristic using HP 4156A, C-V is HP 4280A f-C is HP 4294A

        정신분열병 환자의 인터류킨-2 생산능

        급성기 및 관해상태의 정신분열병 환자와 정상 대조군의 IL-2 생산능을 비교함으로써, 정신분열병의 여러 발병원인 중 자가면역 병인론의 증거인 세포성 면역방응의 저하 및 질병상태와 관련한 세포성 면역기능의 변화를 일부 한국인 정신분열병 환자에서 조사에서 조사하고자 하였다. 방 법 : DSM-IV의 정신분열병 진단기준에 적합한 급성기 환자 19명(남 : 10, 여 : 9)과 관해상태의 환자 21명(남 :12, 여: 9) 및 정상대조군 40명(남 : 20, 여 : 20)을 연구대상으로 하였다. 피험자로부터 채혈한 혈액에서 임파구를 분리하여 phytohemagglutinin으로 T임파구를 자극하고 세포배양하여 증식시킨후 IL-2immunoassay kit을 이용하여 T임파구의 IL-2 생산능을 측정하였다. 결 과 : IL-2 생산능은 성별에 관계없이 정신분열병 환자군에서 정상 대조군에 비하여 유의하게 감소하였다. (t=5. 97, p<0.001). 질병상태에 따라 급성기 정신분열병 환자군 및 관해상태 정신분열병 환자군으로 구분하여 정상대조군의 IL-2 생산능과 비교한 결과, IL-2 생산능이 정상 대조군에서 가장 높았고 다음은 관해상태 환자군이었으며 급성기 환자군에서 가장 낮았다. 세 군 사이에는 모두 유의한 차이가 있었다. (F=35.35, p<0.001). 결 론: 일부 한국인 정신분열병 환자에서 질병상태와 유의하게 관련된 세포성 면역기능의 저하가 관찰되며, 이는 정신분열병의 다양한 원인중 자가면역 기전에 의한 발병을 시사한다. Objects : This study was designed to examine the decreased cellular immune response and the change in cellular immunity related to to clinical status in Korean schizophrenic patients. Methods : The subjects were 19 acute schizophrenics and 21 remitted schizophrenics who met the DSM-IV criteria of schizophrenia and 40 healthy volunteers as the normal control. After the lymphocyte was separated from blood which had been drawn from the subjects and then T lymphocyte was stimulated by PHA and proliferated by cell culture, the examiner measured IL-2 productivity by IL-2 immunoassay kit. Results : IL-2 productivity of the schizophrenics was significantly lower than that of the normal controls. There was no significant difference between male and female in two groups. In the comparison by clinical status, acute schizophrenic group was the lowest, remitted schizophrenic group was next, and normal control group was the highest in the IL-2 productivity, with statistically significant difference among the groups(p<0.001). Conclusion : We suggest that there is a decrease in the cellular immune response which is significantly related to clinical status in some Korean schizophrenics. This finding supports that onset of schizophrenia may be the result of autoimmune mechanism, which is one of the various etiologic factors in schizophrenia.

        Susceptibility of Oral Bacteria to Essential Oil of Artemisia capillaris Thunb.

        Objective : The aim of this work is to investigate the antibacterial activity of the essential oil obtained from Artemisia capillaris (A. capillaris), as the development of microbial resistance to antibiotics make it necessary to constantly look for new and active compounds effective against pathogenic bacteria. Methods : The crushed materials of A. capillaris (1 kg) were subjected to steam distillation for 3 h, using a modified Clevenger type apparatus in order to obtain essential oil. Diethyl ether was the extracting solvent kept at 25°.... The essential oil was analyzed by gas chromatography (GC) and gas chromatography/mass spectrometry (GC/MS). The essential oil and the composition were tested for antimicrobial activities against 15 different genera of oral bacteria. Results and Conclusion : The components of the essential oil identified were: β-pinene (9.36%), camphor (3.32%), 1,8­cineole (4.38%), artemisia alcohol (3.32%), β-caryophyllene (11.08%), γ-cadinene (4.23%), and capillene (32.74%). The essential oil of A. capillaris exhibited considerable inhibitory effects against all oral bacteria tested, while their major components demonstrated various degrees of growth inhibition.

      • Serum bilirubin levels are positively associated with glycemic variability in women with type 2 diabetes

        <P><B>Abstract</B></P><P><B>Aims/Introduction</B></P><P>Glycemic variability is known to induce oxidative stress. We investigated the relationships between glycemic variability and serum bilirubin levels, an endogenous anti‐oxidant, in patients with diabetes.</P><P><B>Materials and Methods</B></P><P>A cross‐sectional study was carried out with 77 patients with type 2 diabetes who had been recruited to two clinical studies from 2008 to 2014. There were no participants with diseases of the pancreas, liver, biliary tract and chronic renal insufficiency. Glycemic variation was calculated by a continuous glucose monitoring system, and correlation analyses were carried out to evaluate their association with bilirubin levels. Multiple linear regression was carried out to identify independent factors influencing bilirubin levels and glycemic variation.</P><P><B>Results</B></P><P>Among the participants, 42.3% were men. The mean (standard deviation) age was 61.5 years (10.4 years), body mass index was 24.2 kg/m<SUP>2</SUP> (2.8 kg/m<SUP>2</SUP>), diabetes duration was 17.7 years (9.5 years), hemoglobin A<SUB>1c</SUB> was 60.7 mmol/mol (7.1 mmol/mol; 7.7 [0.7]%) and bilirubin was 11.8 μmol/L (4.10 μmol/L). Serum bilirubin levels were not different according to age, body mass index and hemoglobin A<SUB>1c</SUB>. However, the mean amplitude of glucose excursion was positively associated with bilirubin levels in women (<I>r</I> = 0.588, <I>P</I> < 0.001). After adjustment with duration of diabetes, serum albumin, liver enzymes, and mean glucose, the correlation between bilirubin and mean amplitude of glucose excursion remained significant (<I>r</I> = 0.566, <I>P</I> < 0.001). Multiple linear regression analyses showed that bilirubin was an independent determinant for the mean amplitude of glucose excursion in women. 1,5‐Anhydroglucitol was also associated with bilirubin levels in women.</P><P><B>Conclusions</B></P><P>Bilirubin level within the physiological range might be an independent predictor for glycemic variability in women with type 2 diabetes.</P>

        Rhythmic Serotonin N-Acetyltransferase mRNA Degradation Is Essential for the Maintenance of Its Circadian Oscillation

        <B>ABSTRACT</B><P>Serotonin <I>N</I>-acetyltransferase (arylalkylamine N-acetyltransferase [AANAT]) is the key enzyme in melatonin synthesis regulated by circadian rhythm. To date, our understanding of the oscillatory mechanism of melatonin has been limited to autoregulatory transcriptional and posttranslational regulations of AANAT mRNA. In this study, we identify three proteins from pineal glands that associate with <I>cis</I>-acting elements within species-specific AANAT 3′ untranslated regions to mediate mRNA degradation. These proteins include heterogeneous nuclear ribonucleoprotein R (hnRNP R), hnRNP Q, and hnRNP L. Their RNA-destabilizing function was determined by RNA interference and overexpression approaches. Expression patterns of these factors in pineal glands display robust circadian rhythm. The enhanced levels detected after midnight correlate with an abrupt decline in AANAT mRNA level. A mathematical model for the AANAT mRNA profile and its experimental evidence with rat pinealocytes indicates that rhythmic AANAT mRNA degradation mediated by hnRNP R, hnRNP Q, and hnRNP L is a key process in the regulation of its circadian oscillation.</P>

        Mitochondrial Complexes I and II Are More Susceptible to Autophagy Deficiency in Mouse β-Cells

        <P><B>Background</B></P><P>Damaged mitochondria are removed by autophagy. Therefore, impairment of autophagy induces the accumulation of damaged mitochondria and mitochondrial dysfunction in most mammalian cells. Here, we investigated mitochondrial function and the expression of mitochondrial complexes in autophagy-related 7 (<I>Atg7</I>)-deficient β-cells.</P><P><B>Methods</B></P><P>To evaluate the effect of autophagy deficiency on mitochondrial function in pancreatic β-cells, we isolated islets from <I>Atg7</I><SUP>F/F</SUP>:RIP-<I>Cre</I>+ mice and wild-type littermates. Oxygen consumption rate and intracellular adenosine 5'-triphosphate (ATP) content were measured. The expression of mitochondrial complex genes in <I>Atg7</I>-deficient islets and in β-TC6 cells transfected with si<I>Atg7</I> was measured by quantitative real-time polymerase chain reaction.</P><P><B>Results</B></P><P>Baseline oxygen consumption rate of <I>Atg7</I>-deficient islets was significantly lower than that of control islets (<I>P</I><0.05). Intracellular ATP content of <I>Atg7</I>-deficient islets during glucose stimulation was also significantly lower than that of control islets (<I>P</I><0.05). By Oxygraph-2k analysis, mitochondrial respiration in <I>Atg7</I>-deficient islets was significantly decreased overall, although state 3 respiration and responses to antimycin A were unaffected. The mRNA levels of mitochondrial complexes I, II, III, and V in <I>Atg7</I>-deficient islets were significantly lower than in control islets (<I>P</I><0.05). Down-regulation of <I>Atg7</I> in β-TC6 cells also reduced the expression of complexes I and II, with marginal significance (<I>P</I><0.1).</P><P><B>Conclusion</B></P><P>Impairment of autophagy in pancreatic β-cells suppressed the expression of some mitochondrial respiratory complexes, and may contribute to mitochondrial dysfunction. Among the complexes, I and II seem to be most vulnerable to autophagy deficiency.</P>

      • 후륜 서스펜션 부싱고무에 대한 정/동특성 해석 및 차량 성능 특성 평가

        Static and dynamic analysis was performed for the design of TCB which is used in rear suspension system of light-weight vehicle. Engineering sample was made and tested for static stiffness estimation. Guide-line of static FEM analysis was prepared by comparing test data and analysis result and other static stiffness of untested direction was measured. Dynamic analysis based on viscous-elasticity theory and viscoelastic test datawas done. The relationship between static-stiffness and dynamic-stiffness was examined. System analysis with rear suspension system of test vehicle found the difference in stiffness and compliance steer performance between conventional bush and TCB and the design objective of TCB was established.
