      • A case of cowden syndrome with multi-systemic tumors and thyroid cancer

        ( Jongeun Lee ),( Hyun Jeong Byun ),( Chan Seong Park ),( Youngkyoung Lim ),( Ji-hye Park ),( Jong Hee Lee ),( Dong-youn Lee ),( Joo-heung Lee ),( Jun-mo Yang ) 대한피부과학회 2019 대한피부과학회 학술발표대회집 Vol.71 No.1

Cowden syndrome is caused by mutation in phosphatase and tensin homolog (PTEN) gene, belonging to PTEN hamartoma tumor syndrome. A 21-year-old female presented with small papules on palms and neck. In addition, there were solitary papules under the nose and the upper lip and skin tags on both axilla. Her past medical history included intramuscular arteriovenous malformation versus hemangioma on right lower leg, cyst in the right ovary, multiple follicles in the left ovary, fibroadenoma in the left breast, and phyllodes tumor and intraductal papilloma in the right breast. Moreover, additional mass in thyroid turned out to be a papillary thyroid carcinoma. The mutation analysis for PTEN hamartoma syndrome confirmed pathogenic variant in the PTEN gene. A 3mm-punch biopsy on the left palm revealed regular orthokeratosis and hyperkeratosis consistent with palmoplantar keratosis. During the follow up visits, the patient developed hematochezia due to numerous hyperplastic and lymphoid polyps in stomach through rectum. Cowden syndrome possesses increased risk of developing malignancies including breast, thyroid, and colorectal cancers. Therefore it is important for dermatologists to recognize symptoms of the possible multi-systemic tumors in these patients. Herein, we present a case of Cowden syndrome with multi-systemic tumors and thyroid cancer.

      • A case of follicular mucinosis in a child at unusual location

        ( Jongeun Lee ),( Hyun Jeong Byun ),( Chan Seong Park ),( Youngkyoung Lim ),( Ji-hye Park ),( Jong Hee Lee ),( Dong-youn Lee ),( Joo-heung Lee ),( Jun-mo Yang ) 대한피부과학회 2019 대한피부과학회 학술발표대회집 Vol.71 No.1

Follicular mucinosis (FM) is usually presented as welldemarcated erythematous or skin-colored patches with follicular papules at head or neck. It is a rare entity in children. A healthy 4-year-old boy presented with a 1-month history of an erythematous patch at left dorsum of nose. The patch was rather uneven because of follicular papules and firm on palpation. The pathologic findings showed deposition of blue amorphous material in sebaceous gland with lymphocytic infiltration, suggesting inflammation of sebaceous gland without histopathologic features of mycosis fungoides such as epidermotropism of lymphocytes or lymphoid atypia. The lesion was completely healed after 6 weeks of topical steroid (0.25% prednicarbate). Although it has been described to follow benign courses in children, close follow up is warranted as it is difficult to distinguish idiopathic FM and lymphoma-associated FM. In English literature published so far, head accounted for around 80% in the solitary FM cases. Forehead and chin were the most frequently involved location while FM involving nose was reported only twice, each in multiple and solitary cases. Herein, we report a case of FM in a child at nose treated with topical steroid.

      • Peripheral Nerve Blocks for Lower Extremity Surgery: A Prospective Observational Feasibility Study

( Chunwoo Yang ),( Choon Soo Lee ),( Kyungchul Song ),( Jongeun Kim ),( Wonju Na ),( Hyunkeun Lim ) 경희대학교 경희의료원 2015 慶熙醫學 Vol.30 No.1

Background: Peripheral nerve blocks are clinically useful regional anesthetic technique. Although they are less commonly used, they provide a lot of benefits for lower extremity surgery. The aim of this prospective study was to assess the feasibility and side effects of peripheral nerve blocks for lower extremity surgery. Methods: The sixty patients undergoing lower extremity surgery were prospectively included. A sciatic nerve block was performed according to the classic posterior approach. A femoral nerve block was performed with the injection of 1 cm lateral to the femoral artery at skin-crease level. Stimulating needles of 120 mm for the sciatic nerve and 50 mm for the femoral nerve were used with a nerve stimulator. Block procedure time, patient discomfort, success rate, block duration, adverse events, and patient satisfaction were assessed. Results: The sixty patients were enrolled (40 men and 20 women) with the following characteristics: mean (standard deviation) of age was 52 (15) yr, height was 164 (8) cm, and weight was 65 (11) kg. The success rate was 85% (95% confidence interval 73.9 . 91.9%) and 15% of the patients required supplementation. Local anesthetic systemic toxicity was observed in three patients (0.05%), and no neurological deficits occurred. All patients were satisfied with this technique. Conclusions: This study showed that peripheral nerve blocks provide effective anesthesia and analgesia with a small complication rate and a high degree of patient acceptance for lower extremity surgery.

Short Seller, Firm, and Auditor Behavior

        춘정은(Jongeun Chun),양전선(Joonsun Yang) People&Global Business Association 2021 Global Business and Finance Review Vol.26 No.3

Purpose: Although a small part of arbitrage trading, short selling has a significant impact on capital markets. This study empirically examines relationships among short selling, earnings managements, and auditor behaviors. Design/methodology/approach: We employed short selling and abnormal short selling as independent variables. Audit hours, audit fees, and (absolute) value of discretionary accruals as dependent variables. In this study, we basically include ordinary least-squares regression analyses using panel data and interaction analyses. Findings: This study verifies the relationship among short selling, firm earnings management, and auditor behavior, showing positive relationships between short selling and current and future audit fees, audit hours, and current audit quality. Further, higher audit risk, caused by the interaction between abnormal short selling and aggressive earnings management, is reflected in future audit fees and hours. Research limitations/implications: Due to the financial data provided on an annual basis, the daily short selling data was aggregated on an annual basis. This study suggests that short selling is related to firm earnings management behavior, as well as to the auditor behaviors. Therefore, regulators should consider the impacts of short selling on company and auditors as well as investors. Originality/value: This study provides evidence that the positive relationship between short selling and audit fee is not only caused by risk-premium channels, but also by audit-effort channels. Furthermore, we find that the interaction effects of short selling and earnings managements occur only in abnormal short selling. This seemed to impose penalties on short selling exceeding forecasts when auditors considered earnings managements and short selling at the same time.

Effect of Motor Imagery Training on Somatosensory Evoked Potentials and Upper Limb Function in Stroke Patients

Jongbae Choi,Jongeun Yang 국제물리치료연구학회 2020 Journal of International Academy of Physical Ther Vol.11 No.1

Background: Motor imagery is the mental representation of an action without overt movement or muscle activation. However, few previous studies have demonstrated motor imagery training effects as an objective assessment tool in patients with early stroke. Objective: To investigate the effect of motor imagery training on Somatosensory Evoked Potentials (SSEP) and upper limb function of stroke patients. Design: A quasi-experimental study. Methods: Twenty-four patients with stroke were enrolled in this study. All subjects were assigned to the experimental or control group. All participants received traditional occupational therapy for 30 minutes, 5 times a week. The experimental group performed an additional task of motor imagery training (MIT) 20 minutes per day, 5 days a week, for 4 weeks. Both groups were assessed using the SSEP amplitude, Fugl-Meyer assessment of upper extremity (FMA UE) and Wolf motor function test. Results: After the intervention, the experimental group showed significant improvement in SSEP amplitude and FMA UE than did the control group. Conclusion: These findings suggest that the MIT effectively improve the SSEP and upper limb function of stroke patients.

      • A case of yellow nail syndrome with myocardial infarction and hypothyroidism

        ( Youngkyoung Lim ),( Jongeun Lee ),( Hyun Jeong Byun ),( Chan Seong Park ),( Jae Ho Lee ),( Ji-hye Park ),( Jong Hee Lee ),( Dong-youn Lee ),( Joo-heung Lee ),( Jun-mo Yang ) 대한피부과학회 2018 대한피부과학회 학술발표대회집 Vol.70 No.1

Yellow Nail Syndrome (YNS) is a rare disorder that almost always occurs after 50 years of age but a juvenile or familial form has also been observed. It is chracterized by yellow nail discoloration, pulmonary manifestations, and lower limb lymphedema. Nail manifestation includes slow growth, thickening, hardening, transversal ridging, onycholysis, onychorrhexis, and paronychia. Pulmonary manifestation shows bronchitis, bronchiectasis, pneumonia, and pleural effusions. Chronic sinusitis is frequently associated with the triad of YNS. A 61-year-old male visited our clinic with nail dystrophy which started 18 months ago. Nail changes were observed including nail thickening, yellow color change, and transverse ridging. Result of potassium hydroxide (KOH) preparation, and fungus culture were negative. Periodic acid-Schiff (PAS) staining of nail clipping specimen revealed no fungal hyphae. There had been history of bronchiectasia with bronchiolitis, pneumonia, and chronic sinusitis. The patient had also experienced myocardial infarction (MI) and hypothyroidism. Herein, we present a very rare case of YNS with both MI and hypothyroidism.

      • Efficacy of low level laser therapy(LLLT) and evaluation for the risk factors of radiodermatitis in patients with lung or head & neck cancer : A single institution explorative study

        ( Hyun Jeong Byun ),( Jongeun Lee ),( Chan Seong Park ),( Youngkyoung Lim ),( Ji-hye Park ),( Jong-hee Lee ),( Dong-youn Lee ),( Joo-heung Lee ),( Jun-mo Yang ),( Dongryul Oh ) 대한피부과학회 2018 대한피부과학회 학술발표대회집 Vol.70 No.2

Background: Head and neck cancers have historically been treated with radiotherapy. However, side effects on the skin such as erythema, desquamation, erosion are common and referred to as radiodermatitis. Recent study on radiodermatitis of breast cancer patients revealed the effectiveness of low-level laser therapy (LLLT). Objectives: The primary endpoint is to evaluate the clinical efficacy of LLLT in patients receiving high dose radiation therapy. The secondary endpoint is to evaluate the risk factors of radiodermatitis Methods: Patients with lung or head and neck cancer who were planning to receive high dose radiation therapy were included in the study. The subjects received LLLT three times a week for 6 weeks. The degree of radiodermatitis was evaluated using the CTCAE. The subjects were divided into two groups according to the maximum grade, and analyzed for risk factors for radiodermatitis. Results: Total 33 subjects were analyzed. One week after LLLT was completed, the proportion of CTCAE grade greater than 2 was 42.4%, and greater than 3 was 9.1%. No clinically significant side effect has occurred. Age, BMI, site of treatment, total dose, duration, concurrent chemotherapy, scar line, smoking status were evaluated as risk factors. The odds ratio for each factor did not seem to increase the risk of radiodermatitis. Conclusion: LLLT could have effects on radiodermatitis of the neck and supraclavicular area, especially for grade greater than 3.

      • A study of primary cancers developed in patients with cutaneous malignant melanoma: a single institution retrospective study

        ( Hyun Jeong Byun ),( Jongeun Lee ),( Chan Seong Park ),( Youngkyoung Lim ),( Ji-hye Park ),( Jong-hee Lee ),( Dong-youn Lee ),( Joo-heung Lee ),( Jun-mo Yang ) 대한피부과학회 2018 대한피부과학회 학술발표대회집 Vol.70 No.2

Background: Considering hypothesized genetic and environmental risk factors for melanoma, research on the relevance of other primary cancers is necessary. It has been described that Caucasians with melanoma have a risk of developing secondary primary cancer in several studies. However, studies of Asians are few. Objectives: The objectives were to assess the incidence of primary cancers according to the site and the stage of melanoma, and compare mortality rates between patients with primary cancers and those without. Methods: Patients were divided according to the primary skin site, and stages of melanoma. The risk of developing primary cancer was estimated by calculating standardized incidence ratio(SIR). Results: Among 924 patients diagnosed as cutaneous malignant melanoma, 71(7.68%) patients developed 75 primary cancers. Thyroid cancer accounted for 21.33%, followed by stomach cancer, breast cancer, and LGI cancer. Age and sex adjusted SIR for primary cancers was 6.02(95% CI 4.70-7.59). The prevalence of primary cancer was not significantly different among the primary sites of melanoma, and it was significantly higher in regional metastasis group compared to patients with distant metastasis. The mortality risk was significantly lower in patients with other primary cancers than in those without primary cancers. Conclusion: Further studies are needed for more reliable results. Also, screening for cancers according to the schedule should be strongly recommended for additional cancer incidence.
