      • Metabolic Tumor Volume Measured by Preoperative 18F-FDG-PET/CT Predicts for Recurrence in Endometrial Cancer

        Metabolic tumor volume (MTV) and total lesion glycolysis (TLG) are functional tumor parameters which determine the metabolic activity of tumors with increased 18 fluorodeoxyglucose (18F-FDG) uptake on positron emission tomography/computed tomography(PET/CT). The goal of this study was to evaluate whether these parameters have prognostic value in patients with endometrial cancer. Between 2004 and 2009, 84 patients with endometrial cancer who had undergone preoperative 18F-FDG PET/CT were included in this study. We measured the SUVmax, SUVavg, MTV, and TLG of the primary tumor. Patients` clinicopathologic characteristics and follow-up information were collected from hospital charts and clinic records retrospectively. Cox proportional hazards analysis and Kaplan-Meier method were used to identify the predictors for recurrence. Receiver operating curve (ROC) analysis was used to determine the cutoff value of continuous variables for predicting recurrence. The median age was 51 (range 24-76) years. The median follow-up period was 49 (range 3-85) months. International Federation of Gynecology and Obstetrics (FIGO) stage included: I (58), II (11), III (13), and IV (2). Histology included: endometrioid (63), papillary serous (10), carcinosarcoma (9), clear cell (1), and adenocarcinoma (1). There were 12 cases of recurrence and 9 cases of disease-specific death. The median progression free survival (PFS) duration was 48 (1-85) months. In univariate analysis, factors predicting for recurrence were myometrial invasion (p=0.012), lymphovascular space invasion (p=0.029), lymph node metastasis (p<0.001), non-endometrioid histology (p=0.002), advanced FIGO stage (p<0.001), MTV (p=0.001), and TLG (p=0.052) while SUVmax and SUVavg were not. However, multivariate analysis showed that only lymph node metastasis (p=0.001 , hazard ratio [HR] 9.286, 95% confidence interval [CI] 2.614-32.986), and MTV (p=0.009 , HR 1.007, 95% CI 1.002-1.013) remained as independent risk factors for recurrence. The area under the ROC plot for discriminating recurrence of MTV using the cutoff value of 17.15 was 0.679. The Kaplan-Meier survival graph showed that patients with a high MTV (≥17.15) had a significantly lower PFS rate than those with a low MTV (<17.15; p=0.034, log-rank test). MTV measured by preoperative 18F-FDG PET/CT was a significant independent prognostic factor predicting for recurrence in patients with endometrial cancer. This functional tumor parameter may be considered to plan further treatment and follow-up after operation.

      • Comparison of single-port versus three-port laparoscopic myomectomy: a retrospective matched cohort study

        The aim of this study was to compare the surgical outcomes between single-port versus three-port laparoscopic myomectomy. This was a retrospective matched cohort study. We included patients who underwent single-port or three-port laparoscopic myomectomy between 2009 and 2011. We matched all cases of single-port laparoscopic myomectomy (SLM) 1:2 with three-port laparoscopic myomectomy (TLM) based on age, body mass index, diameter of largest myoma, and number of removed myoma. Clinicopathologic data of these patients were reviewed by medical records. During study period, 20 cases of SLM were performed. There were no statistically significant differences between the SLM and TLM groups in age, body mass index, diameter of largest myoma, and number of removed myoma. Mean age was 33.6±7.9 years versus 34.1±8.1 years, mean body mass index was 22.6±3.9 kg/m2 versus 22.9±4.3 kg/m2, and mean myoma diameter was 6.54±2.1 cm versus 7.08±2.49 cm, respectively. There were no differences in operating time (101.1±29.1 min vs. 105.8±25.3 min, p=0.545), estimated blood loss (202.8±161.3 mL vs. 195.2±161.3 mL, p=0.865), postoperative drop of hemoglobin level (2.1±1.6 mg/dL vs. 2.0±1.4 mg/dL, p=0.614), and transfusion requirement (5.56% vs. 6% p=0.596). However, single-port group had significantly shorter postoperative hospital stay (2.11±0.32 vs. 2.48±0.68, p<0.05), lower postoperative pain scores at POD 0 (3.9±0.9 vs. 4.5±0.9, p<0.05) and less postoperative opioids requirement (24.8% vs. 50.0%, p<0.05). Operation related complications occurred in 1 (5%) and 2 (4%) patients of single-port and three-port laparoscopic myomectomy group, respectively (p=0.371). This is the first matched cohort study to compare the surgical outcomes between single-port versus three-port laparoscopic myomectomy. Single-port laparoscopic myomectomy appeared to be as feasible and safe as three-port laparoscopic myomectomy. It was more minimally invasive surgery with more favorable operative outcomes in terms of postoperative pain, analgesics requirement and cosmetic outcome.

        Ecological Characteristics of Natural Habits of Deutzia paniculata, a Rare and Endemic Woody Species in Korea

        Deutzia paniculata Nakai, a rare and endemic plant, has limited distribution throughout the North and South Gyeongsang provinces of South Korea. The D. paniculata community grows mostly on the stony slopes of forests, valley edges, and rock layers at 250-960 m in altitude, where deciduous trees are dominant and high humidity is maintained. Correlation analysis of vegetation and environmental factors found that the Walter's dogwood-mulberry community was correlated with soil acidity (pH). Whilst the queritron community had correlations with distance from the valley, rock rate and slope. The natural habitat of the Palgongsan Mountain in Daegu is known to have high genetic diversity, had eight D. paniculata individuals recorded from 2014 to 2018, and 12 individuals recorded in 2020 (new individuals due to a newly created space within the herbaceous layer caused by grass mowing works), it is therefore unlikely that the community would perish unless there was an artificial disturbance. To conserve the natural habitats of D. paniculata, oppression by Sasa borealis, damage, increase in crown density of the upper layer, overexploitation, and absence of seedlings should be carefully investigated. In addition, response measures should also be established and the impact on seed fullness and the reproductive characteristics of D. paniculata recorded. To restore declined genetic diversity, individuals from high genetic diversity regions, such as Palgongsan Mountain, should be artificially transplanted.

        Objective Concurrent chemoradiotherapy is usually administered to patients with locally advanced cervical cancer (LACC). Extended-field chemoradiotherapy is required if para-aortic lymph node (PALN) metastasis is detected. This study aimed to construct a predictionmodel for PALNmetastasis in patients with LACC before definitive treatment. Methods Between 2009 and 2016, all consecutive patients with LACC who underwent para-aortic lymphadenectomy at two tertiary centers were retrospectively analyzed. A multivariate logistic model was constructed, from which a prediction model for PALN metastasis was developed and internally validated. Before analysis, risk grouping was predefined based on the likelihood ratio. Results In total, 245 patients satisfied the eligibility criteria. Thirty-four patients (13.9%) had pathologically proven PALN metastases. Additionally, 16/222 (7.2%) patientswith negative PALNs on positron emission tomography/computed tomography (PET/CT) had PALNmetastasis. Moreover, 11/105 (10.5%) patients with both negative PALNs and positive pelvic lymph nodes on PET/CT had PALN metastasis. Tumor size on magnetic resonance imaging and PALN status on PET/CT were independent predictors of PALN metastasis. The model incorporating these two predictors displayed good discrimination and calibration (bootstrap-corrected concordance index = 0.886; 95% confidence interval = 0.825-0.947). The model categorized 169 (69%), 52 (22%), and 23 (9%) patients into low-, intermediate-, and high-risk groups, respectively. The predicted probabilities of PALN metastasis for these groups were 2.9, 20.8, and 76.2%, respectively. Conclusion We constructed a robust model predicting PALN metastasis in patients with LACC that may improve clinical trial design and help clinicians determine whether nodal-staging surgery should be performed.

        A prospective multicenter clinical study on the efficiency of detachable ball- and spring-retained implant prosthesis

        PURPOSE. This prospective clinical study was conducted to evaluate the clinical usefulness of the freely detachable zirconia ball- and spring-retained implant prosthesis (BSRP) through a comparative analysis of screw- and cement-retained implant prosthesis (SCRP). MATERIALS AND METHODS. A multi-center, randomized, prospective clinical study evaluating the clinical usefulness of the detachable zirconia ball- and spring-retained implant prostheses was conducted. Sixty-four implant prostheses in 64 patients were examined. Periodic observational studies were conducted at 0, 3, 6, and 12 months after delivery of the implant prosthesis. Factors such as implant success rate, marginal bone resorption, periodontal pocket depth, plaque and bleeding index, and prosthetic complications were evaluated, respectively. RESULTS. During the 1-year observation period, all implants survived without functional problems and clinical mobility, showing a 100% implant success rate. Marginal bone resorption was significantly higher in the SCRP group than in the BSRP group only at the time of implant prosthesis delivery (P = .043). In all observation periods, periodontal pocket depth was slightly higher in the BSRP group than in the SCRP group, but there was no significant difference (P > .05). The modified plaque index (mPI) scores of both groups were moderate. Higher ratio of a score 2 in modified sulcus bleeding index (mBI) was observed in the BSRP group in the 6- and 12-months observation. CONCLUSION. Within the limitations of this study, the newly developed zirconia ball- and spring-retained implant prosthesis could be considered as an applicable and predictable treatment method along with the existing screw- and cement-retained prosthesis.

        노화 생쥐의 복부지방에서 수영운동에 의한 AMPK와 GLUT4 단백질 변화

        본 연구의 목적은 노화된 생쥐에 있어서 수영운동에 따른 AMPK와 GLUT4의 발현을 알아보는 것이다. 이에 본 실험에서는 노화된 생쥐에게 수영운동을 시행하였으며, 몸무게 및 복부지방의 무게를 측정하였고, 지방세포의 크기변화를 Oil-Red 염색법으로, 지방조직에서 AMPK와 Glut-4의 단백질을 분석하였다. 연구대상은 2개월 SWV계 숫컷 생쥐 10마리를 청년대조군(n=10), 12개월 노화된 SWV계 숫컷 생쥐 20마리를 노화대조군과 노화운동군(n=10)의 3군으로 분류하였고, 유산소운동은 주당 5회 40분씩 시행하였으며, 주 1회 체중을 측정하였다. 체중의 변화는 노화대조군이 다른 군에 비해 실험 종결 때까지 높은 체중을 유지하였으나, 노화운동군에서는 수영운동 2주후부터 7주까지 지속적으로 체중이 줄어(p<.05, 001)들었다. 혈당변화는 노화운동군이 다른 두 군에 비해 2주부터 뚜렷한 감소(p<.05)를 보였으며 실험종결 때까지 낮은 혈당을 유지하였다. 8주간 유산소운동 후 복부지방량의 변화는 노화운동군이 다른 두 군에 비해 확실하게 줄어들었고(p<0.001), 노화대조군의 지방세포크기는 비대해져 있었으나, 노화운동군에서는 작은 지방세포의 크기를 보여 주었다. 특히 AMPK와 GLUT-4의 발현은 노화운동군에서 다른 두 군에 비해 뚜렷하게 증가되었다. 이상의 결과를 종합하면 8주간의 수영운동은 노화된 생쥐의 체중 및 복부지방을 감소시켜 지방비대의 억제효과를 나타내었고, AMPK의 단백질발현증가는 지방생성 증가를 억제함을 알 수 있었으며, GLUT-4의 단백질발현증가는 혈당의 수치를 조절하는 효과가 있었다. 따라서 장기간의 수영운동은 노화로 인한 대사의 기능을 향상시켜 대사성질환으로 부터 보호할 수 있을 것으로 생각된다. The present study was designed to examine the protein changes of AMPK and GLUT-4 by swim exercise in fat tissue of aging mice. We performed aerobics exercise by swimming, measured body weight(BW)and measured blood glucose(BG), and performed Oil-Red stain for adipocyte and western blot for AMPK and GLUT-4 protein in fat tissue. SWV(n=30) mice at age of 2 months and 12months old were divided into 3 groups as follows: (1) Old control group (n=10, O-Con), (2) Young control group(n=10, Y-Con) (3) Old+exercise group (n=10, O-Ex). The swimming exercise was performed 5 days /week with duration of 40minutes for 8weeks. BW and BG were measured once per week. Results showed that the BW of O-Con group was is higher than other groups, interestingly, BW of Old-Ex was significantly decreased (p<.05, p<.001) from 2nd. BG of O-Ex groups showed markedly decreased (p<.05, p<.001) from 2 weeks after exercise and continuously lower than other groups. Retroperitoneal fat fed weight of O-Ex group was significantly decreased (p<.001) and adipocyte size of O-Con group was bigger than other groups, but O-Ex group smaller sized adipocyte than other groups. Particulary, AMPK and GLUT-4 which are related with lipid metabolism was markedly inhibited compared to Y-Con group, whereas O-Ex group showed increased of AMPK and GLUT-4 protein expression than other groups. These results suggest that swim exercise for 8 week should inhibit adipocyte hypertrophy by controling the BW and decreasing retroperitoneal fat fed through AMPK protein expression and controling protective of BG the by decreasing the BG through GLUT-4 protein expression in fat tissue of aging mice. Thus, exercise may be helpful to improvement of lipid metabolism function and may be protective effect from lipid metabolism disease.

      • Analysis of Harvesting Part for Radish Harvester

        The agricultural labors are rapidly decreasing due to the aging of the agricultural population and the feminization. In Korea, radishes account for around 78.1% of the root vegetables in 2014, and the ratio of labor in harvesting and transporting work is about 26.7%. Therefore, it needs to develop the agricultural mechanization technology, especially radish harvester, because radish harvester of pulling type is directly contacted with soil during harvesting. Then, strength analysis of harvesting part is needed. In this study, finite element analysis was carried out using ANSYS Workbench in order to find out the weak point of the harvesting part of the harvester. The 3D model of the harvesting part including driving pulleys, shafts, and V-belts was designed by using CATIA V5 R18. The maximum load of 16.82 Nm measured in the field test was input to the driving shaft. The driving pulleys, shafts, and V-belts were simulated, and analyzed the deformation and equivalent stress. Simulation results revealed the maximum deformation and equivalent stress on the driving shaft. The results showed that the driving shaft of the radish harvester is the weakest point. Thus, the simulation results should be sufficiently considered during the design of the driving part of the harvesting part.
