We report the case of a seven-year-old boy with an ingested foreign body, which was retained within the appendix for a known duration of ten months, ultimately requiring appendectomy. The ingested foreign body was incidentally discovered by abdominal x-ray at an emergency room visit for constipation. Despite four bowel cleanouts, subsequent x-rays showed persistence of the foreign body in the right lower quadrant. While the patient did not have signs or symptoms of acute appendicitis, laparoscopic appendectomy was performed due to the risk of this foreign body causing appendicitis in the future. A small metallic object was found within the appendix upon removal. This case highlights the unique challenge presented by foreign body ingestions in non-verbal or developmentally challenged children and the importance of further diagnostic workup when concerns arise for potential retained foreign bodies.
The Vortex and the Gyres: Blake, Yeats, and the Shapes of History

This essay offers a brief comparison of William Blake and Yeats’s ideas of history and historical change by examining the prevalent figures used to delineate the shape of history: the vortex in Blake and the gyres in Yeats. It is argued that the two shapes allow the poets to represent radically distinct views on apocalypse and the possibility of release from history.
Clinical and preclinical tolerance protocols for vascularized composite allograft transplantation

The field of vascularized composite allografts (VCAs) has undergone significant advancement in recent decades, and VCAs are increasingly common and accepted in the clinical setting, bringing hope of functional recovery to patients with debilitating injuries. A major obstacle facing the widespread application of VCAs is the side effect profile associated with the current immunosuppressive regimen, which can cause a wide array of complications such as infection, malignancy, and even death. Significant concerns remain regarding whether the treatment outweighs the risk. The potential solution to this dilemma would be achieving VCA tolerance, which would allow recipients to receive allografts without significant immunosuppression and its sequelae. Promising tolerance protocols are being studied in kidney transplantation; four major trials have attempted to withdraw immunosuppressive treatment with various successes. The common theme in all four trials is the use of radiation treatment and donor cell transplantation. The knowledge gained from these trials can provide valuable insight into the development of a VCA tolerance protocol. Despite similarities, VCAs present additional barriers compared to kidney allografts regarding tolerance induction. VCA donors are likely to be deceased, which limits the time for significant pre-conditioning. VCA donors are also more likely to be human leukocyte antigen–mismatched, which means that tolerance must be induced across major immunological barriers. This review also explores adjunct therapies studied in large animal models that could be the missing element in establishing a safe and stable tolerance induction method.
What Korean Students Gain from Studying Abroad: English Proficiency and Cultural Awareness

This study explores how the experience of studying abroad impacted Korean students in relation to their professed improvement in their English abilities and their awareness about culture. It was based on qualitative inquiry into the lives of twenty-six Korean students who studied abroad. Verbal descriptions of experiences were collected through semi-formal interviews and informal conversations over a period of twenty-seven months, which were taped and transcribed. Data was analyzed inductively and findings emerged. The study findings show that all the students believed that their listening and speaking skills improved, and students who studied abroad longer were able to go well beyond their own expectations in relation to listening and speaking abilities. However, in regard to improvement of reading, and especially writing abilities, only degree seeking students thought these skills improved significantly. In addition, all students reported gaining cultural awareness, especially about differences between Korean culture and the host culture. However, the depth of their cultural awareness did not depend as much on length of their stay in the host culture as the kind of experience the student had. Study implications and limitations are also discussed.
