자궁내막증에서 Cyclins-CDKs 와 CDKIs에 의한 세포주기 조절경로에 관한 연구

목적 : 본 연구는 cisplatin이 자궁경부암세포주 HeLa세포에서 세포 고사를 유발하는 신호전달 과정에 세포내 산화제의 축적을 포함하는지의 여부를 알아보기 위하여 수행하였다. 연구 방법 : 자궁경부암 세포주 HeLa세포를 이용하여 체외 실험을 통해 항암치료중 활성산소종의 생성여부와 항산화 물질을 세포내 투입하여 cisplatin 유래 활성 산소종에 의한 세포사멸 보호효과를 flow cytometry에 의해 측정하였다. 결과 : cisplatin에 의하여 HeLa 세포주에서 ROS가 생성되었으며 항산화제 GSH전처리 시 ROS의 생성이 억제되었다. cisplatin에 의해서 활성화된 caspase-3, -9 protease는 GSH, 전처리시 활성화가 뚜렷이 억제되고 있음을 알 수 있었다. 결론 : cisplatin에 의해서 유도된 ROS의 영향이 caspase의 활성화에 관여를 하고 있음을 있었고, 또한 caspase계들의 활성은 ROS에 의한 것이며 세포고사를 일으키고 있음을 알 수 있었다. Objective : To determine whether oxidants are formed as part of the cisplatin-induced apoptotic process, intracellular markers of oxidative stress were examined. Methods : Apoptotic death of HeLa cells by cisplatin was confirmed by flow cytometry. Results : The pre-treatment with glutathione (GSH) significantly attenuated cisplatin-induced apoptosis through the reduction of reactive oxygen species (ROS) accumulation and diminished caspases-3 and 9 protease activity. Furthermore, z-VAD-fink, an inhibitor of pan-caspase, effectively inhibited the activation of caspases and prevented apoptosis by cisplatin, although cisplatin-induced ROS generation was not attenuated. Conclusion : These data indicate that ROS may play a role as an upstream mediator of caspases. Taken together, our results suggest that oxidative stress mediates cisplatin-induced apoptosis in HeLa cells.

간엽성 연골육종은 전형적인 연골육종과는 달리 매우 드문 것으로 알려져 있으며, 골격계 부위에서는 하지, 특히 대퇴골에서 대부분 발생하며, 다음으로 두안면부, 골반부에서 발생하는 것으로 보고되어 왔다. 11세 남아가 좌측 후족부 통증으로 타 병원에서 양성 골 종양 추정 하에 수술적 제거술을 시행 받고, 수술 후 조직검사 결과, 악성 종양으로 진단되어, 본원으로 전원 되었다. 조직 재 판독 결과, 좌측 종골 간엽성 연골 육종으로 진단되어, 슬하부 절단을 시행하였다. 외래 경과 관찰 중 술 후 3개월에 흉부 컴퓨터 촬영상 다발성 폐 전이성 결절 소견 보여 전이절제술 및 항암 치료를 시행하였다. 문헌상 간엽성 연골육종이 족저부 연부 조직에 발생한 경우는 1예가 있으나 종골 골조직에 발생한 경우는 없어 문헌 고찰과 함께 보고하는 바이다. Mesenchymal chondrosarcoma is very rare and mostly occurs in lower extremity, especially femur, head and face, and pelvis in order. An eleven-year old male patient suffered from left heel pain for several months and underwent biopsy at another hospital. A Malignant bone tumor was suspected, so the patient was transferred to our hospital. We reviewed the outside pathologic slide, which revealed the diagnosis of mesenchymal chondrosarcoma. It was treated by a below knee amputation. Three months later, a chest computed tomography (CT) revealed multiple metastatic nodules in both lungs. All metastatic nodules were surgically excised and the patient received postoperative chemotherapy. As far as our knowledge, mesenchymal chondrosarcoma in the calcaneus is the first case in the world. Thus, we report a case of mesenchymal chondrosarcoma in the calcaneus with thorough review of documents.

고립성 골연골종에서 기원한 골육종의 재발 및 육종 성분의 조직학적 변화

가장 흔한 양성 골 종양인 고립성 골연골종의 악성 변화는 대부분이 연골 육종이며, 골육종은 매우 드문 것으로 알려져 있다. 본원에서는, 30세 여자 환자의 대퇴골 원위부 후방에 생긴 고립성 골연골종에서 기원한 것으로 보이는 낮은 등급의 골수내 골육종이 초기 치료 후 3회 재발을 관찰하였으며, 이 과정에서 관찰한 병리 조직상 연골모세포성, 골모세포성 및 섬유모 세포성 골육종으로 각기 다른 조직학적 분화를 확인할 수 있었다. 이에 관한 보고는 현재까지 확인이 되지 않아, 매우 희귀한 사례로 판단되어 치험 사례를 문헌 고찰과 함께 보고하고자 한다. Malignant transformation of a solitary osteochondroma is rare, and usually takes form of a chondrosarcoma. We present a case of low grade osteosarcoma arising in a solitary osteochondroma of the right femur in a 30-year-old woman. As the lesion was initially continuous with corresponding components of the parent bone, so the possibilities of other diagnoses were excluded. After the initial excision, there were 3 times of recurrences during the follow up period of 3 years. The histologic subtypes of the recurred osteosarcomas were different each other, which were high grade chondroblastic, osteoblastic and fibroblastic respectively.

Second-Generation Metal-on-Metal Total Hip Arthroplasty

목적: 제 2세대 금속-금속 관절면을 이용한 고관절 전치환술의 단기 추시 결과를 보고하고자 한다. 대상 및 방법: 2000년 4월부터 2002년 3월 사이에 현대적 금속-금속 관절면을 이용한 일차성 고관절 전치환술을 시행 받은 환자 중 2년 이상 추시가 가능하였던 165명 169예의 고관절을 대상으로 하였다. 모든 환자에서 임상적 및 방사선적 평가를 시행하였다. 결과: Harris점수는 수술 전 평균 53.9점에서 최종 추시 평균 94.2점으로 향상되었다. 모든 삽입물에서 골침습성 고정을 확인할 수 있었다. 9명(10예)의 환자에서 대퇴골 대전자부에 국한된 골용해 소견을 보였다. 골용해 소견이 있는 환자들은 대조군에 비해서 코발트에 대한 피부 패치 검사 양성 반응률이 높게 나타났다(p=0.03l). 2명의 환자에서 각각 재발성 탈구와 대전자부 병적 골절의 위험성 때문에 재수술을 시행하였다. 회수된 삽입물들에 관절면의 마모소견은 없었으나, 삽입물 주위 조직에 대한 현미경 검사에서 지연형 과민반응에 해당되는 조직 소견이 관찰되었다. 결론: 현대적 금속-금속 관절 디자인을 이용한 고관절 전치환술의 단기 추시 결과는 대부분의 환자에서 고무적이었으나, 금속에 대한 알레르기 반응의 과거력이 있는 환자들에게는 삽입을 피해야 할것으로 사료된다. Purpose: To report the short-term results of second-generation metal-on-metal total hip arthroplasty (THA). Materials and Methods: One hundred and sixty-five patients (169 hips) were followed for more than 2 years after primary THA using a modem metal-on-metal bearing conducted between April 2000 and March 2002. All patients were examined clinically and radiographically. Results: The average Harris hip score improved from 53.9 points preoperatively to 94.2 points at the time of the last follow-up. All prostheses demonstrated radiographic evidence of bone ingrowth. Nine patients (10 hips) had an osteolytic lesion localized at the greater trochanter. Patients with osteolysis had a significantly higher rate of positive skin-patch tests for cobalt than controls (p=0.031). Of these, two hips underwent revision operation due to recurrent dislocation and imminent fracture through the greater trochanter, respectively. There was no evidence of metal wear on the retrieved bearing surfaces, but microscopic analysis of peri prosthetic tissues revealed abnormal histological findings consistent with delayed-type hypersensitivity. Conclusion: Early results of THA using the current metal-on-metal bearing design were encouraging in most of the patients. However, this implant bearing should be avoided in patients with a history of allergic reaction to metals.
