鮎川の習作期は<国家の領土拡大のための侵略戦争の開始>という政治ㆍ国家レベル、<詩の新しい領域の開拓>という文学ㆍ詩壇レベル、そして、早稲田高等学院入学による青春時代の到来という個人レベル、での三つの出来事とともに出発した。當時、西洋と日本では、文学の不安、特に詩の危機を迎えていた。その原因については、雑誌『新領土』に掲載された一連の評論を通じて垣間見ることができる。文学の大衆化と商業化による知識人の文学離れと遊戯化、「目先きばかり」の戦争文学の氾濫、「世界的標準」を教示できる批評家の不在、ラジオ放送と映画というニューメディアの登場、そして、文学における社会性の喪失があげられる。そこで、戦争という外部世界の変貌を、自作の中に採り入れることで文学の社会性回復を試みていたと思われる。その時、トマスㆍマンの『魔の山』の主人公が結核療養所から参戦のため、現実に連れ戻されて死んで行ったのは、自分と同じ世代の運命を象徴していることに、鮎川は気づいていた。そこから、彼の「遺言執行人」としての自己定位が始まったのである。

Augmentation with Transcortical Wiring of an Onlay-type Prosthesis for a Defi cient Patella during Revision Total Knee Arthroplasty

The management of patellae with a severe bony deficiency during revision total knee arthroplasty is a challenging problem. However, using a technique of augmentation with transcortical wiring of an onlay-type prosthesis allowed the authors to revise adefi cient patellae successfully. After making the decision to revise the existing patellar component, the procedure was found to betechnically straightforward. Furthermore, the procedure does not require sophisticated instruments, only an onlay-type prosthesis,cement and wires. This technique entails fi xing wires to the three pegs of the patellar component, passing the wires through drillholes in the anterior cortex and, after compression of a cemented prosthesis, augmenting the fi xation by twisting the wires anteriorly. We believe that stable fi xation and painless articulation will be obtained with the described technique for defi cient patellae.

자가 아킬레스 건과 장력 부하 기법을 이용한 전방십자인대 재건술 후 자기 공명 영상을 이용한 이식건의 평가

목적: 장력부하기법과 자가 아킬레스건을 이용하여 이식한 전방십자인대의 자기공명영상 소견을 분석하고 이를 정상전방십자인대의 자기공명영상 소견과 비교 평가하고자 하였다. 대상 및 방법: 1995년 1월부터 1996년 11월까지 장력부하기법과 자가 아킬레스건을 이용하여 전방십자인대 재건술을 시행 받은 환자 중 추시 자기공명영상을 검사한 21명 21예(그룹 A)의 자기공명영상을 분석하였다. 대조군(그룹 B)은 반월상 연골 파열로 관절경적 수술을 받은 환자 중 관절경적 소견상 정상 전방십자인대가 확인된 50명으로 구성되었다. 자기공명영상의 시상면 및 관상면에서 이식건의 위치를 측정하여 이를 정상 십자인대와 비교하였다. 결과: 그룹 A의 평균 전방십자인대 시상면 각도($55.7^{\circ}{\pm}5.6^{\circ}$, 범위, $47.2{\sim}68.8^{\circ}$)는 그룹 B의 각도($58.7^{\circ}{\pm}3.8^{\circ}$, 범위, $50.4{\sim}67.5^{\circ}$)보다 작았다(p=0.036). 그러나 평균 전방십자인대-Blumensaat 선 사이의 각도(그룹 A: $8.1^{\circ}{\pm}4.9^{\circ}$, 범위, $1.7{\sim}22.0^{\circ}$, 그룹 B: $8.6^{\circ}{\pm}3.6^{\circ}$, 범위, $2.6{\sim}18.1^{\circ}$)와 평균 전방십자인대 관상면 각도(그룹 A: $64.9^{\circ}{\pm}9.1^{\circ}$, 범위, $46.9{\sim}76.4^{\circ}$, 그룹 B: $65.9^{\circ}{\pm}4.4^{\circ}$, 범위, $57.7{\sim}75.2^{\circ}$)는 통계적으로 유의한 차이를 보이지 않았다(p=0.88) (p=0.62). 시상면에서 그룹 A의 평균 경골 터널 위치($31.9{\pm}7.1%$, 범위, $22.4{\sim}47.9%$)는 그룹 B의 위치($37.0{\pm}4.9%$, 범위, $18.5{\sim}44.7%$)보다 더 앞쪽에 위치하였다(p=0.005). 하지만 관상면에서의 위치는 두 군(그룹 A: $46.3{\pm}2.8%, 범위, $42.1{\sim}52.5%, 그룹 B: $45.7{\pm}2.8%, 범위, $41.0{\sim}49.1%) 사이에 통계적으로 유의한 차이를 보이지 않았다(p=0.392). 결론: 장력부하기법과 자가 아킬레스건을 이용하여 전방십자인대 제건술을 시행하였으며 추시 자기 공명 영상 검사를 통해 이식건이 정상 전방십자인대와 비슷하게 위치하였음을 확인할 수 있었다. Purpose: To evaluate the postoperative magnetic resonance image (MRI) findings of anterior cruciate ligament (ACL) reconstructed with a tension load technique using auto-Achilles tendon, and to compare the results with knees with a native ACL. Materials and Methods: We evaluated 21 postoperative MRI scan of 21 patients (group A) who had undergone ACL reconstruction between January 1995 and November 1996. The control group (group B) consisted of 50 patients whose meniscus tear had been operated by arthroscopy and whose ACL was intact. We measured the orientation of the graft in the sagittal and coronal planes and compared it with that of the native ACL. Results: The mean sagittal angle of the ACL angle in group A ($55.7{\pm}5.6^{\circ}$, range $47.2{\sim}68.8^{\circ}$) was statistically lesser than group B ($58.7{\pm}3.8^{\circ}$, range $50.4{\sim}67.5^{\circ}$) (p=0.036). But there was no statistically significant difference between the two groups with regard to the mean ACL-Blumensaat line angle (group A: $8.1^{\circ}{\pm}4.9^{\circ}$, range $1.7^{\circ}{\sim}22.0^{\circ}$, group B: $8.6^{\circ}{\pm}3.6^{\circ}$, range $2.6^{\circ}-18.1^{\circ}$) and the mean coronal angle of the ACL (group A: $64.9^{\circ}{\pm}9.1^{\circ}$, range $46.9^{\circ}{\sim}76.4^{\circ}$, group B: $65.9^{\circ}{\pm}4.4^{\circ}$, range $57.7^{\circ}{\sim}75.2^{\circ}$)(p=0.88, p= 0.62). In the sagittal plane, the mean center of tibial insertion of the ACL graft in group A ($31.9{\pm}7.1%$, range 22.4-47.9%) was positioned more anteriorly than group B ($37.0{\pm}4.9%$, range $18.5{\sim}44.7%$)(p=0.005). But in the coronal plane, there was no statistically significant difference between the two groups(group A: $46.3{\pm}2.8%$, range $42.1{\sim}52.5%$, group B: $45.7{\pm}2.8%$, range $41.0{\sim}49.1%$)(p=0.392). Conclusion: We performed an ACL reconstruction with the tension load technique using auto-Achilles tendon and we found that the graft orientation in MRI was as good as that of the native ACL.

Soft Tissue Balancing of Varus Arthritic Knee in Minimally Invasive Surgery Total Knee Arthroplasty: Comparison between Posterior Oblique Ligament Release and Superficial MCL Release

Purpose: To assess proper soft tissue balancing of the varus arthritic knee between posterior oblique ligament (POL) release group and superficial medial collateral ligament (SMCL) release group. Materials and Methods: This retrospective study was performed on 186 patients who underwent minimally invasive surgery (MIS) total knee arthroplasty (TKA) from January 2011 to December 2011. Eighty-three patients were in the group of SMCL release and 103 patients were in the POL release group. We intended to use a 10 mm polyethylene insert (PE) during TKA, and retrospectively compared the actual thickness of PE between POL release group and SMCL release group. Results: The mean PE thickness was 10.59±1.3 mm (range, 8 to 15 mm) in POL group and 11.88±1.8 mm (range, 10 to 18 mm) in SMCL group (p=0.001). We found a significant difference in the mean PE thickness between POL release group and SMCL release group. Conclusions: POL and deep MCL releases in MIS-TKA would be beneficial for varus deformity correction in the osteoarthritic knee.

萩原朔太郎と近代日本 - 同時代性を中心に -

従来、萩原朔太郎は近代日本のアウトサイダー詩人として見られてき た。しかし、前橋のセセッション様式の書斎の斬新さ、また東京の工場地帯とその周辺の住宅街、アパートなどに顕著に見られる同時代の変化への敏感な反応、こういったものから、朔太郎は時代から外れたアウトサイダーではなくむしろ時代の最先端を走り、それを自らの言語空間の中に受容してきたことがわかる。本稿では、<時代のパイオニアとしての朔太郎像>を詩と日記、當時の資料分析を通じて明らかにした。

日露戦争と『帝国文学』

日露戦争初期における東大アカデミズムと『帝国文学』の動きを、『帝国文学』1904年4月号の3篇の戦争詩と、同時期に刊行された教科書を中心に考察した。 まず、坪井九馬三の詩「征露進軍歌」は、『新体詩抄』に収録されている外山正一の「抜刀隊」の「我が官軍」というテーマを継承していた。 次に、坪井、上田万年、芳賀矢一等の東大アカデミズムの中枢グループは、<中等>と<軍人>用の教科書作りに深く携わっていたが、「振天府」をはじめ、日清戦争関連のテキストが中等と軍人用の教科書に繰り返し採用され、日清戦争勝利のシンボル化していた。 そして、各教科書には、様々な儀式に必要な祝辞と弔辞のテキストが載せられていた。そして、芳賀の詩「祝捷行軍歌」は、「捷報」ムードを盛り上げるにとどまらず、脱亜論までを強く打ち出していた。

Japanese Modern Literature and the war hero This paper discusses the process of deification of KyuGunsin found in Aikoku-Shisyu and Gunsin-Ni-Tsuzuke. Aikoku-Shisyu is a collection of patriotic poems read aloud on radio after the outbreak of the World War II. Miyoshi Tatsuji’s work, “The names of nine pearls”, is a poem that praises the KyuGunsin and compares them to pearls based on Japanese lyrical tradition. Gunsin-Ni-Tsuzuke was published to present KyuGunsin as heroes and puts emphasis on inheriting their patriotism.
