        JAK2 V617F mutation in myelodysplastic syndrome, myelodysplastic syndrome/myeloproliferative neoplasm, unclassifiable, refractory anemia with ring sideroblasts with thrombocytosis, and acute myeloid leukemia

        <P><B>Background</B></P><P>The JAK2 V617F mutation has been noted in the cases of polycythemia vera, essential thrombocythemia, and primary myelofibrosis patients. This mutation occurs less frequently in acute myeloid leukemia (AML) and other hematologic diseases, such as myelodysplastic syndrome (MDS); myelodysplatic syndrome/myeloproliferative neoplasm, unclassifiable (MDS/MPN-U); and refractory anemia with ring sideroblasts with thrombocytosis (RARS-T).</P><P><B>Methods</B></P><P>Patients diagnosed with hematologic diseases other than MPN who visited Seoul St Mary's Hospital from January 2007 to February 2010 were selected. A total of 43 patients were enrolled in this study: 12 MDS, 9 MDS/MPN-U, 7 RARS-T, and 15 AML patients. The diseases were diagnosed according to the 2008 WHO classification criteria. Data obtained from JAK2 V617F mutation analysis and cytogenetic study as well as complete blood count and clinical data were analyzed.</P><P><B>Results</B></P><P>Of the 43 patients, 6 (13.9%) harbored the JAK2 V617F mutation. The incidence of the JAK2 V617F mutation in each patient group was as follows: 8.3% (1/12), MDS; 22.2% (2/9), MDS/MPN-U; 14.3% (1/7), RARS-T; and 13.3%, (2/15) AML. The platelet count was higher than 450×10<SUP>9</SUP>/L in 3 of the 6 patients (50%) harboring the JAK2 V617F mutation, and it was in the normal range in the remaining 3 patients. Among the 6 patients, 1 MDS and 1 MDS/MPN-U patients had the 46,XX,del(20)(q11.2) karyotype.</P><P><B>Conclusion</B></P><P>The JAK2 V617F mutation is associated with an increased platelet count in MDS, MDS/MPN-U, RARS-T, and AML patients. Cytogenetic abnormalities of del(20)(q11.2) occurred in 1/3 of patients with the JAK2 V617F mutation but further studies are required to confirm this association.</P>

        Transfusion support in hematopoietic stem cell transplantation

        Transfusion support for hematopoietic stem cell transplantation (HSCT) is an essential part of supportive care, and compatible blood should be transfused into recipients. As leukocyte antigen (HLA) matching is considered first and as the blood group does not impede HSCT, major, minor, bidirectional, and RhD incompatibilities occur that might hinder transfusion and cause adverse events. Leukocyte reduction in blood products is frequently used, and irradiation should be performed for blood products, except for plasma. To mitigate incompatibility and adverse events, local transfusion guidelines, hospital transfusion committees, and patient management should be considered.

        스포츠 경기장의 음주 관련 정책 개입 효과 분석

        스포츠 경기장은 가족이 함께 운동경기를 관람할 수 있는 공공장소이나 음주 및 음주폐해가 많이 발생하는 장소이기도 하다. 폐해를 예방하려면 경기장 내 음주를 감소시키는 것이 필요하다. 본 연구는 스포츠 경기장에서 음주가 얼마나 이루어지는지, 경기장의 음주 관련 정책이 관람객의 음주여부에 어떤 영향을 미치는지를 확인하고자 하였다. 이를 위해 2014년부터 2019년까지 6년간 잠실 야구경기장에서 시행되었던 음주 모니터링 자료를 활용하여 ARIMA 분석을 하였다. 분석결과 음주 관련 규정이 없었던 2014년에 비해 음주 관련 규정이 시행된 2015년 이후 음주자 비율이 증감을 반복하면서 감소하는 추세를 보였으며, 규정을 엄격하게 준수할수록 음주자 비율이 감소하는 것을 확인하였다. 즉 주류반입금지규정과 주류이용가능성 제한 규정을 엄격하게 준수할수록 음주자 비율이 유의하게 감소하였으며, 특히 주류반입금지규정의 준수 정도가 더 크게 영향을 미치는 것으로 확인되었다. 이러한 결과는 스포츠 경기장에서의 음주 관련 규정을 마련하는 것과 함께 엄격하게 시행하는 것이 스포츠 경기장에서의 음주 및 음주폐해예방 감소에 기여할 수 있다는 것을 입증한 것으로 볼 수 있다. 이러한 효과가 나타날 수 있었던 것은 정책 시행 정도를 지속적으로 모니터링하고 감시하는 지역사회의 조직적 노력이 수반되었기 때문이다. It is increasingly recognized that alcohol consumption is a key factor in harms caused by alcohol misuse at a public place like a professional sports stadium, where family members, including children, come together to enjoy sports events. This paper aims to describe the prevalence of alcohol use and to analyze the effect of multi-component alcohol interventions on reducing alcohol consumption among spectators at a professional baseball stadium in Seoul. Data under analysis were derived from annual reports of 6 years, from 2014 to 2019, on alcohol use and policy implementation monitored at Jamsil professional baseball stadium, initiated by Songpa Public Health Center(SPHC). A time-series analysis was employed to examine the trends in the prevalence of alcohol use at sporting events over a period of 6 years. In 2015 the Korea Baseball Organization initiated a campaign, SAFE, a multi-component intervention to reduce alcohol-related problems at sports venues across the nation. The campaign included restriction on alcohol availability by imposing alcohol restriction on entry, the prohibition of alcohol sales after 7th inning, the imposition of drink limit per person. In addition to the campaign, SPHC mobilized a coordinated effort to support the campaign through sharing information on annual monitoring reports of alcohol use and advocating for stricter enforcement of SAFE policy among stakeholders including a civil society, professional baseball clubs, a venue management agency, and public health professional. ARIMA analysis showed that the campaign contributed to reduction of prevalence of alcohol use, from 42.2% to 18.3%. The results suggest that the restriction of alcohol availability at sporting venues, even though it is self-regulated, lead to reduction of alcohol use if implementation of it is closely monitored with community mobilization followed. Policy implications for health promotion were discussed.

        Evaluating Diagnostic Tests for Helicobacter pylori Infection Without a Reference Standard: Use of Latent Class Analysis

        Evaluation of diagnostic tests requires reference standards, which are often unavailable. Latent class analysis (LCA) can be used to evaluate diagnostic tests without reference standards, using a combination of observed and estimated results. Conditionally independent diagnostic tests for Helicobacter pylori infection are required. We used LCA to construct a reference standard and evaluate the capability of non-invasive tests (stool antigen test and serum antibody test) to diagnose H. pylori infection compared with the conventional method, where histology is the reference standard. A total of 96 healthy subjects with endoscopy histology results were enrolled from January to July 2016. Sensitivity and specificity were determined for the LCA approach (i.e., using a combination of three tests as the reference standard) and the conventional method. When LCA was used, sensitivity and specificity were 83.8% and 99.4% for histology, 80.0% and 81.9% for the stool antigen test, and 63.6% and 89.3% for the serum antibody test, respectively. When the conventional method was used, sensitivity and specificity were 75.8% and 71.1% for the stool antigen test and 77.7% and 60.7% for the serum antibody test, respectively. LCA can be applied to evaluate diagnostic tests that lack a reference standard.

        음주운전과 음주문제와의 관련성

        Objectives: This study was conducted to describe relations between the level of problematic drinking and drinking drive. Methods: This study employed the secondary data analysis in which raw data was derived from Korea National Health & Nutrition Examination Survey collected in 2007. A sample of 1,146 adults aged 19 year and over who were motorcycle or car drivers was analyzed by using logistic regression analyses. Results: Year prevalence rate of drinking drive was 17.4%. The higher level of problematic drinking was, the more they had drinking drive, and odd ratio to do drinking drive among those with alcohol use disorder was 7.5 times higher compared to social drinkers. Drinking drive was associated with sex, occupation, safety consciousness level, and the level of problem drinking. Conclusions: A positive relation was found between level of problem drinking and drinking drive after controlling for socio-demographic characteristics and safety consciousness of the driver

        Diagnosis of Liver Fibrosis With Wisteria floribunda Agglutinin-Positive Mac-2 Binding Protein (WFAM2BP) Among Chronic Hepatitis B Patients

        Background: Wisteria floribunda agglutinin-positive Mac-2 binding protein (WFA-M2BP) is a protein with altered glycosylation that reacts with lectin, and was recently identified as a useful non-invasive biomarker for the diagnosis of liver fibrosis in patients with hepatitis C virus infection.This study aimed to evaluate the diagnostic efficacy of WFA-M2BP for liver fibrosis in the context of hepatitis B virus (HBV). Methods: We enrolled 151 patients infected with HBV. Liver biopsy and elastography (Fibroscan) were performed during the initial visit. Fibrosis was graded according to the Knodell histologic activity index (F0–3). WFA-M2BP levels were determined with an automated immunoassay analyzer (M2BPGi, HISCL-5000, Sysmex, Japan). The diagnostic efficacy of WFA-M2BP was compared with those of various conventional or composite biomarkers, including enhanced liver fibrosis (ELF) score, Fibroscan, aspartate transaminase (AST)-toplatelet ratio index (APRI), and FIB-4, based on the area under the ROC curve (AUC) value. Results: The majority of patients were at fibrosis stages F1 and F2. The F2 and F3 AUC values for WFA-M2BP were similar to those for FIB-4, APRI, ELF, and Fibroscan, although the latter showed the best diagnostic efficacy. The diagnostic accuracy of all tested biomarkers for F2 and F3 was 60–70%. In multivariate analysis, WFA-M2BP, ELF, and platelet count significantly predicted stage ≥F2, whereas only platelet count significantly predicted F3. Conclusions: WFA-M2BP can support a diagnosis of liver fibrosis with similar diagnostic efficacy to other biomarkers, and predicted liver fibrosis stage ≥2 among patients with chronic hepatitis B.

        지식경영을 활용한 기업인수 후 합병후 통합(PMI): 한일은행과 상업은행의 합병 사례를 중심으로

        The purpose of this study lies in examining how knowledge management was applied to PMI(Post Merger Integration) in bank merger. Individuals and organizations in Korea have experienced a lot of changes since the 1997 foreign exchange crisis. In such a situation, individuals came to think the only thing to rely on was personal knowledge. Since organizations had to lay off workers in order of their birth year, not based on whether or not individuals had the knowledge necessary for the organizations, they needed to have such a system that could use their explicit knowledge and even outside knowledge or customers` knowledge as IT developed by changing individuals` tacit knowledge into explicit knowledge as needed in order to continue to maintain their competitiveness and for their own development. Thus, each firm started to pitch for the introduction of knowledge management. Individuals started to store their own experience and knowledge in their homepages or blogs. It was Woori Bank, the merged bank of Hanil Bank and Commercial Bank of Korea, that introduced the knowledge management system and Shared Service Center, in which knowledge creation is available, for the first time in the banking business. Its previous name was Hanbit Bank. Hanbit Bank wanted to construct an advanced bank system, bringing in their chief information officer(CIO) from a foreign bank and introducing an IT software used in Spanish banks to adjust all banking processes to it. However, they could not help giving up the plan in the middle of the road since there was a great difference between Spain and Korea in the financial system and more than 30% of software package had to be changed. In this situation, PMI was delayed, and customer inconvenience continued, which made the integration of organizational cultures slow down. As a breakthrough in this situation, knowledge management was introduced. To integrate knowledge of two organizations in the process of PMI is an important job for all merger candidates. This study aims at presenting the successful results from using knowledge management as a means to PMI ahead of other financial institutions so they can apply them to their organizations. PMI was not achieved properly after the two banks had been merged as Hanbit, but entering the era of Woori Bank, workers were integrated under one standard organization following the organizational and knowledge integration, and knowledge management was introduced for an efficient sharing of knowledge among members. A great number of mergers have occurred up to now, but Woori Bank is the first case that used knowledge management as a means to both PMI and competitiveness enhancement. Probably it was an appropriate time when Woori Bank introduced knowledge management as it was organized. Since Hanil Bank and Commercial Bank of Korea had not introduced knowledge management until then, it could use knowledge management as a means to PMI. Using knowledge management, it could create a new organizational culture and increase competitiveness in the banking industry.
