Clavulanic Acid Is a Leading Culprit Beta-Lactam in Immediate Allergic Reactions to Penicillins

Purpose: Clavulanate, a beta-lactam associated with amoxicillin, is frequently prescribed in patients at all ages. Recent data implicate amoxicillin-clavulanate in up to 80% of beta-lactam allergy cases. We assessed clavulanate’s role in inducing allergic reactions to this combination treatment, with a focus on selective immediate reactions. Methods: Adults (≥ 16 years) reporting a history of immediate reactions to amoxicillin-clavulanate were evaluated through a beta-lactam allergological workup, using modified European Academy of Allergy and Clinical Immunology guidelines. Patients first underwent skin testing, and if negative, drug provocation tests. Expected outcomes were: Group A, subjects with immediate reaction to classical penicillin group determinants (penicilloyl polylysine, minor determinants mixture, and/or penicillin G); Group B, subjects with selective immediate reaction to amoxicillin; Group C, subjects with selective immediate reaction to clavulanate and Group D, those immediate reactions with co-sensitization to clavulanate plus penicillin group determinants or amoxicillin. Results: Of 1,170 included patients, 104 had immediate reactions: 36.5% to penicillin group determinants (Group A), 26.9% to amoxicillin (Group B), 32.7% to clavulanate (Group C), and 3.8% to clavulanate plus penicillin determinants or amoxicillin (Group D). Diagnosis was made by skin testing in 79%, 75% and 47% of the patients, respectively, in the first 3 groups (P < 0.001). Drug provocation tests were necessary to establish most other diagnoses. Anaphylaxis predominated over urticaria/angioedema in all groups. Conclusions: Selective immediate reactions to clavulanate accounted for over a third of cases with confirmed reactions after amoxicillin-clavulanate intake, with more than half experiencing anaphylaxis. Within this group, skin test sensitivity was below 50%. People taking amoxicillin-clavulanate may also be co-sensitized to both drugs.
Zn-22Al-2Cu Alloy Phase Transformations at Different Homogenizing Temperatures

The effect of the homogenizing temperature on phase transformations and hardening behavior was studied in a Zn-22Al-2Cu (wt%) alloy homogenized for 7 days at 315, 350, and 390°C. The homogenized alloy was subsequently quenched and aged naturally at room temperature or artificially at 200°C for different time periods. The β phase was unstable at room temperature and it decomposed through the β→α+ηreaction during natural aging. An increase in the homogenizing temperature caused a decrease in the kinet-ics of the decomposition of the β phase in the alloy. Natural aging also caused an increase in the hardness due to the formation of nanometric grains with α and η phases and the presence of an intermetallic εphase. This result agrees with the time exponents (n) between 1.5 and 2.6, which were obtained using the Johnson-Mehl-Avrami-Kolmogorov equation for the decomposition of the β phase. The transformed frac-tion curves exhibited the fastest and slowest growth kinetics of the â phase decomposition at homogeniz-ing temperatures of 350 and 390°C, respectively. The artificial aging induced the formation of equilibrium phases and began a decrease in hardness due to the coarsening process of the equilibrium α, η, and τ'phases.
Severe Weakness of Hip Flexor after Iliopsoas Tenotomy: Two Case Reports

Hip arthroscopies are becoming a standard surgical technique, with psoas tenotomy being a relatively common procedure during this operation. A 37-year-old male and a 42-year-old female with internal hip snapping came to our department. Arthroscopic partial psoas tenotomy of the iliopsoas portion of the conjoint tendon was performed, but its results were bad. Patients reported preoperative unilateral low-back pain and weakness when flexing the hip. Unilateral atrophy of the lumbar psoas and fatty multifidus were detected in both cases. Patients at risk of unsatisfactory outcomes after psoas tenotomy should be ideally identified prior to surgery. Warning symptoms, physical examination, and imaging studies should be considered to avoid unsatisfactory results.

Where concrete structures are designed to have a service life of over 100 years, their performance must be monitored, for the prediction models available are fraught with uncertainties that need to be elim- inated. The present study was conducted to meet that need by monitoring a pilot structure for low and intermediate radioactive waste storage. Long-term operation of the sensors was observed to be adequate to determine the value of the parameters that characterise structural durability, such as corrosion current density. The parameters analysed were correlated to calculate their reciprocal impact: where applied in conjunction with artificial intelligence tools, temperature, for instance, was found suitable for finding activation energy and expansion coefficients and detecting outliers. The results showed the pilot structure to perform satisfactorily.

To determinate the antimicrobial effect of chloramphenicol and aqueous extract against multidrug-resistant enterohemorrhagic Escherichia coli (EHEC) and Salmonella enterica serovar Typhimurium in CD-1 mice. Aqueous extract was isolated from Hibiscus sabdariffa calyces. The minimum inhibitory concentration (MIC) and the minimum bactericidal concentration (MBC) of chloramphenicol and aqueous extract were determined for EHEC and S. Typhimurium. Nine groups of six mice each were formed. Three groups were inoculated orally with 1 × 104 colony-forming units (CFU) of S. Typhimurium, three groups were inoculated with 1 × 104 CFU of EHEC and the remaining three groups were not inoculated. Six hours postinoculation, the mice of some groups were orally administered solutions of aqueous extract (50 mg/mL), chloramphenicol (82 μg/mL), or isotonic saline. The EHEC and S. Typhimurium concentration in all mice feces was determined. For both pathogens, the MIC and MBC values of aqueous extract were 20 y 50 mg/mL, respectively; for chloramphenicol, they were between 17.5 and 82 μg/mL. EHEC and S. Typhimurium were not detected in the feces of mice that were administered aqueous extract on the 2nd and 3rd days posttreatment. Furthermore, these mice recovered from the infection. In contrast, in mice not treated, or treated with chloramphenicol alone, pathogens were isolated from their feces throughout the study, and some mice died. The H. sabdariffa calyx extracts could be an alternative to control multidrug-resistant bacteria in humans and animals.

The aortic bifurcation and iliac vessels are common sites of atherosclerotic occlusive disease causing the clinical expression known as “Leriche’s syndrome”. An aortobifemoral bypass grafting in the setting of a septic groin remains a significant challenge to vascular surgeons. We present a 65-year-old male with complete occlusion of the distal aorta and iliac arteries; he had undergone a leftaxillo-femoral and femoral-femoral artery bypass 2 years prior to our evaluation. Owing to a complex graft infection in the right groin and worsening lower extremity ischemia, we performed an aortobifemoral reconstruction through the right obturator membrane. This report highlights the safety and efficacy of the obturator bypass for avoiding infected groins while preserving vascular continuity and durability with 78 months of secondary patency rate.

Marfan syndrome is a connective tissue disorder associated with aortic dissection, aneurysmal degeneration and rupture. These cardiovascular complications represent the main cause of mortality, therefore repair is indicated. We present a 35-year-old woman who experienced acute onset of chest pain. Her imaging revealed a chronic DeBakey type I dissection with aortic root dilation and descending thoracic aneurysmal degeneration. She underwent a Bentall procedure and endovascular exclusion of the descending thoracic aortic aneurysm. She was closely followed and 2 years later a computed tomography angiography (CTA) revealed the aneurysmal degeneration of the thoracoabominal aorta and bilateral iliac arteries. The patient underwent a composite reconstruction using multivisceral branched and bifurcated Dacron grafts. At 5 years from her last surgery, a CTA revealed no new dissection or further aneurysmal degenerations. Aortic disease in Marfan patients is a complex clinical problem that may lead to secondary or tertiary aortic reconstructions; close follow-up is mandatory.

Background and Purpose Ischemic stroke is a common cause of death worldwide. In clinical practice it is observed that many individuals who have experienced an ischemic stroke also suffer from simultaneous comorbidities such as heart failure, which could be directly associated with a worse clinical prognosis. Therefore, this study analyzed outcomes in terms of the severity of the event, inhospital mortality, duration of hospital stay, and inhospital recurrence of the episode, in order to determine the implications resulting from the presentation of both pathologies. Methods This was a retrospective-cohort, hospital-based study. Results The study included 110 subjects with heart failure (exposed) and 109 subjects without heart failure (nonexposed). The incidence of inhospital mortality was 27.27% in exposed patients and 9.17% in nonexposed patients (p<0.001), and the presence of heart failure increased the risk of death by 92% (p=0.027). According to scores on the National Institutes of Health Stroke Scale, the median severity was worse in exposed than nonexposed patients (16.1 vs. 9.2, p=0.001). The median hospital stay was 9 days in subjects with heart failure and 7 days in nonexposed patients (p=0.011). The rate of inhospital stroke did not differ significantly between exposed and nonexposed patients (1.82% vs. 0.92%, p=0.566). Conclusions Individuals with heart failure who suffer from an acute ischemic stroke show worse clinical outcomes in terms of mortality, event severity, and duration of hospital stay.

Fermentation with filamentous fungi in a bioreactor is a complex dynamic process that is affected by flow conditions and the evolution of the rheological properties of the medium. These properties are mainly affected by the biomass concentration and the morphology of the fungus. In this work, the rheological properties of a fermentation with the fungus Beauveria bassiana under different hydrodynamic conditions were studied and the rheological behavior of this broth was simulated through a mixture of carboxymethyl cellulose sodium and cellulose fibers (CMCNa-SF). The bioreactor was a 10 L CSTR tank operated at different stir velocities. Rheological results were similar at 100 and 300 rpm for both systems. However, there was a significant increase in the viscosity accompanied by a change in the consistence index, calculated according to the power law model, for both systems at 800 rpm. The systems exhibited shear-thinning behavior at all stir velocities, which was determined with the power law model. The mixing time was observed to increase as the cellulose content in the system increased and, consequently, the efficiency of mixing diminished. These results are thought to be due to the rheological and morphological similarities of the two fungal systems. These results will help in the optimization of scale-up production of these fungi.
Experimental and numerical study on the detection of fatigue failure in hydraulic turbines

Detecting fatigue cracks in hydraulic turbine runners is costly, as it requires stopping the unit, emptying it of water and accessing the runner for inspection. Thus, an alternative way based on monitoring the changes of the structural modal response induced by the formation and growth of a crack was investigated. To do so, the crack propagation induced by a resonance was numerically predicted and experimentally machined on a disk-like structure that resembles a Kaplan turbine runner. The analysis of the results shows how the different stages of the fatigue crack growth can be monitored based on the change of the natural frequencies and mode shapes of several specific modes. Based on the obtained results, a structural health monitoring system is being designed to monitor the turbine runner modes of vibration without the need to stop and inspect the unit.
