        Retrospective long-term analysis of bone level changes after horizontal alveolar crest reconstruction with autologous bone grafts harvested from the posterior region of the mandible

        Purpose: The goal of this study was to evaluate the long-term success of horizontal alveolar crest augmentation of the retromolar region of the mandible with particulated bone, as well as factors affecting subsequent peri-implant bone loss. Methods: A total of 109 patients (68 female, 41 male) suffering from alveolar ridge deficiencies of the maxilla and mandible were included in this study. All patients were treated with particulated retromolar bone grafts from the mandible prior to the insertion of endosseous dental implants. Mesial and distal peri-implant crestal bone changes were assessed at six time points. Several parameters, including implant survival and the influence of age, gender, localisation of the implant, diameter, covering procedures, and time points of implant placement, were analysed to identify associations with bone level changes using the Mann-Whitney U-test, the Kruskal-Wallis test, and Spearman’s rank-order correlation coefficient. Results: A total of 164 dental implants were placed in the maxilla (n=97) and in the mandible (n=67). The mean observation period was 105.26 ± 21.58 months after implantation. The overall survival rate was 97.6% after 10 years. Overall, peri-implant bone loss was highest during the first year, but decreased over time. The mean amount of bone loss after 10 years was 2.47 mm mesially and 2.50 mm distally. Bone loss was significantly influenced by implant type and primary stability. Conclusions: The use of particulated autologous retromolar bone grafts is a reliable technique for the horizontal reconstruction of local alveolar ridge deficiencies. Our results demonstrate that implants placed in augmented bone demonstrated similar bone level changes compared to implants inserted in non-augmented regions.

        Der Beitrag zeigt die Bedeutung und rechtlichen Implikationen elektronischer Dokumente im deutschen Strafverfahren auf. Beleuchtet werden zweierlei Aspekte: 1. Das elektronische Dokument als Beweismittel im Strafverfahren, 2. Die geplanten Anderungen im Rahmen der Einfuhrung elektronischer Akten im Strafverfahren. Im Rahmen der Erorterung des ersten Aspektes wird zunachst beleuchtet, wie elektronische Dokumente uberhaupt als Beweismittel in das Strafverfahren eingefuhrt werden konnen. Sodann wird die Problematik der ruckstandslosen Manipulierbarkeit erlautert bevor die einzelnen Schritte der gerichtsfesten Beweissicherung dargestellt werden. Im zweiten Teil werden die vom Bundesministerium der Justiz Mitte 2012 vorgestellten Anderungsvorschlage zur Strafprozessordnung (StPO) betrachtet. Geplant ist die Abkehr von Papierakten und die Einfuhrung elektronischer Akten im Strafverfahren. Die Losungsvorschlage und die mit der Einfuhrung zusammenhangenden Problemstellungen werden skizziert.

        축제의 역동적 흐름의 관점에서 본 현재의강릉 단오제의 연행(演行)적 문제점

        연행(演行)적(performative) 관점에서 보면 민속축제라는 것은 많은 사람들이동시에 나타나게 하는 에너지적 사건으로 여겨질 수 있고 축제의 모든 움직임들은 에너지적 흐름(flow)으로 간주할 수 있다. 민속축제의 에너지적 흐름은 일상적 습관과의 다른 규칙성과 반복성을 특징으로 하는데, 에너지 흐름의 규칙적인패턴들은 민속축제의 의식(儀式)과 놀이에서 뚜렷하게 나타난다. 무당굿, 유교식 제의, 가면극, 농악, 민속놀이, 시가행렬 등과 같은 다양한 형식들이 포괄되는 강릉단오제의 경우에는 주로경로(徑路), 횡단(橫斷), 왕복진자(往復·振子), 승강(昇降), 순환원형, 집중확산, 상호침투와 같은 흐름 패턴이 형성된다. 이러한 흐름들은 한편으로 신체적·체험적 의미, 다른 한편으로는 문화적·종교적·상징적 의미를 갖고 있으며 심리학자 미하이 칙센트미하이(MihalyiCsikszentmihalyi)가 특별한 정신적 상태로서 정의하는 ``흐름(flow)`` 체험과 관련하여 이해할 수도 있다. 이렇게 보면 축제에서 활성화되는 심신(心身)적 에너지흐름은 축제에 그 내재적·체험적 의미를 부여시키며 축제의 정신과 특징을 살려준다. 그러나 오늘날에 진행되는 민속축제에서는 옛날의 전통 축제에 없었던, 새로추가된 요소들로 인하여 전통적 민속축제의 자연스러운 에너지흐름이 방해되고일시적으로 중단되거나, 혹은 완전히 정지될 수 있는데, 이러한 흐름(flow)을 방해하거나 중지시켜버리는 현상은 멈춤(freeze)이라고 부를 수 있다. 강릉단오제에서멈춤 현상은 주로 공간적 환경(예를 들면 전통연희에 어울리지 않은 무대형식),매체적 상황(과다한 촬영활동), 그리고 행사의 조직적 구조(부적당한 교육행사) 등세 가지 요인과 관련되어 있다는 것으로 볼 수 있다. 민속 축제의 정신과 문화적·심리적 의미를 직접적으로 체험하게 해 주는 흐름을 계속해서 활성화하고 유지할 수 있기 위해서 멈춤의 문제에 대해 의식화할 필요가 있다고 생각한다. From a performative perspective, a folk festival can be regarded as an energetic event, created by the collective movements of all the festival participants. The collective movements of the festival can be described as specific forms of energetic flow. The energetic flow we find in ritual and play two elements which can be considered as main constituents of the traditional folk festival is distinguished by certain patterns, whose regularity and repetitiveness are fundamentally different from those of everyday routines, since they do not serve to automatize certain actions but to stress their specific meaningfulness. The fundamental energetic patterns, which can be found at the Dano Festival in Gangneung, which in this study serves as a representative example of the traditional Korean folk festival, as it comprehends various artistic and ritual forms, such as procession, shaman ritual, Confucian ritual, mask dance, folk games, folk music etc., can be described as path traverse, see-saw, up and down, circle, concentration and diffusion, and interpenetration. Cultural and symbolic meanings, but also physical experience. All these patterns can be understood on the one hand in terms of specific cultural symbolic meanings, and on the other as purely physical and mental experiences, sometimes also related to special states of mind, called the "flow experience" by the American psychologist Csikszentmihalyi. Flow can be regarded as the experiential quality that provides an intuitive understanding of the cultural and psychological meaning of the festival. In today`s folk festival, however, there can sometimes be found some modern elements, which may constraint the natural flow of the traditional festival, a phenomenon which can be described with the concept of "freeze". The cases of freeze that occur in the Dano Festival of Gangneung are considered to be mainly due to the three following factors: spatial environment (e.g. inappropriate staging of traditional play), use of media (e.g. excessive use of cameras), and organizational structure of the festival (e.g. lacking distance between traditional contents and educational programs). In order to preserve the flow and the spirit of the traditional folk festival it seems to be desirable to be aware of the problems of freeze that may occur in the context of modern forms of traditional folk festivals.

        Development and Validation of an Energy Management System for an Electric Vehicle with a split Battery Storage System

        Within the project ‘e performance’ supported by the German Ministry of Education and Research (BMBF) an electric vehicle, powered by two lithium-ion battery packs of different capacity and voltage has been developed. The required Energy Management System (EMS) in this system controls the current flows of both packs independently by means of two individual dc-dc converters. It acts as an intermediary between energy storage (battery management systems-BMS) and the drivetrain controller on the vehicle control unit (VCU) as well as the on-board charger. This paper describes the most important tasks of the EMS and its interfaces to the BMS and the VCU. To validate the algorithms before integrating them into the vehicle prototype, a detailed Matlab / Simulink-model was created in the project. Test procedures and results from the simulation as well as experiences and comparisons from the real car are presented at the end.

        Mid- to long-term success rate and functional outcomes of acromioclavicular injections in patients with acromioclavicular osteoarthritis

        Background: Acromioclavicular (AC) osteoarthritis (OA) is a frequent pathology of the shoulder in elderly patients. Drug injection plays an important role in treatment of AC OA. Literature has demonstrated excellent short-term results regarding shoulder function and pain. However, mid- to long-term results are lacking. The aim of this study was to assess the efficacy of a single intra-articular AC injection in patients with AC OA and to identify predictive factors for success. Methods: A retrospective study was performed to analyze success rate, shoulder function, and pain perception after a single intra-articular injection in patients with AC OA. Success was defined as the absence of reinterventions such as additional injection or surgery. Outcome measures were 1-year success rate and clinical outcome scores of Numeric Rating Scale (NRS) for pain, Oxford Shoulder Score, and Subjective Shoulder Value. Results: Ninety-eight patients participated in this study. At a median final follow-up of 0.8 years (interquartile range, 0–6), 57 of these patients (58%) had undergone a reintervention. The 1-year success rate was 47% (95% confidence interval, 37%–57%), with NRS at rest as the sole factor significantly associated with success. Thirty patients not requiring reintervention reported significant improvement from baseline for all reported outcome measures at final follow-up. Conclusions: AC injections offer a 1-year success rate of 47%. The AC injection produces good mid- to long-term clinical outcomes regarding shoulder function, quality of life, and pain perception in one-third of patients. Further research is essential to analyze mid- to long-term outcomes of AC injections.
