      • A case of neonatal lupus erythematosus

        A 1 month old female was referred to our clinic for her newly developed skin lesion, with mother currently being treated for autoimmune hepatitis. The lesion exhibited erythematous annular patches on whole body. FANA titer was 1:640 in infant and Anti-SSA, SSB antibodies were positive in both mother and infant. Punch biopsy showed interface dermatitis with linear deposition of immune complex deposition of IgM along the dermo-epidermal junction. Based on the findings the patient was diagnosed as neonatal lupus erythematosus. Echocardiogarphy showed only patent foramen ovale, EKG and other laboratory evaluations did not show any abnormalities. She received topical steroid treatment and skin lesions faded gradually during following 7 months. Neonatal lupus erythematosus is a rare acquired autoimmune disease occurring in an infant whose mother has autoimmune disorder. The disease is caused by trans-placental passage of maternal IgG antibodies against SSA, SSB, and/or U1RNP. Cutaneous and cardiac complications are most common presentations. Hepatic, hematological, neurological, and pulmonary system may also be involved. Diagnosis of NLE is made by clinical features with serological findings. Most symptoms are transient, but cardiac problems can persist and is associated with mortality. Therefore careful diagnosis and evaluation needed. Herein we report a case of NLE with reviewing literatures.

      • Simultaneous and Interference-Free Detection of Hydroquinone and Catechol on Poly (Evans Blue)-Modified Glassy Carbon Electrode

        <P>A simple and sensitive electrochemical sensor for the simultaneous detection of hydroquinone (HQ) and catechol (CT) is developed based on a poly (Evans blue) (PEB)-modified anodized glassy carbon electrode (AGCE). The PEB/AGCE sensor shows excellent electrocatalytic activity and reversibility toward the oxidation of both HQ and CT in phosphate buffer solution (PBS, pH 7.0). Both AGCE and PEB/AGCE can easily resolve the redox responses of HQ and CT from their mixture solution with the same peak-to-peak potential (Delta Ep) separations of ca. 0.10 V, whereas bare GCE cannot resolve them. The PEB/AGCE sensor exhibits significantly higher sensitivity with a linear dynamic range 1-100 mu M for both HQ and CT. Furthermore, the sensor shows detection limits (S/N = 3) of ca. 0.13 and 0.12 mu M for HQ and CT, respectively, with sensitivities of ca. 1.01 and 0.95 mu A/cm(2)/mu M, respectively. The sensor does not exhibit any significant interference for the detection of HQ and CT in the presence of common interferences. This study provides a simple and sensitive method with excellent recoveries for the simultaneous and quantitative determination of HQ and CT from tap water. (C) 2016 The Electrochemical Society.         Primary Radiation Therapy of Polymorhic Reticulosis

        1979년부터 1987년까지 서울대학교 병원에서 상기도에 국한된 다형성 세망중으로 근치적 방사선 치료를 받은 33명의 환자의 치료성적을 분석하였다. 발열, 체중감소, 도한등의 전신적 증상은 대상 환자중 48%에서 나타났으며 침윤병소로는 비강이 85%로 가장 많았고 부비동 및 구개부가 각각 33%와 30%이었다. 전체 환자의 5년 및 10년 생존율은 각각 47%와 40%이었다. 방사선 조사야 내에서 재발한 경우와 그렇지 않은 경우간의 방사선 선량 차이는 통계학적으로 유의하였으며 조사야 경계부위에서 재발한 경우는 그렇지 않은 경우에 비해서 조사야 면적이 작았다. 추적 관찰 기간중 악성 임파종 4예, 골수 세망증 1예 등의 전신적 전이가 확인되었다. 따라서 방사선 치료를 시행하여 다형성 세망증의 국소 치료율은 향상시키기 위해서는 최소 4,500cGy의 선량과 병소주위의 조직을 충분히 포함하는 조사야를 사용해야 하며 전신적 전이를 효과적으로 치료할 화학요법 면에서의 개선이 필요하다고 판단된다. From 1979 to 1987, 33 patients with polymorphic reticulosis (PMR) limited in the upper airway were treated with primary radiation therapy and the analysis of treatment results was undertaken. Systemic symptoms such as fever, night sweats, and weight loss were noted in 48%. The nasal cavity was most frequently involved (85%), although involvement of PNS (33%) and palate (30%) was not uncommon. The 5 and 10 year actuarial survival rates were 47% and 40%, respectively. The difference in NSD between patients with in-field failure and those without it was significant statistically. Also, field size was significantly smaller in patients with marginal failure than those without it. During the follow-up period, systemic failure was found in 5 patients (diffuse histiocytic lymphoma in 4, histiocytic medullary reticulosis in 1). In this study, we can suggest that total radiation dose of at least 4500 cGy with generous treatment volume should be delivered to achieve better local control and that the development of an effective systemic chemotherapeutic regimen is required to improve the survival after systemic relapse.

        Change in management and outcome of mechanical ventilation in Korea: a prospective observational study

        Background/Aims: There are few studies describing contemporary status of mechanical ventilation in Korea. We investigated changes in management and outcome of mechanical ventilation in Korea. Methods: International, prospective observational cohort studies have been conducted every 6 years since 1998. Korean intensive care units (ICUs) participated in 2010 and 2016 cohorts. We compared 2016 and 2010 Korean data. Results: Two hundred and twenty-six patients from 18 ICUs and 275 patients from 12 ICUs enrolled in 2016 and 2010, respectively. In 2016 compared to 2010, use of non-invasive ventilation outside ICU increased (10.2% vs. 2.5%, p = 0.001). Pressure-control ventilation was the most common mode in both groups. Initial tidal volume (7.1 mL/kg vs. 7.4 mL/kg, p = 0.372) and positive end-expiratory pressure (6 cmH2O vs. 6 cmH2O, p = 0.141) were similar, but peak pressure (22 cmH2O vs. 24 cmH2O, p = 0.011) was lower in 2016. More patients received sedatives (70.7% vs. 57.0%, p = 0.002) and analgesics (86.5% vs. 51.1%, p < 0.001) in 2016. The awakening (48.4% vs. 31.0%, p = 0.002) was more frequently attempted in 2016. The accidental extubation rate decreased to one tenth of what it was in 2010 (1.1% vs. 10.2%, p < 0.001). The ICU mortality did not change (31.4% vs. 35.6%, p = 0.343) but ICU length of stay showed a decreasing trend (9 days vs. 10 days, p = 0.054) in 2016. Conclusions: There were temporal changes in care of patients on mechanical ventilation including better control of pain and agitation, and active attempt of awakening.

      • A pulsating protrusion on temporal area after punch biopsy

        A 62 year old man visited us for pulsating protrusion with 1x1.1cm size on R. temporal area. He had taken 4mmpunch biopsy on same area under the suspicion of verruca vulgaris 2 weeks ago. In past medical history, he was being treated with Rt. cerebral infarction with warfarin and aspirin. Hematoma was suspected and we decided to wait until it subsides. However, the lesion persisted after 4months, and we performed ultrasonography. It showed mixed-echoic subcutaneous lumen with fistula. Pseudoaneurysm on superficial temporal artery branch was clinically suspected and we planned to incise and ligate the artery. However, the patient wanted close observation and it became pulseless and began to flatten after 5months. The lesion spontaneously subsided and nofurther recurrence is seen to this day. Although uncommon, vascular complications may occur after punch biopsy. These include post-biopsy bleeding, hematoma and rarely pseudoaneurysm. Pseudoaneurysm is well-known sequela of iatrogenic injury and consists of an organized hematoma that communicates with the injured artery via a sinus tract. Suture ligation of the implicated artery is considered the treatment of choice. Here, we report a case of pseudoaneurysm occurred after punch biopsy on forehead with spontaneous remission for educational meaning, and dermatologists should keep in mind that this rare complication can be encountered even by small punch biopsy.

      • In vitro and in vivo photodynamic therapy of otitis media in gerbils.

        <P>OBJECTIVES/HYPOTHESIS: The aim of this study was to evaluate the antibacterial effects of photodynamic therapy (PDT) on common bacteria causing otitis media with effusion (OME). METHODS: An in vitro study was carried out using a hematoporphyrin derivative sensitizer (Photogem; Lemonosov Institute of Fine Chemical, Moscow, Russia) and a 632-nm diode laser on Haemophilus influenzae, Moraxella catarrhalis, and Streptococcus pneumoniae. The presence of colony-forming units of the bacteria was examined, the microscopic structures of the bacteria were examined by transmission electron microscopy (TEM), and flow cytometry of the bacteria was performed. An in vivo PDT study was performed using gerbils. S. pneumoniae or H. influenzae were injected into bullae. The Photogem was injected into the bullae 2 days later when OME developed, and transcanal irradiation with the 632-nm diode laser (90 J) was performed. Middle ear and bulla were washed with Dulbecco's phosphate buffered saline (DPBS) and the washed DPBS was cultured. The presence of bacterial colonies was examined. RESULTS: The PDT was effective in killing all three kinds of bacteria. TEM showed damaged bacterial cell membranes and cytoplasmic structures, and the flow cytometry showed a lower number of viable bacteria in the PDT group compared to the control group. PDT was effective in killing S. pneumoniae in 87% of the infected bullae with OME, whereas it was effective in eradicating H. influenzae in 50% of the infected bullae with OME. CONCLUSIONS: The results of these studies demonstrated that PDT may be effective to treat otitis media. PDT may have clinical implications in the treatment of otitis media that is resistant to antibiotic therapy.</P>

        경구용 백신수송체용 GFP 함유 마이크로스피어의 제조 및 평가

        In order to design the oral vaccine delivery system, we prepared the alginate microspheres containing GFP (green fluorescent protein) as a model drug by spray method. To optimize the preparation conditions of microspheres, we investigated the effects of various parameters including nozzle pressure, nozzle opening angle, and concentrations of sodium alginate and calcium chloride. The prepared microspheres were evaluated by measuring their sizes, loading efficiency, and morphology. The particle size of microspheres was affected by the concentration of sodium alginate and calcium chloride, nozzle pressure, and nozzle opening angle. As the concentration of sodium alginate increased, GFP loading efficiency and particles size of microsphere also increased. However, it was observed to be difficult to spray the sodium alginate solution with concentration greater than 1.5% (w/v), due to high viscosity. The pressure over 3 kgf/㎠ didn`t affect the size of particles. As a result, the spraying method enabled us to prepare microspheres for oral vaccine delivery system. In this study, microspheres prepared with 1% (w/v) sodium alginate had greater loading efficiency and better spherical shape.

      • Power System Real-Time Event Detection and Associated Data Archival Reduction Based on Synchrophasors

        <P>The aim of this paper is to present methods on real-time event detection and data archival reduction based on synchrophasor data produced by phasor measurement unit (PMU). Event detection is performed with principal component analysis and a second order difference method with a hierarchical framework for the event notification strategy on a small-scale microgrid. Compared with the existing methods, the proposed method is more practical and efficient in the combined use of event detection and data archival reduction. The proposed method on data reduction, which is an “event oriented auto-adjustable sliding window method,” implements a curve fitting algorithm with a weighted exponential function-based variable sliding window accommodating different event types. It works efficiently with minimal loss in data information especially around detected events. The performance of the proposed method is shown on actual PMU data from the Illinois Institute of Technology campus microgrid, thus successfully improving the situational awareness of the campus power system network.</P>

      • An Event-Oriented Method for Online Load Modeling Based on Synchrophasor Data

        <P>The objective of this paper is to present a new “event-oriented” method of online load modeling for the Illinois Institute of Technology (IIT) microgrid based on synchrophasor data produced by phasor measurement units. Several load models and their parameter estimation methods are proposed. The online load modeling process is based on an adjustable sliding window applied to two different types of load step changes. The load modeling tests and related analysis of the synchrophasor data from the IIT microgrid are demonstrated.</P>
